pages with entries certified by the Corporate

Secretary)6. Copy of Certificate of Registration/Latest

Certificate of Amendment if there is a change

in Corporate name7. Proof of Operation (any two of the

a. Income Tax Returnb. Mayors Permit

c. Certificate of Franchise issued in favor of

Petitioner corporationd. News Clippings of activities undertaken by

the corporation within one (1) year priorto the filing of the Petition

e. Contracts entered into by the corporation

within one (1) year prior to the filing of
