EXECUTIVE SUMMARY
Information for five retrieval campaigns was reviewed and events which caused either retrieval
shut-down or delay were noted by equipment type. Once documented, the findings were
evaluated to identify areas with repeated events. The information was obtained from the
Retrieval and Closure Operational Engineer Log Books, the Base Operations Log Books, the
retrieval completion reports, and the notes recorded by the Process Engineers in the material
balance calculation spreadsheets. Several hundred Problem Evaluation Requests, also known as
PERS, were reviewed and determined to not provide new pertinent information. The Document
Management Control System database was searched for lessons learned reports. No lessons
learned reports were found for the campaigns being evaluated.
There have been three distinct types of retrieval systems used in 241-C Tank Farm retrieval
efforts: the Modified Sluicing Waste Retrieval System, the Mobile Arm Retrieval System –
Sluicing, and Mobile Arm Retrieval System – Vacuum. The campaigns evaluated in this report
used either the Waste Retrieval System or the Mobile Arm Retrieval System – Sluicing. The
five campaigns evaluated are the 241-C-101 bulk retrieval campaign the 241-C-107 retrieval
campaign, 241-C-111 bulk retrieval campaign, the 241-C-112 bulk retrieval campaign, and
241-C-112 hard heel retrieval campaign.
A total of 487 events occurring over 390 operating days were documented. These events were
sorted into a number of categories as detailed in Section 3.0, “Methodology.” Analysis of these
events resulted in nine specific recommendations for areas needing improvement or further
review, as detailed in Section 5.0, “Recommendations.”

1.0 INTRODUCTION
This document reviews information for five bulk and hard heel retrieval (HHR) campaigns to
identify areas for equipment and operational process improvement. The evaluation addresses
both equipment reliability and availability.
2.0 SCOPE
Information for five retrieval campaigns was reviewed and events which caused either retrieval

2.1 EVALUATED RETRIEVAL SYSTEMS
There have been three distinct types of retrieval systems used in 241-C Tank Farm retrieval
efforts: the Modified Sluicing Waste Retrieval System (WRS), the Mobile Arm Retrieval System
(MARS) – Sluicing, and MARS – Vacuum. The five campaigns evaluated in this report used
either the WRS or the MARS – Sluicing.
There are three styles of sluicers used in the sluicer-based retrieval systems: the standard sluicer,
the Extended Reach Sluicer System (ERSS) sluicer, and the arm-mounted sluicer used with the
MARS. All of the sluicers are hydraulically driven. The standard sluicer is capable of pan and
tilt movement. The ERSS consists of an extendable boom-mounted sluicer capable of pan and
tilt movement. The boom allows the ERSS sluicer nozzle to be placed closer to the waste
therefore increasing the sluicing effectiveness in breaking up solid waste in the single-shell tank
(SST). The ERSS are also equipped with two high-pressure water nozzles located on either side
of the sluicing nozzle.
2.1.1 Modified Sluicing Waste Retrieval System
A WRS includes three main components in the SST being retrieved and two in the receiving
double-shell tank (DST). The three SST components are two sluicers and a slurry pump. The
DST components are a slurry distributor, and a supernatant pump. The sluicers are generally
placed in risers toward the outside of the SST and the slurry pump located in the center. In the
DST, the slurry distributor and supernatant pump are co-located in the central pump pit. The
slurry distributor is a passive device which is manually raised or lowered. The supernatant pump
may be either electrically or hydraulically driven while the slurry pump is hydraulically driven.
During retrieval, the slurry pump is lowered as the SST waste level lowers. The supernatant

pump is raised as the solids level in the DST increases. The slurry distributor is raised and
lowered as necessary to minimize the amount of solids in suspension and therefore returned to
the SST in the supernate.
Hose-in-hose-transfer lines (HIHTLs) connect the two tanks via a portable valve pit and a
portable diversion box. Other ancillary equipment required for a WRS-based retrieval include
active ventilation systems on both the SST and DST tanks, leak detection, and in-tank cameras
and associated lighting systems.
2.1.2 Mobile Arm Retrieval System – Sluicing
A MARS includes two main components in the SST being retrieved and two in the receiving
DST. The two SST components are an arm-mounted sluicer and a slurry pump. The DST
components are a slurry distributor and a supernatant pump. The sluicer arm and slurry pump
are generally placed in risers located in the center of the SST. In the DST, the slurry distributor

2.2 EVALUATED CAMPAIGNS
The retrieval campaigns evaluated are discussed in the following sections. The campaigns were
selected based on their use of MARS, standard, and ERSS sluicer technology, and their use of
diverse media to mobilize the waste.
2.2.1 241-C-101 Bulk Retrieval
Tank 241-C-101 was retrieved using a WRS with ERSS sluicers. Retrieval occurred from
December 2012 until September 2013 with the following non-equipment failure-related
interruptions:
Resources were diverted from the 241-C-101 retrieval to support the 241-C-107 retrieval
on March 19, 2013, and were not available until May 14, 2013;
Resources were diverted to support the 241-C-107 retrieval efforts again from May 18,
2013, until June 2, 2013;

Delays for installation and testing of high-pressure water support equipment occurred
from June 20, 2013, to July 8, 2013, and from September 3, 2013, to September 5, 2013;
and
An event investigation interrupted retrieval from August 22, 2013, until September 3,
2013.
The sluicing media used for the more readily retrieval solids was supernate from 241-AN-101.
High pressure water was the media used for larger pieces of hard waste that could not be broken
up by typical sluicing methods.

April 2014 with intermittent interruptions to support 241-C-104 and 241-C-109 retrieval efforts.
The sluicing media used for the more readily retrievable solids was supernate from 241-AN-106.
High pressure water was the media used for larger pieces of hard waste that could not be broken
up by typical sluicing methods.

2.2.3 241-C-111 Bulk Retrieval
Tank 241-C-111 was retrieved using a WRS with standard sluicers. Retrieval occurred from
September 2010 until November 2010.
The sluicing media used was supernate from 241-AN-101.

2.2.4 241-C-112 Bulk Retrieval

2.2.5 241-C-112 Hard Heel Retrieval
Tank 241-C-112 HHR used a WRS with one standard sluicer and one ERSS sluicer. Retrieval
occurred from November 2013 until January 2014.
Concentrated 19.4 M sodium hydroxide was used during the two soaking and recirculation
periods. The sluicing media used for the majority of the retrieval was supernate from
241-AN-101. Hot water was used at the end to allow for dissolution of any water soluble salts
remaining in the tank.

2.3 EVALUATED EQUIPMENT
Equipment included in the evaluation can be separated into two categories: SST-specific
equipment and shared equipment. The shared equipment includes the SST ventilation systems,
all of the DST equipment, the majority of the HIHTLs, the valve pits, the diversion boxes, and
control trailers.
The 241-C Tank Farm SSTs were retrieved to either 241-AN-101 or to 241-AN-106. The shared
equipment associated with the 241-AN-101 receiver tank is:
The POR137-WT-TRLR-001 control trailer,
The POR138-WT-VP-001 valve pit,
The POR134-WT-DB-001 diversion pit,
The 241-AN-101 transfer pump, and
The HIHTL which connects them.
Four of the campaigns evaluated were retrieved to 241-AN-101. These are the 241-C-101,
241-C-111, and both of the 241-C-112 retrievals. In addition to these tanks, 241-C-102 and
241-C-104 used 241-AN-101 as their receiver tank.
The shared equipment associated with the 241-AN-106 receiver tank is:
The POR104-WT-VP-001 valve pit,
The POR209-WT-DB-001 diversion pit,
The 241-AN-106 transfer pump, and
The HIHTL which connects them.

The 241-C-107 retrieval utilized 241-AN-106 as its receiver tank. MARS activities have a
dedicated control trailer, POR246-WT-TRLR-001. Other tanks which used 241-AN-106 as their
receiver tank include 241-C-103, 241-C-106, 241-C-108, 241-C-109, and 241-C-110. These
tanks used WRS for retrieval and used a WRS-dedicated control trailer.
All seven of the 241-AN Tank Farm tanks use the same primary tank ventilation system, the AN
ventilation tank primary (VTP). DSTs are required to maintain active ventilation and are
connected to the ventilation systems continuously. The 16 SSTs in the 241-C Tank Farm only
require active ventilation during retrieval activities and are usually passively ventilated. There
are two main ventilation systems in 241-C Tank Farm, POR008 and POR107. POR107 has been
connected to POR03 in order to increase the exhaust flow rate provided by the system. Many of
the SSTs in 241-C may be connected to either the POR008 ventilation system or to the
POR107/POR03 ventilation system.
Each of the SSTs includes sluicers, either standard, ERSS, or arm-mounted, and adjustable
height slurry pumps. In the WRS configuration, each of the sluicers is equipped with a hydraulic
power unit (HPU). Sluicer functions are controlled via a hydraulic valve manifold. The MARS
arm-mounted sluicer is also hydraulically driven and controlled with hydraulic valves. The
slurry pumps also are equipped with an HPU.
The DSTs each have an adjustable height slurry distributor as well as an adjustable height
supernatant pump. The pumps used in 241-AN-101 are hydraulically driven and as such are
equipped with an HPU. Tank 241-AN-106 used both electrically driven and hydraulically driven
pumps during the retrieval campaigns evaluated.
Sluicing activities are conducted using in-tank cameras, two per SST, which allows the operator
to determine the retrieval progress and direct the sluicer nozzle. These are portable camera units
with a built in light. In addition to the camera light, additional in-tank lighting is provided for
each camera.
Water at 241-C Tank Farm is provided from a common source, POR135.
These components and the instrumentation monitoring them were included in this evaluation.
3.0 METHODOLOGY
The information contained in Appendices A through E document a total of 487 events occurring
over 390 operating days. These events were sorted into a number of categories which were
suggested by the development of Appendices A through E. These categories are:
Hydraulic leak events,
Ventilation events,
Camera and lighting events,
Supernatant pump start-up and shut-down events,
Supernatant pump flow rate events,
Slurry pump flow rate events,

Slurry pump movement events,
Sluicer / ERSS / arm movement events,
Hydraulic temperature events, and
Other miscellaneous events.
Tables documenting the events by day and category for each retrieval campaign are contained in
Appendices F through J. Events were tallied in each category for each day. If a particular event
happened multiple times on a given day, it was counted as a single instance since it was likely
reoccurring due to not having completely resolved the initiating event. If a new initiating event
occurred on the same day, it counted as a new event in the tally. For example, if the ventilation
system was shut-down due to a continuous air monitor (CAM), it counted as a single event, even
if the ventilation system was shut down multiple times for CAM problems on that day. If there
were also seal pot issues on the same day, it would count as a separate event on that day.
Event categorizations by category over time for the entirety of the campaigns reviewed are
included in Appendix K.
4.0 FINDINGS
4.1 SIGNIFICANT DELAY EQUIPMENT FAILURES
There were four equipment failures which required component replacements resulting in
significant interruptions in the retrieval process. These failures were:
The AN106-WT-P-22 electric pump which failed during retrieval activities;
The AN106-WT-P-23 electric pump which failed during pre-installation testing;
The AN106-WT-P-24 hydraulic pump which failed during retrieval activities; and
The 241-C-107 slurry pump which failed during retrieval activities.
Retrieval activities did not resume for 208 days after the failure of the AN106-WT-P-22 pump
and subsequent failure of the AN106-WT-P-23 pump during testing. After the AN106-WT-P-24
pump failure, retrievals did not resume for an additional 241 days. Retrievals resumed 198 days
after the failure of the 241-C-107 pump. All of the pump failures were associated with the
241-C-107 MARS retrieval efforts. Given that the other retrieval efforts associated with the
241-AN-106 receiving tank are not within the scope of this review, it is not possible to evaluate
the total number of hours of pump use prior to failure.
No other failures were noted which resulted in a sustained suspension of retrieval activities.
4.2 MINOR DELAY EQUIPMENT FAILURES
The vast majority of failures resulted in short duration shutdowns or reduced capacity during
continued operations. The typical short duration shutdown impact was less than one shift with a
few extending into one or two days while the equipment was undergoing troubleshooting. The
other failures included here typically resulted in reduced capacity such as lost mobility in a

sluicer nozzle or reduced flow from a sluicer nozzle. The events are summarized by tank in the
following subsections. Trends and analysis of the findings are discussed in Section 4.3.
4.2.1 241-C-101 Bulk Retrieval
The information documented in Appendix A was sorted into the categories listed in Section 3.0,
“Methodology.” The resulting table is in Appendix F. A total of 110 entries were made
documenting issues over 101 days of operation. Figure 4-1 shows the distribution of the events
between categories. No trends were noted within the 241-C-101 information. Analyses of each
of the categories are summarized in the following subsections.

4.2.1.1. Hydraulic Leak Events. Eight hydraulic leak events were recorded during the
241-C-101 bulk retrieval campaign. Two of these events were associated with the slurry pump
HPU. One event was associated with the supernatant pump HPU. The other five were
associated with the two sluicers.
The slurry pump HPU leaks were at a flange on the HPU. The supernatant pump hydraulic leak
was at the pressure hose connection point of the HPU. All three of these leaks appear to have
been repaired with minimal impact to the retrieval efforts.
One of the sluicer leak entries was associated with the number 1 sluicer. The leak was apparent
during operation of the horizontal movement function. The hose providing the function was

disconnected. Operation of the number 1 sluicer continued utilizing vertical movement of the
nozzle.
The remaining four hydraulic leak events noted were associated with the number 2 sluicer.
Those four entries documented the discovery and effects of the same two failures. The leaks
associated with the number 2 sluicer were at the rotary union. The initial entry identified both
leaks. The horizontal motion function had a drip-wise leak at the rotary union. A steady stream
of hydraulic fluid was observed at the rotary union during vertical motion. Sluicing continued
with the number 2 sluicer using the horizontal motion functions while the hoses to the vertical
motion functions were disconnected. Later investigation attributed these leaks to electro-spark
damage to the Teflon hydraulic hoses as identified in RPP-RPT-55036, “Summary Evaluation of
ERSS Hydraulic Hose Failures”. Corrective actions are in place to address this issue.
4.2.1.2. Ventilation Events. Seventeen entries were logged associated with ventilation issues.
Ten of the events were associated with the POR107 ventilation system, one was related to the
241-C-101 demister, and the remaining six were associated with the 241-AN VTP.
Eight of the 17 entries were associated with the CAMs, 5 on the POR107 ventilation system and
3 on the 241-AN system. Three entries documented the efforts associated with a single event of
the POR107 glycol heater smoking. The five entries note issues with the POR107 seal pot
temperature, the POR107 low flow alarm, the POR107 sample pump, high differential pressure
across the 241-C-101 demister, and entering an extended outage for 241-AN VTP upgrades for
compliance with RPP-13033, “Tank Farms Documented Safety Analysis.” One entry simply
noted that the 241-AN ventilation system was shut down.
4.2.1.3. Camera and Lighting Events. Fourteen entries were logged associated with the in-
tank cameras and lighting systems. Only one of the camera and lighting entries was for the
241-AN-101 camera. The remaining 13 entries were for the 241-C-101 in-tank cameras and
lights.
There were five entries associated with the in-tank lighting, including a stop work on the use of
all Alhberg lighting and three for the cameras and lighting. The camera and lighting installed in
Riser 3 had the majority of entries with replacement being noted ten times. The cameras and
lighting installed Riser 8 required replacement seven times.
4.2.1.4. Supernatant Pump Start-Up and Shut-Down Events. The 241-C-101 retrieval data
did not contain any information indicating the 241-AN-101 supernatant pump was having start-
up or shut-down issues.
4.2.1.5. Supernatant Pump Flow Events. Four entries noted problems with the 241-AN-101
supernatant pump flow rate. Three of these events were likely related to inlet screen plugging.
The fourth documents a loss of pump HPU speed increase/decrease controls. The HPU speed
control issue self-corrected as the system worked as designed during troubleshooting.

4.2.1.6. Slurry Pump Low Flow and Plugging Events. Twenty low flow and plugging events
were noted during the 241-C-101 campaign. Two of these events were associated with final
pump down as part of the volume displacement activities. The other 18 appear to have been
expected events as there was no indication that engineering or maintenance was notified.
4.2.1.7. Slurry Pump Movement Events. Three entries documented difficulties in moving the
slurry pump. All three events appear to have been due to material below the pump creating a rest
for the pump. The logs do not indicate that engineering or maintenance was notified, suggesting
this was expected behavior from the pump.
4.2.1.8. Sluicer / Extended Reach Sluicer System / Arm Movement Events. A single event
was noted during the 241-C-101 campaign. The sluicer number 1 controls were operating
backward, e.g., the command to move the nozzle in the upward direction resulted in downward
motion, etc. Engineering confirmed that the sluicer was oriented 180 degrees out causing the
backward nozzle controls. This was neither the first nor the last use of sluicer number 1.
4.2.1.9. Hydraulic Temperature Events. Twenty-one entries noted hydraulic system
temperature anomalies. Five instances were for low temperature, the other 16 were for high
temperature.
4.2.1.10. Miscellaneous Events. Twenty-two events were noted which did not appear to either
fit within one of the other categories or to reoccur. Figure 4-2 shows the distribution of the
events over time. The trend does not show that there was a noticeable difference in the rate of
occurrence associated with campaign start-up.

4.2.1.11. Total Number of Events. Figure 4-3 is a graph showing the distribution of the 110
events over time. As with the rate of miscellaneous events, the trend does not show that there
was a noticeable difference in the rate of occurrence associated with campaign start-up. The
events occur at a steady rate over the duration of the campaign. This suggests that the
construction and start-up testing processes are of sufficient quality as to not adversely impact
operations.

4.2.2 241-C-107 Retrieval
The information documented in Appendix B was sorted into the categories listed in Section 3.0,
“Methodology.” The resulting table is in Appendix G. A total of 231 entries were made
documenting issues over 136 days of operation. Figure 4-4 shows the distribution of the events
between categories. No trends were noted within the 241-C-107 information. Analyses of each
the categories are provided in the following subsections.

4.2.2.1. Hydraulic Leak Events. Nine hydraulic leak events were noted during the 241-C-107
campaign. Two events documented a drop in the supernatant pump hydraulic fluid level with no
apparent leakage immediately prior to failure of the pump. Two events were associated with a
spray leak by a filter in the POR237-WT-ENCL-400 hydraulic cabinet. A fitting leak at
POR241-WT-HPU-001 was noted as was a leak at the constant tension hoist reel in the MARS
containment box. Two events documented a seal leakage from the wrist sweep stack valve. The
final hydraulic leak event noted a steady stream of fluid from POR237-WT-ENCL-400.
4.2.2.2. Ventilation Events. Ventilation issues were noted 37 times during the campaign.
Thirty-two of the entries were associated with POR107, three were associated with POR03, and
one each with POR008 and the AN VTP.
Twenty of the events noted were associated with either the CAM or the record sampler. One of
these events was the result of a confirmed high radiation condition on the sample paper.

Thirteen events were associated with the instrument flow rates. The other six events were noted
as CAM failures.
The other 17 events documented a variety of issues including 2 for Human-Machine Interface
(HMI) issues and 3 for control valve problems. Four events noted involved the 241-C-107
pressure transmitter. Five events were associated with problems with the stack temperature
differential. There was a single instance noted of low stack flow and two instances of the
systems shutting down without a cause being identified.
4.2.2.3. Camera and Lighting Events. Two camera and lighting events were noted. The first
event noted that the Riser 2 and Riser 7 cameras in 241-C-107 had both degraded to near failure
and that retrieval was not able to continue with the resulting visibility. The second event noted
that the 241-AN-106 camera had failed.
4.2.2.4. Supernatant Pump Start-Up and Shut-Down Events. Supernatant pump start-up or
shut-down events were noted on five occasions. Two of the events were associated with signal
problems for the flow control valves. The other three instances were simply described as “timed
out.”
4.2.2.5. Supernatant Pump Flow Events. There were 13 events noted associated with low
flow from the supernatant pump. Two were associated with the failure of the AN106-WT-P-22
electric pump. Two occurred when the pump speed was being ramped up or down. One event
was associated with flow control valve issues while another was documented as being due to the
MARS’ arm being moved to 90 degrees. Seven other events were associated with blockage of
the intake screen.
4.2.2.6. Slurry Pump Low Flow and Plugging Events. Plugging of the slurry pump intake
screen and associated shut down was noted on 71 days. Many of these days had in excess of five
events of shut down per shift worked. The logs do not indicate that engineering or maintenance
was notified, suggesting this was expected behavior from the pump. RPP-RPT-54959 notes that
the supernate used as sluicing medium starting in early 2014 reacted with the solids, creating
phosphate crystallized waste solids agglomerations within 241-C-107. Subtracting the events
after the introduction of the phosphate bearing supernate leaves 45 days where there were low-
flow related shut downs not related to the phosphate crystallized waste solids agglomerate. A
quick count indicates that in excess of 250 shut downs were experienced during these 45 days.
As this is the only example of the MARS retrieval technology included within the scope of this
analysis, it is not possible to determine if the reason the waste was not being broken down into
pieces small enough to be transferred was due to the waste type or due to the retrieval
technology.
There were four additional events documenting issues with the slurry pump speed sensors. The
first entry simply states that there were issues with the speed sensors. The second and third
entries documented the troubleshooting evolution associated with the failure of both speed
sensors. The final entry states that the speed sensor shows 0 rpm on start-up. No additional
information was provided.

4.2.2.7. Slurry Pump Movement Events. The slurry pump and back stop could not be moved
as desired 11 times during the campaign. Nine of the events were associated with the pump; the
other two were for issues with the back stop.
Pump movement was limited due to the lower cable limit six times. There was a single instance
of the pump not lowering because the contactor had tripped and one from having blockage
beneath it. The final entry discusses the slurry pump dragging when the turntable was rotated
which affected the reeving of the cable onto the winch. There were no apparent long-term ill-
effects to the winch.
There was one instance of the backstop being limited by the lower cable limit and one instance
of the backstop not moving without a cause being given.
4.2.2.8. Sluicer / Extended Reach Sluicer System / Arm Movement Events. There were 15
entries associated with the movement of the MARS’ arm. Seven of the events were related to
HMI operation. Problems with the constant tension hoist resulted in five entries. Two entries
were associated with failure of the proximity switches and the associated compensating efforts.
There was one documented instance of problems with the HPU and one where the curb feeler
stopped actuating.
4.2.2.9. Hydraulic Temperature Events. The hydraulic fluid temperature was either too high
or too low eight times.

4.2.2.10. Miscellaneous Events. Fifty-five events were noted which did not appear to either fit
within one of the other categories or to reoccur. Figure 4-5 is a graph showing the distribution of
the events over time. The trend does not show that there was a noticeable difference in the rate
of occurrence associated with campaign start-up.

Five leak events were associated with the rotary union. During an interview with 241-C-107
project personnel, it was discovered that the leaks observed were not hydraulic leaks as
originally believed from the entries, but were waste leaks (WRPS-PER-2012-1321, “Evidence of
Previous leakage from the Rotary Union on the C-107 MARS Unit” and WRPS-PER-2012-1346,
“C-107 MARS Containment Box Rotary Union Drip Leak”). The project determined that the
leaks were the result of the manufacturing methods used when fabricating the rotary union. A
new rotary union was fabricated using a modified manufacturing method and installed in the
241-C-107 MARS. No leaks were observed at the rotary union after the new union was
installed.

4.2.3 241-C-111 Bulk Retrieval
The information documented in Appendix C was sorted into the categories listed in Section 3.0,
“Methodology.” The resulting table is in Appendix H. A total of 37 entries were made
documenting issues over 40 days of operation. Figure 4-7 shows the distribution of the events
between categories. No trends were noted within the 241-C-111 information. Analyses of each
the categories are provided in the following subsections.

4.2.3.1. Hydraulic Leak Events. The 241-C-111 campaign log books did not document any
hydraulic leak events.
4.2.3.2. Ventilation Events. There were nine events noted associated with the ventilation
system in the log books. All nine events were associated with the 241-C Farm ventilation
systems, split between POR008 and POR03.
POR008 had two events. The first event noted was the system making significant noise and
cycling the variable frequency drive. Toward the end of the campaign, the system was shut
down on rate of increase in differential pressure.
POR03 was involved with seven events. Five of the events were associated with the CAM, one
documented software modifications, and one addressed a low plenum flow condition.

4.2.3.3. Camera and Lighting Events. Camera and lighting events were noted seven times
during the campaign. The first entry may reflect the initial camera and lighting installation. A
second entry later in the campaign documents replacing the Riser 7 camera and lighting. The
Coffman lights were noted to have high amps on three occasions, once in Riser 1, once in Riser 7
and one entry did not indicate which riser was affected. The Riser 7 high amp entry was
followed shortly by the light in Riser 7 failing.
Three entries discussed concerns with the camera. The camera was noted to have lost the ability
to rotate. Shortly afterward an entry stated that the camera was adjusted. The entries did not
indicate which camera was involved. The Riser 1 camera was noted to have electrical issues
early in the campaign with burned wiring and a failed ground-fault circuit interrupter breaker.
4.2.3.4. Supernatant Pump Start-Up and Shut-Down Events. A single event was noted
indicating the supernatant pump had difficulties with start up or shut down. The pump would not
come up to speed in a timely fashion and would time out. Four attempts to start the pump all had
the same result – they timed out before the pump could reach operating speeds. Engineering
provided direction to wait 5-10 minutes after starting the HPU before attempting to increase the
supernatant pump speed. The log book indicates that this approach was successful.
4.2.3.5. Supernatant Pump Flow Events. The supernatant pump was observed to have 11
flow events. All of these events appear to be related to the speed indication and how it affected
the flow rate. Ten of these events took place over 14 operating days. During these 14 days,
operations continued with intermittent pauses for trouble shooting.
4.2.3.6. Slurry Pump Low Flow and Plugging Events. The slurry pump was noted to have
two low flow events. The first was associated with material build up and appears to have been
an expected event as there was no indication that engineering or maintenance was notified. The
second coincided with a supernatant pump shut down, also on low flow. The shift log noted that
the reason for the second shut down was unknown.
4.2.3.7. Slurry Pump Movement Events. One entry noted that the slurry pump went slack
when attempting to lower the pump. The logs do not indicate that engineering or maintenance
was notified, suggesting this was expected behavior from the pump.
4.2.3.8. Sluicer / Extended Reach Sluicer System / Arm Movement Events. The 241-C-111
campaign log books did not document any sluicer movement events.
4.2.3.9. Hydraulic Temperature Events. The 241-C-111 campaign log books did not
document any hydraulic temperature excursions.

was a noticeable difference in the rate of occurrence associated with campaign start-up. The
events occur at a steady rate over the duration of the campaign. This suggests that the
construction and start-up testing processes are of sufficient quality as to not adversely impact
operations.

4.2.4 241-C-112 Bulk Retrieval
The information documented in Appendix D was sorted into the categories listed in Section 3.0,
“Methodology.” The resulting table is in Appendix I. A total of 69 entries were made
documenting issues over 72 days of operation. Figure 4-10 shows the distribution of the events
between categories. No trends were noted within the 241-C-112 bulk retrieval information.
Analyses of each the categories are provided in the following subsections.

4.2.4.1. Hydraulic Leak Events. There were six notations of hydraulic leak events. Five of
these notations address the actions surrounding a leak associated with sluicer number 1. The
other notation documents a slurry pump HPU drip.
4.2.4.2. Ventilation Events. Nine ventilation events were documented during the 241-C-112
bulk retrieval campaign. One of these events documented that both POR008 and the 241-AN
VTP systems were off-line due to a power outage. There were four instances where the POR008
seal pot transmitter was indicating either a high or low level. It was noted that the low level
events appeared to be related to low ambient temperatures affecting the sensing legs. There were
two instances where the documentation simply stated that the 241-AN VTP was off-line. There
were also two notations documenting POR008 high differential pressure events.
4.2.4.3. Camera and Lighting Events. There were 15 entries associated with the cameras and
lighting. Four of the notations documented camera issues without indicating there were
problems with the lights. Four indicated instances where both the cameras and the lighting were

inoperable or significantly degraded. Of these four, three of them were associated with a single
event where both of the cameras and all of the lights were impacted. The delay associated with
this event was longer than typical for camera or lighting issues due to poor weather curtailing
field activities.
There were seven entries which only addressed lighting issues. By the end of the campaign, as
soon as a light was replaced, the one in the other riser would fail, to the point that operations was
noting that only one light at a time would be used in an attempt to conserve the lights.
4.2.4.4. Supernatant Pump Start-Up and Shut-Down Events. The supernatant pump had
start-up or shut-down events noted seven times. Six of these events address start-up concerns,
while the remaining one is for a delay in the pump shutting down. All seven of the events appear
to be based on controlling the pump from the control trailer instead of at the HPU. All six
instances related to pump start-up include discussion on the pump amps not increasing. The log
books indicate that the pump would either start later that day or the next day without
maintenance or repair.
4.2.4.5. Supernatant Pump Flow Events. Events related to supernatant pump flow were noted
six times. All six events appear to be related to intake screen blockage. Operations documented
several attempts to clear the blockage with drain back with varying success. Flushing of the line
was used in two instances which cleared the blockage and the flow rates returned to the expected
ranges.
4.2.4.6. Slurry Pump Low Flow and Plugging Events. The slurry pump was noted to have
low flow and plugging events on seven days. This appears to have been anticipated behavior as
there was no indication that engineering or maintenance was notified.
4.2.4.7. Slurry Pump Movement Events. The slurry pump was noted to have hampered
movement on four occasions. In all four instances, it is likely that the pump was resting on
solids which prevented the pump from going lower into the tank. This appears to have been
anticipated behavior as there was no indication that engineering or maintenance was notified.
4.2.4.8. Sluicer / Extended Reach Sluicer System / Arm Movement Events. A single entry
for sluicer number 2 indicates that the sluicer’s range of motion was limited to around 180
degrees.
4.2.4.9. Hydraulic Temperature Events. Two events were noted where the hydraulic fluid
temperature disrupted operations. Both entries document low temperature conditions.

4.2.5 241-C-112 Hard Heel Retrieval

documenting issues over 42 days of operation. Figure 4-13 shows the distribution of the events
between categories. No trends were noted within the 241-C-112 HHR information. Analyses of
each the categories are provided in the following subsections.

4.2.5.1. Hydraulic Leak Events. The 241-C-112 HHR campaign log books did not document
any hydraulic leak events.
4.2.5.2. Ventilation Events. Fourteen ventilation events were noted during the 241-C-112
HHR campaign. Three events were related to the POR008 seal pot level transmitter sending
erroneous high or low level signals. Two events documented the 241-AN VTP being shut down.
The remaining nine events document POR008 high differential pressure across various
components. All nine of these events appear to have been correctly indicated. Although
engineering was contacted for direction to proceed under the high differential pressure
conditions, maintenance was not called for equipment repair.
4.2.5.3. Camera and Lighting Events. The cameras and lighting were noted to have impacted
the retrieval four times during the 241-C-112 HHR campaign. The four entries document that
the Riser 6 camera was replaced twice, the riser 3 camera was replaced once, the riser 6 lights
were replaced once, and the riser 3 lights were replaced twice.

campaign log books did not document any supernatant pump start-up or shut-down events.
4.2.5.5. Supernatant Pump Flow Events. The 241-C-112 HHR campaign log books did not
document any supernatant pump flow events.
4.2.5.6. Slurry Pump Flow Events. There were two instances noted of slurry pump flow
excursions. The first instance resulted from a speed sensor failure. The second entry noted a
low flow event resulting from blocking of the intake screen.
4.2.5.7. Slurry Pump Movement Events. A single event was identified where the slurry pump
was raised and then lowered. The end position of the pump appeared the same as the original
position despite being raised and lowered different amounts. The log noted that there were
several turns of slack on the pump. As neither engineering nor maintenance were notified, it is
likely that the pump was resting on solids.
4.2.5.8. Sluicer / Extended Reach Sluicer System / Arm Movement Events. One entry
documented that sluicer number 2 was unable to rotate toward the sample sleeve opening. Thirty
minutes later, sluicer number 2 was fully rotating again.
4.2.5.9. Hydraulic Temperature Events. Four hydraulic fluid temperature excursions were
noted. Three of the events were associated with the slurry pump HPU while the other was
associated with the supernatant pump HPU. These events were in addition to those instances
where the ambient temperature drove the overall pit temperatures below the range where
transfers could occur.

4.3 EQUIPMENT FAILURE TRENDS
There were 487 events noted in the retrieval information reviewed in this analysis. The table
tallying the events is contained in Appendix K. Figure 4-16 shows the distribution of the events
between categories. Trends were not observed on an individual retrieval basis as documented
above. Of interest in this finding is that there was not an increase in events at the start or the end
of the retrieval activities indicating that the failures are not entirely related to installation
concerns or equipment wear. Comparison of the total number of events per retrieval campaign
did not reveal any significant trends. An evaluation of the events based on the categories
described in Section 3.0, “Methodology,” is detailed in the following subsections.

4.3.1 Hydraulic Leak Events
Hydraulic leaks were noted a total of 23 times. Nine of these events were noted during the
241-C-107 retrieval activities. No leaks were noted during the 241-C-111 bulk retrieval
activities or the 241-C-112 HHR activities.
Four of the events were related to the slurry pumps, with an additional three associated with
supernatant pumps. Twenty-one were specified as having come from the sluicer associated
equipment. Eleven of the sluicer related failures were on the MARS arm. The majority of the
hydraulic leaks resulted in the equipment continuing in service with reduced retrieval efficiency.
Notable exceptions to this trend were the two hydraulic leak entries immediately prior to the

failure of supernatant pump AN106-WT-P-24. Seven of the leaks were in components external
to the tank while the other 21 occurred inside the tank or tank headspace.
One particular category of equipment was noted as leaking on multiple occasions. Two of the
hydraulic leaks from the 241-C-101 bulk retrieval were from a rotary union. Further
investigation into the 241-C-101 hydraulic leaks attributed these to electro-spark damage to the
Teflon hydraulic hoses. Waste leaks from the rotary union are discussed in Section 4.3.10.
4.3.2 Ventilation Events
Eighty-six entries in the various logs were connected to the four ventilation systems – POR107,
POR03, and POR008 in 241-C Farm and the 241-AN VTP. The 241-AN VTP being shut down
affected the retrieval campaigns 12 times. POR03 was shut down in a manner that delayed
retrieval 10 times while POR008 was noted as being shut down unexpectedly 22 times. POR107
impacted retrieval 42 times.
The ventilation failure mode which occurred most frequently involved the CAM or record
samplers. The stack particulate emissions monitoring systems disrupted retrieval 32 times
during the various campaigns. Many of these alarms were due to low record sample or CAM
sample flow. It is recommended that the record sample and CAM systems and the rate of
associated alarms be further investigated to explore design improvements (e.g., a different
vacuum pump) and alternate operational strategies (e.g., increasing the rate of filter paper change
out).
The next most common alarm condition was high differential pressure across various ventilation
components including the pre-filter, heater, and filters. The majority of the high differential
pressure events were associated with the hot water use during the 241-C-112 HHR activities.
Although they did not result in alarms, visibility issues (i.e., foggy conditions within the tank)
were noted many times during the high-pressure wall washing at the end of the various retrieval
activities. Reducing the rate at which the hot water is sprayed may help reduce the number of
differential pressure alarms. Increasing the tank’s ventilation flow rate would help with the
foggy conditions; however, it may increase the rate that the filter is loaded with water, which is
causing the high differential pressure alarms.
4.3.3 Camera and Lighting Events
Camera and lighting events are fairly common and occurred during all of the evaluated retrieval
campaigns. A total 42 entries were noted where at least one of these components required
replacement. Frequently, the cameras and lights were replaced at the same time. In total, the
cameras were replaced 32 times, as were the lights. Although these camera and lighting systems
serve their function well, there may be a more cost-effective solution than continuing to replace
the failed components with the same ones currently implemented in the field.

4.3.4 Supernatant Pump Start-Up and Shut-Down Events
Unexpected conditions during the start up or shut down of the supernatant pump were noted 13
times during the 5 retrieval campaigns. Ten times the pumps did not start or stop and were
described as having timed out. Generally the timing out was indicative of either a problem with
the signal from the control trailer to the HPU or the hydraulic fluid being too cold.
The other common failure was related to the operation of the supernatant line flow control
valves. The 241-C-107 retrieval activities had several events where the flow control valves did
not respond as anticipated which resulted in the pump shutting down for low flow conditions.
The logs indicate that the flow control valve problems were signal related and not associated
with valve or actuator failure.
4.3.5 Supernatant Pump Flow Events
There were 34 entries associated with supernatant pump flow events. Sixteen of the events were
associated with the intake screen being blocked, apparently an anticipated behavior as there was
no indication that engineering or maintenance was notified. Two of the events were documented
just prior to the failure of the AN106-WT-P-22 electric pump. Two other events were related to
operation of the flow control valves or the sluicer.
Fourteen events were related to issues with the speed sensors and the pumps’ response to
increases and decreases in speed. Evaluation of the pump speed and flow rate control devices
and the correct device to use for monitoring for each parameter during retrieval campaigns may
reduce the number of operational upsets.
4.3.6 Slurry Pump Low Flow and Plugging Events
Slurry pump low flow events were noted on 102 days. One event was attributed to the pump
timing out. There were two events clearly related to the final pump down process and four
events caused by issues with the speed sensors.
The slurry pump was shut down due to low flow on 99 days. The majority of these are during
the 241-C-107 retrieval campaign. On the 28 days where low flow shut down the pumps during
the other campaigns, one or two events were typically noted per day. During the 241-C-107
campaign, most of the days documented more than five low flow slurry pump events per shift
worked. As noted above, over one three-day period, the 241-C-107 slurry pump was shut down
due to low flow 34 times. The total number of 241-C-107 events was not tallied for this
analysis; however, it is likely that the pump was shut down on low flow more than 200 times.
The logs do not indicate that engineering or maintenance was notified, leading one to believe that
this was anticipated behavior. As the 241-C-107 campaign is the only example of the MARS
retrieval technology included within the scope of this analysis, it is not possible to determine if
the reason the waste was not being broken down into pieces small enough to be transferred was
due to the waste type or due to the retrieval technology. An evaluation of retrieval campaigns
which would include multiple retrieval efforts using the MARS technology is recommended to

determine if the MARS equipment should include additional equipment to reduce waste chunk
sizes.
4.3.7 Slurry Pump Movement Events
The slurry pump did not adjust its position as desired on 20 occasions. Ten of these events were
associated with a build-up material under the pump, restricting its downward progress. Seven
events were recorded when the 241-C-107 slurry pump reached the lower cable limit and was
unable to be further lowered. There was one instance where the pump did not move which was
attributed to the contactor having been tripped. The final event documented an occasion where
the 241-C-107 pump dragged on the bottom of the tank when the turntable was rotated.
4.3.8 Sluicer / Extended Reach Sluicer System / Arm Movement Events
Eighteen events were noted where the sluicer apparatus did not respond as anticipated. Fifteen
of these were during the 241-C-107 campaign. As the 241-C-107 campaign is the only example
of the MARS retrieval technology included within the scope of this analysis, is it not possible to
determine if the relatively high percentage of events associated with the 241-C-107 retrieval is
due to the MARS approach or is specific to the equipment installed in the 241-C-107 retrieval
equipment.
Eight of the events were attributed to problems with the control system, with seven of them from
the 241-C-107 campaign. The 241-C-107 campaign also noted five events where the constant
tension hoist, used to maintain the cables and lines leading to the end effector and its associated
nozzles and sluicer cannon, did not respond correctly and impacted the retrieval efforts.

4.3.9 Hydraulic Temperature Events
Thirty-five entries noted that the HPU temperature was either too high or too low. This does not
include entries when the in-pit air temperature readings were below the acceptable limit for
transfers under the waste transfer structure temperature monitoring program. Although the HPU
fluid temperatures were not at shut-down criteria levels, they were close enough that either the
system was not responding as desired or increased monitoring was required to ensure the system
was operating within the acceptable range. Figure 4-17 documents the number of hydraulic
temperature event per operating day by season. The findings suggest that the HPU cooling and
heating systems are not adequate for the temperature extremes of the depths of winter or the
height of summer.

4.3.10 Miscellaneous Events
Many events did not fall into the categories listed in Section 3.0, “Methodology,” and did not
reoccur frequently enough to result in generation of a new category. There were 109 of these
entries noted during the 390 operating days. These were graphed over the number of operating
days in Figure 4-18. The graph shows a steady trend that does not change noticeably over time.
This indicates that the events are not related to campaign start-up or campaign completion.

Five 241-C-107 system leaks were from the MARS rotary union. The 241-C-107 project
determined that these waste leaks were the result of the manufacturing methods used when
fabricating the rotary union. Corrective actions are in place for both the hydraulic hose electro-
spark issue discussed in Section 4.3.1 and the fabrication issues with the MARS rotary union.
Further monitoring of these components is recommended to ensure that the corrective actions
have addressed the issues.

operating days in Figure 4-19. The graph shows a steady trend that does not change noticeably
over time. This indicates that on average the events are not related to campaign start-up, or
campaign completion.

The events were checked to determine if there were seasonal changes. Figure 4-20 shows the
breakdown by category of the events normalized by operating day by season. With the exception
of the hydraulic thermal events discussed in Section 4.3.9 and the ventilation seal pot transmitter
issues discussed in Section 4.2.4.2, no other significant trends were observed. The slight trend
shown by the miscellaneous events and total number of events indicating that they are more
likely to occur during the summer months may be attributed to the number of weather-related
events noted during the summer months. The miscellaneous category is where events such as
rain induced leak detection events and weather-related work/rest regimes are documented. This
skews the data indicating a seasonal trend.

5.0 RECOMMENDATIONS
This document reviewed the various log books and reports associated with five retrieval
campaigns: 241-C-101 bulk retrieval, 241-C-107 retrieval, 241-C-111 bulk retrieval, 241-C-112
bulk retrieval, and 241-C-112 HHR. A total of 487 events over 390 operating days were noted
that impacted retrieval duration and/or equipment effectiveness. These were graphed over the
number of operating days as shown in Figure 4-19. The graph shows a steady trend that does not
change noticeably over time. This indicates that on average the events are not related to
campaign start-up, campaign completion, or seasonal changes (with the exception of the
hydraulic thermal events discussed in Section 4.3.9 and the ventilation seal pot transmitter issues
discussed in Section 4.2.4.2).
Analysis of the events results in the following recommendations.
Monitor the rotary unions in future MARS installations where the manufacturing method
corrective actions have been implemented to determine if the waste leaks have been
resolved.
The record sample and CAM systems and the rate of associated alarms should be
reviewed to determine if a different design (e.g., a different vacuum pump) and/or

The camera and lighting systems should be evaluated to determine if there may be a more
cost-effective solution than continuing to replace the failed components with the same
ones currently implemented in the field.
Signal transmission for both supernatant and slurry pump control should be reviewed.
Pump start-up, shut-down, and flow control issues were attributed to the system either
timing out or responding erratically in multiple instances.
An evaluation of the parameters and equipment used to control the supernatant and slurry
pumps (e.g., pump speed, flow control valves, flow rate indication, etc.) may lead to
changes in the operating strategy which could improve equipment responsiveness and
reduce inadvertent shut downs. The log entries frequently refer to pump speed with flow
indication and flow rate with speed indication.
An evaluation of retrieval campaigns which would include multiple retrieval efforts using
the MARS technology is recommended to determine if the MARS equipment should
include additional equipment to reduce waste chunk sizes. As the 241-C-107 campaign is
the only example of the MARS retrieval technology included within the scope of this
analysis, it is not possible to determine if the reason the waste was not being broken
down into pieces small enough to be transferred was due to the waste type or due to the
retrieval technology.
An evaluation of the HPU cooling and heating systems is recommended. The 35 entries
associated with either high or low HPU temperatures suggest that the HPU cooling and
heating systems are not adequate for the temperature extremes of the depths of winter or
the height of summer.
An evaluation of the total cost per gallon retrieved should be done for each technology.
A consistent level of detail of the log entries in the various log books is recommended
and would make reviews of this type more fruitful. The logs reviewed for this report do
not consistently identify if a system was restarted after shut down or why the shut down
occurred. Very little can be determined, for example, when the entry simply states that
the 241-AN VTP was shut-down. It is not always clear from the entries on that day if the
retrieval activities were impacted, how long the ventilation system was down, or why the
system went down in the first place.
