Caso clínico Paciente de 54 años con antecedentes de HTA que presenta dolor pélvico asociado a alteración del ritmo intestinal, anorexia y pérdida de 12 Kg de peso en 1 año. En colonoscopia, neoformación que obstruye la luz a 28 cm con biopsia de adenocarcinoma. CEA 5.5 ng/ml. TAC: metástasis hepáticas múltiples bilaterales.
Se intervino quirúrgicamente mediante hemicolectomía izquierda. AP: Adenocarcinoma bien diferenciado T3N2 (8/19 g).
Se remite a Oncología Médica. KRAS: Mutación Gly13Asp

Evolución del caso clínico El paciente se incluyó en estudio MACRO, correspondiéndole la rama de XELOX + Bevacizumabde mantenimiento. Obtuvo una respuesta parcial mantenida durante 17 meses.
Se suspendió oxaliplatino por neuropatía al 5º mes y capecitabina por tox. GI tras 15 meses.
No hubo toxicidad atribuible a Bevacizumab A la progresión de la enfermedad…

Caso clínico: Evolución del paciente
Inclusión en estudio TML 18147 Recibió 2ª línea con FOLFIRI + Bevacizumab Alcanzó Enfermedad Estable como máxima respuesta Recibió tratamiento durante 8 meses, momento en que progresó durante el tratamiento (SLP 8m)
Se incluyó en ensayo CORRECT (Regorafenib vs Placebo en 3ª línea).
Falleció por progresión de la enfermedad a los 32 meses de iniciada la primera línea.
