Donors Report

Cataract Surgery Camp conducted by The Vision Mission Singapore, in collaboration with Trilochan

Netralaya in November 2014 at location in Sambalpur, Odisha, India.

EXECUTIVE SUMMARY

Sambalpur is a remote location in the state of Odisha in India. In February 2014,

The Vision Mission was involved in a camp in this location to conduct 300 free

cataract surgeries across 3 camps. Following up from our very successful project,

we decided on doing 2 more mission trips this year, and try and restore eye-sight to

another 700 of the poorest. The first of the two trips was organized in November

2014, with 2 camps during which we conducted 350 free cataract surgeries for the

poorest people of the region. The second of these trips would be in December 2014

with a target of another 350 surgeries, thereby taking our total for the year to 1000.

About The Vision Mission

The Vision Mission is a Singapore-based non-profit organization started by doctors

and professionals, with the purpose of providing free eye care to those in need in

Southeast and South Asia. We seek to serve as a conduit enabling transnational

collaboration of dedicated ophthalmic experts in providing free high quality eye care

and surgery to patients in areas of need, with a view to eradicate treatable vision

impairment and allow eye care access to all. We wish to set up and operate

sustainable eye care programs focusing on treating cataracts in areas of need, in

collaboration with local ophthalmologists

About Trilochan Netralaya

Trilochan Netralaya (TN) was founded by Dr Shivaprasad Sahoo. TN and Dr

Shivaprasad have worked tirelessly and selflessly over the years in trying to

eradicate curable blindness in the region. TN has existing camps in Sambalpur and

Kalahandi, and therefore also possesses the methods to bring in people for camps.

In TN, The Vision Mission has found an ideal complementary partner for achieving

its objective.

About Sambalpur, and the need there

Sambalpur is a remote location in the state of Odisha in India. Odisha (formerly

Orissa) is among the poorer states in India. Odisha is home to a large tribal

populace, or Adivasis (known officially as Scheduled Tribes in India), and

Scheduled Castes. The western districts such as Kalahandi have a majority

population of SC/ST. Typically, the districts and its people tend to be the poorest,

with very little access to healthcare.

MISSION TRIP DETAILS

Following from our successful trip in February 2014, we decided to do more

projects in Odisha to support the wonderful team at Trilochan Netralaya. We also

decided to go for a target of 1000 cataract surgeries over 2014, over 2 more projects

in November and December of 2014.

Our Last Trip in February 2014

In February 2014, we conducted 3 camps in Sambalpur and Bhawanipatna

(Kalahandi district) in Odisha State in India. We funded and conducted 300

cataract surgeries for the poorest. We also worked on complex cases for which the

local doctors wanted second opinions - from various subspecialties such as neuro-

ophthalmology, pediatric ophthalmology, cornea, glaucoma, uveitis and retina.

This Trip

We had 2 camp sites in Sambalpur

between 21-Nov-2014 to 30-Nov-2014.

The first location was Betty’s Ashram –

a quaint and remote facility a little off

Sambalpur town. The second location

was at TN’s hospital in Sambalur.

Across the 2 camps, we conducted 350

free cataract surgeries along with

helping with many other items.

A team of 3 comprising of Dr Chen Jianping, Avinash Jayaraman and Mohamed

Farook made the trip down from Singapore. The journey to Sambalpur was made

via a flight from Singapore to Kolkata, from where they took a train to Sambalpur.

The Objectives of this Mission Trip included:

To conduct 350 cataract surgeries using the Small Incision Cataract Surgery technique (SICS)

To support the doctors in Sambalpur in their initiatives

To follow up from our last trip as and how required

To follow up on our set up of subspecialty services in glaucoma and retina

To follow up on our set up of Tele-medicine system

To train a team of optometrists

The Next Trip

The Next trip is to be undertaken from 13th December to 20th December 2014. A

team of extremely gifted and experienced surgeons from Singapore – Dr Rupesh

Agrawal, Consultant Ophthalmologist at National Healthcare Group Eye Institute at

Tan Tock Seng Hospital; Adj Assoc Prof Shamira Perera who is Senior Consultant

at the Singapore National Eye Centre; Arun Kumar Narayanaswamy who is with

the SNEC & Singapore Eye Research Institute.

HOW WE HAVE HELPED

Free Cataract Surgeries

350 free cataract surgeries were

conducted in Sambalpur through the

camps funded by The Vision Mission,

and its benefactors. We present some

specific cases studies in the page to

follow. These people in Sambalpur spent

years of their life suffering without the

benefit of proper sight, because they did

not have the equivalent of $30 to cure themselves. They can now see! The donors

money helped fund not just the surgery, but also the following:

- Travel costs for bring the patients to the camp, and then back to their village

- Stay and Accommodation – for the purpose of rest and recuperation. Given

that the villages are a 100 kms away, it would not be possible for the

patients to go back home and them return 3 days later for their Post-Op

checkup. Therefore, facilities need to be in place to ensure they can stay at

the camp.

- Food – during their stay at the camp

- Lenses

- Cost of nurses and other staff

- Cost of equipment, medicines

Some case studies in the following page:

Sapura Bhoi: lives alone in the village of Salesing.

With her husband no more, and a daughter married

off and living far away, Sapura lives off on the Rs 300

pension provided by the government. She has been

blind in both eyes due to cataracts for over 5 years.

Needless to say, fending for herself would have not

been easy. Sapura’s story might sound grim, but her

spirit is far from that. Sapura will come back in 3

months time to get her next eye fixed.

Gopal: Little Gopal is aged 8, and suffered from

bilateral congenital cataracts. The team led by Dr

Shiva operated on him. Gopal is a vivacious young

lad and readily engaged in banter with the staff

whilst on the operating table. Gopal likes school,

gave us an impromptu rendition of ABCD, and spoke

of how he gets served eggs two times a week on the

mid-day meal scheme.

Ramba Sahoo: has no children or husband. She has

been alone, and living with her nephew. She too has

been blind on both eyes. She came over to the camp

to get her right eye operated. She spoke about how

she felt great to be finally able to see again. Ramba

was extremely thankful for the free surgery camp

without which she had no hope.

Neura: Baby Neura is Gopal’s (see above) younger

brother. Just like Gopal, Neura is unable to see on

both eyes. Given that Neura is just above a year old,

the operation needed to be performed under general

anaesthesia. The operation took about 30 minutes on

the whole to do, and was successful. Neura will be

able to see again, and will not be condemned to a

lifetime of blindness.

Outpatients and Other

Ophthalmic Assistance

Dr Shiva also has an outpatient

clinic facility where patients with a

wide range of conditions are seen by

himself and fellow doctors. However,

there are many such cases where

they need expert assistance. We

were able to provide clinical input on

some of their difficult cases, and

refer others over to hospitals and

doctors in Bhubaneswar and Singapore.

Tele-Ophthalmology Setup

We helped in setting up a Tele-Ophthalmology unit using the Forus 3nethra system

at TN. With this, regardless of their location, doctors and technicians will be able to

take high definition photographs of a patient's anterior and posterior segments,

and subsequently upload these images onto a cloud server. From there, designated

foreign Ophthalmologists will receive this report and be able to give their expert

opinion. The idea through this is to help TN get assistance for patients at any point

in time, and not have to wait for the next team to come on a mission trip.

Optometry and Refraction Training

Mohamed Farook held a 5 day training session for paramedical personnel at TN in

vision examination and refraction. It

was a huge task as we had less than

a week of contact time. The biggest

challenge was the language barrier

as the students spoke very basic

English, if at all. Nevertheless, the

five of them were very enthusiastic.

As they had some exposure in vision

examination, they understood basic

principles by end of the training

session.

Jianping has always

wanted to be involved

in humanitarian

work, even before he

entered medical

school, and therefore

seized this chance to

volunteer for this

mission trip.

Mohamed Farook is a

Senior Optometrist at

Singapore National Eye

Centre (SNEC). Farook

has been a regular

volunteer at various

mission trips. Here in

Sambalpur, he is

training up an eager

batch of future

optometrists and is

bringing in global best

practises in the field.

Avinash Jayaraman

is a co-founder of The

Vision Mission.

Avinash was also

involved in TVM’s

maiden trip to

Sambalpur in

February this year. He

has been handling

documentation of the

trip and its activities,

with photographs,

videos and write-ups.

Dr Shivaprasad Sahoo is the founder

and chief surgeon at Trilochan

Netralaya. He has worked tirelessly and

selflessly over the years in trying to

eradicate curable blindness in the

region.

FUTURE PROJECTS

Over a few rounds of discussions with Dr Shiva, and other people on the ground,

we worked out the following possible future initiatives:

(1) The Providing sub-speciality expertise. Sambalpur still needs to have regular

visits by sub-speciality ophthalmologists to help deal with specific and

unique cases. This includes doctors having expertise in glaucoma and

phacoemulsification.

(2) Get a glaucoma training program started. Adj Assoc Prof Shamira Perera, a

Senior Consultant in the glaucoma department of SNEC and Dr Arun

Narayanaswamy who has many years of experience in the subspecialty

through his work in Sankara Nethralaya and SNEC, will guide Dr Shiva

through glaucoma clinical and surgical management principles. Next year we

intend to arrange a local fellowship program in Glaucoma for the local TN

team in glaucoma.

(3) Funding a further 1000-2000 surgeries per year. We started this year with

300, and scaled it up to 1000. Moving ahead we hope to do 2000 for the year

2015, to ensure the good work does not stop, and to ensure that we can

forever eradicate curable blindness from the region.
