Conclusiones de los autores:
• La Cmin de ATV es mayor al administrar ATV (300 mg QD) + LPV/RTV (400/100 mg BID) que con ATV + RTV (300 + 100 mg QD). La AUC0-24h y la Cmax de ATV son iguales.
• Al añadir otros 100 mg de RTV (Período III) las concentraciones de ATV no se modifica. La Cmin de LPV aumenta, sin cambios en la AUC0-12h y la Cmax.
• La combinación ATV+LPV/RTV se toleró bien y proporcionó perfiles PK favorables de ambos IP potenciados.

• Acid reducing agents do not significantly alter LPV or RTV exposure after co-administration with LPV/r 400/100 mg BID or 800/200 mg QD dosed as the tablet formulation.
• The bioavailability of ATV when co-administered with RTV is signif. decreased 48 to 62% by acid reducing agents. The decrease was similar with either OME or RAN administration and contrasts a previous report where famotidine decreased ATV exposure only 14 to 18% following 10 days of simultaneous administration.

Nota: La absorción ATV depende de pH gástrico. En el estudio indicado famotidina y ATV se administraban simultáneamente y la disminución del pH es menor que si se administra 1-2 horas antes. En estudios previos si ATV-FAM se separan 10-12 horas no hay interacción.

Conclusions• In this cohort the majority achieved virological suppression (86%) at the time of TDM with standard LPV/r dosing. In those not virologically suppressed significant adherence issues were identified.
• In pregnancy, the physiological alterations in drug disposition mean that the measured plasma levels may not accurately reflect the levels of free active drug. Therefore plasma drug levels must be interpreted with caution. Nonetheless, the majority had adequate lopinavir trough plasma levels (95%) in the third trimester.
• In our practice use of LPV/r at standard dosing, in the absence of adherence issues, is associated with good virological response and adequate plasma levels.

• NFV Cmin was below the recommended trough level (i.e., 1 mg/L) in 18 patients (45%).
• This result did not alter the median decline in HIV plasma viral load, which was -1.70 log10 after a median time of 15 days, although a trend towards lower decline in HIV plasma viral load was observed in women with low trough level (-1.49 versus -1.85 log10, p 0.08).

• El Cociente Inhibitorio es un importante factor de respuesta en el tratamiento de rescate con IPs/r (TMC114, FPV)
• LPV/r puede disminuir el aclaramiento renal de TDF
• Omepazol y ranitidina no interaccionan con la nueva formulación de LPV/r. Interacción ya conocida con ATV/r

Resumen/Conclusiones
• Rosuvastatina (estatina no metabolizada por CYP450) + LPV/r : Ctrough ROS (x 1,5-1,9). Eficacia y buena tolerabilidad
• EFV- Carbamacepina: Interacción bilate-ral (relevancia clínica para CBZ).
• Embarazo: conc. de IPs. No se aclara conducta a seguir: ¿ dosis, TDM, nada?
• TDM: no universal. Definir subgrupos de riesgo que se beneficien de TDM
