Registration Data:

" Full legal name and social security number:Many Scouts go by a “nickname” or other name than their birthname; for

instance, a Scout may go by “Skip” since birth but his legal name is Martin;when a potential employer attempts to verify Skip’s Eagle Scout status aslisted in Skip’s resume, the National Eagle Scout Service will not be able toimmediately verify that this is indeed the same “Martin” whom now callshimself “Skip.” This is why the application requests both full legal nameand social security number of the Eagle Scout candidate.

If a family or Scout chooses not to place the social security number in thefield indicated, either the Council Scout Executive or the Scout may explainthe absence as an attachment to this application.

Registration Data:The Scout should complete ONLY those lines which apply to his particular

registration status. A Scout does NOT have to become a Varsity Scout norVenturer to meet the requirements for Eagle. Likewise, a Venturer does NOThave to be a registered Boy Scout at the time of application.

" I joined a Boy Scout Troop on: (month)_____ (day)____ (year)____

" I became a Varsity Scout on: (month)_____ (day)____ (year)____

" I became a Venturer on: (month)_____ (day)____ (year)____

(“Venturer” includes Sea Scouting as currently defined by the Venturing Division,Boy Scouts of America. If the Scout is a member of a Sea Scout Ship, heplaces the date of his membership in that Ship in the above line.)

Registration Data:! A boy registered in a Boy Scout Troop or Varsity Scout Team may earn Eagle by first

earning Tenderfoot, Second Class, First Class, Star, and Life Scout ranks in either theTroop or Team setting. In order for a Venturer to become an Eagle Scout, he must havepreviously earned the Tenderfoot, Second Class and First Class ranks as a Boy or VarsityScout. A boy registered in either a Troop or Varsity Scout Team AND in a Venturing unitmay earn the Star, Life and/or Eagle ranks in either unit. A boy registered solely in aVenturing unit may earn the Star, Life, and Eagle ranks only as a member of thatVenturing unit.

The Scout should either find and copy the date listed on his First Class and Star Awardrecognition (pocket) cards, verify the following through official records from his Boy ScoutTroop, Varsity Scout Team, Venturer Crew or Sea Scout Ship, or verify the followingthrough official records maintained at the local Council office in which he was a member of(the date of the Board of Review is the date which should be also on the card orwithin the database or advancement report):

Registration Data:These next questions are solely to provide additional information to the members

of your Eagle Scout Board of Review, the BSA’s National Court of Honor, andthe BSA’s Registration and Statistical Service. If you are NOT SURE about theanswers to these questions, consult your parents or the official records of yourunit which may have this information:

! Were you a Cub Scout? ! YES ! NO

! Were you a WEBELOS (Cub) Scout? ! YES ! NO

! Did you earn the Arrow of Light Award? ! YES ! NO

(the Arrow of Light Award is Cub Scouting’s highest rank and is the only rank insignia from

Cub Scouting which can be worn by Boy/Varsity Scouts and Venturers. The Award lookslike the one illustrated at the left. The WEBELOS Award nor the Compass Points badge arethe “Arrow of Light”. The Order of the Arrow (OA) is likewise NOT the “Arrow of Light.”)

! Had you complete the fifth grade upon becoming a Boy Scout?

! YES ! NO

Registration Data:These final two questions are perhaps two of the most important questions asked on the

application. You must complete ALL requirements for the Eagle Scout Award lessparticipation in an Eagle Board of Review BEFORE your 18th birthday. Boy Scoutadvancement ends on the 18th birthdate of the Scout. The Board of Review can take placewithin three months of the Scout's 18th birthday with no explanation. After that, LocalCouncils have another three month “grace period” in which to conduct an Eagle ScoutBoard of Review for the Scout or Venturer, but a statement by an adult must be attachedto the application explaining the delay. A Board of Review more than six months after themember’s 18th birthday may be held under exceptional circumstances only withpermission from the National Boy Scout Program Division.

A Scout or Venturer must be active in his unit for at least six months after the awarding ofthe Life Scout rank; the six months begin with the successful completion of the Board ofReview for the Life Scout rank. Verification comes from the date listed on the Life Scoutrecognition (pocket) card, or from official records of the unit or the local Council.

! I was born ____ (month) ____ (day) _____ (year)

! I became a Life Scout _____ (month) _____ (day) _____ (year)

You may choose to list individuals younger than 21 or list individualsresiding outside the community in which you reside. They must beable to be reached either by telephone, fax, electronic mail, or USPostal mail to provide the Board of Review with appropriatecharacter information on the Eagle Scout candidate.

Merit Badges:! 21 merit badges, including 12 from a listing of required merit

badges and their alternates, are required to qualify for theEagle Scout Badge. List each merit badge with the date thatthe badge was earned and the unit number in which thebadge was earned in or as a part of. The best way to verifythe badges earned is with the merit badge certificate (card)presented to the Scout; the second best way to verify is bymatching up the dates earned here with the unit or localCouncil copies of the BSA Advancement Report form.Alternative ways include checking against computerized ormanual recordkeeping systems and checkoffs in the Scouts’Boy Scout Handbook.

Theothers, if earned, may be used as an elective merit badge ifdesired by the Scout. The SCOUT OR VENTURER, nobody else, determines which badges will be used to meet the electivemerit badge requirements for Eagle.

Go back and double-check all names, dates, and unit numbers of each merit badgelisted. Also cross-check the dates against the certificate cards or othermethods of date checking. You must list all merit badges you used to advanceto Star and Life. Your Eagle application must show at least 4 merit badges fromthe 15 on the required list, (and at least 6 merit badges total), earned beforethe Star Board of Review date, and at least 7 merit badges from the 15 onthe required list (and at least 11 merit badges total) earned before the LifeBoard of Review date. All 11 merit badges used to earn Star and Life mustbe listed on the application.

The merit badge part of the application is VERY important, as the local Council andthe BSA’s Eagle Scout Service DOES perform some checking and verification ofthe dates and unit in which the merit badge was earned. Errors cause delaysin the application process and in some cases, forces the application back to thelocal Council for clarification.

You are now ready to go to the next part of the application: the Unit - levelLeadership portion.

The dates of service must total atleast 6 months, and must start on or after the date of theLife Board of Review. The date used for the Life Scoutadvancement MUST match the previous date listed (seeslide 9). Additionally, many Scouts and Ventures get theirapplication rejected because their leadership experiencewas less than six months in length. helped you toward that goal (or not). This statement will be read by themembers of your Eagle Board of Review as well as by the members of the BSA’sNational Court of Honor in reviewing your application for the Eagle Scout Badge.Try to be brief as possible, but if you run out of space, use a separate sheet ofpaper and explain your life’s intentions. Also list on that separate sheet of paperany community, club or athletic awards or honors -- and don’t forget thoseadditional leadership opportunities you have participated in such as Chapter orLodge Order of the Arrow Chief, camporee youth staff, and summer camp staffmember.

BSA to Boy Scouts and Varsity Scouts; and is the only Boy ScoutDivision advancement in which male Venturers and Sea Scoutsmay earn (of course, Ventures and Sea Scouts of both sexes mayearn the Silver, Ranger and Quartermaster ranks and ranksleading to those advancements). It is the most respected awardin this nation and those Scouts whom have earned it have met aset of demanding requirements which over the years has notbeen “watered down”. The number of Scouts earning Eagle arefew -- a few more today than before -- but still a very small poolof potential leaders and problem solvers as we move toward thenew century.

Get busy, find those dates and data, fill in these worksheets andcomplete the application -- America NEEDS LEADERS LIKE YOU!
