Use this form to order DVDs of the Loyola production of Shrek The Musical (DVD includes the Senior farewell at the beginning of Act 2). Suggested donation for each DVD is $10.00. The donation should accompany this order form. Checks can be made out to Loyola Catholic School. Please return the form to the High School Office by December 16 for delivery before Christmas.
