How thin areLUMINEERS® BY CERINATE®?

The LUMINEERS No-Prep Techniqueallows LUMINEERS to be placed over existingteeth without the removal of any form of toothstructure. Therefore, anesthesia and temporariesare also not required. The majority of patients fitthis technique. It is a proven fact that bondingporcelain to enamel is a solid, secure bondcompared to bonding porcelain to dentin.

The LUMINEERS Cosmetic Contour(enameloplasty only) Techniquerequires slight modification of the enamel butnever touches dentin during LUMINEERSplacement. Only 0.3 mm-0.5 mm enamel isremoved, causing no sensitivity for the patientand therefore no need for any anesthesia.Minimal contouring is commonly used whencases involve misalignment. Minimal contouringrequires the use of 4-power magnification and afluted carbide diamond.

Traditional Prep Techniqueis most commonly required when a thickporcelain is used. The full-prep technique wasdeveloped to compensate for the thick porcelainavailable to dentists at the time. LUMINEERSmade of Cerinate Porcelain is a major cosmeticbreakthrough. It can be made thick or thin; mostpatients, however, prefer thin LUMINEERS due tothe non-invasive technique. An additionalappointment is required for full-prep, as well asanesthesia and temporaries. And as noted,bonding porcelain to enamel is a solid, securebond compared to bonding porcelain to dentin.

TIPS FOR SUCCESS

The LUMINEERS® BY CERINATE® program is extremely successful and exciting because it is every dentist’s and every patient’s dream. You canprovide a new smile without shots, without the painful removal of sensitive tooth structure, and without the need for even an aspirin before, duringor after the placement of a new, gorgeous LUMINEERS Smile.* Listed below are tips for successful placement of LUMINEERS, or as some dentists liketo think of it: “How to prevent remakes!”

TIPS FOR NEW CUSTOMERS:

• Pre-Training: Make sure that you have attended a LUMINEERS course, especially within the past 12 months. While the LUMINEERS procedure isnot complicated, you will likely encounter challenges you haven’t experienced before. A course on the placement of LUMINEERS will help youavoid any initial start-up pitfalls. You will also learn about the most current products and techniques, as these are in a continuous state ofimprovement.

• Be Selective: Selecting a case of 6–8 units on well-aligned maxillary teeth is your best choice for a first case. Avoid extreme cases (severelystained, overly crooked, large diastemas, gingival recession or missing teeth) until you’re more experienced in the placement technique.

TIPS PRIOR TO SENDING IN A CASE:

• Staff Training: Your staff is critical to the successful placement of LUMINEERS. One of your best investments is the training of your assistant inthis procedure. Proper preparation of the LUMINEERS, the right sequence of use of the materials, the prevention of pre-polymerization and thecorrect mix of the luting agent are all critical parts of your success.

• Shade: It is always easier to make a lighter shade darker, but almost impossible to make a darker shade lighter.

• Single Units: If at all possible, avoid doing a single LUMINEERS. These are difficult to color-match and maintain. Natural dentition adjacent tothe LUMINEERS normally will darken over time. If the patient bleaches, then their natural teeth may end up lighter than the LUMINEERS. It maysave the patient an expense early on, but it will cause dissatisfaction for the patient in the future. If the teeth are harmonious in form, the singlelaminated tooth without reduction will always be too prominent. Whenever possible, encourage the patient to place multiples of 2 or 4, at aminimum, rather than placing one anterior tooth without any reduction.

• Good Impressions: Do you remember how tough your professors were on you in dental school to take good, sharp impressions? It is extremelyimportant that you take a sharp impression because this is what guarantees a successful case, start-to-finish. Always allow trays to remain in themouth for the full amount of time, according to manufacturer’s directions.

NOTE: It is best to pour a model from the impression to make sure the gingival margins are sharp. This will save the time of having toremake impressions.

• Materials: To achieve the highest esthetic value, proper fit, and shade matching, the entire LUMINEERS System is required. The system includesUltra-Bond® Plus Try-In Paste, the Cerinate® Shade Modification System, Ultra-Bond® Plus and Tenure®. Attending a Cerinate Procedurepresentation is highly recommended to ensure proper use of the system.

BONDING LUMINEERS®TO ENAMEL OR A DENTIN SURFACE

1. USE 4-POWER MAGNIFICATION

4-power magnification is essential. Obtain a goodmagnification system from Den-Mat® or someoneelse. Your own prescription may be placed in theeyeglass lenses. You will be better able to performexcellent work and prevent problems.

2. PREPARATION OF LUMINEERS

a. After the patient is seated, treat the LUMINEERSwith Porcelain Conditioner for 30 seconds, rinseand dry well.

b. Apply Cerinate Prime® to the LUMINEERS for 30seconds and blow thin.

c. Place LUMINEERS back into the case box in theirproper slots.

3. SURFACE PREPARATION

a. Clean the teeth with a slurry of Porcelain LaminatePolishing Paste and water.

b. Syringe Paint-On Dental Dam on lingual side ofthe interproximals, covering the gums and teeththoroughly (this aids in clean-up). Light-cure.

c. Etch teeth for 20 seconds with Etch ‘N’ Seal®, rinseand dry.

d. Mix together equal amounts of Tenure® A and B.

e. Apply 5 coats of the A-B mixture to each tooth, or untilglossy, then air-dry the tooth surfaces.

4. TRY-IN OF LUMINEERS

a. Syringe the Ultra-Bond® Plus Try-In Paste into eachof the LUMINEERS.

b. Gently insert the LUMINEERS one at a time and usea brush to clean up the excess Try-In Paste.

c. Hold a mirror approximately 16 inches away fromthe patient’s face and let them examine their smile.

d. Use a Schure 349 instrument or cotton pliers andgently tease the LUMINEERS off the tooth surfaces.

NOTE: If the shade is not what you or the patient want,remove the Try-In Paste from the LUMINEERS and toothsurface with a Skube® or a clean, dry brush. Reapply anew shade of Try-In Paste and check the shade again.

e. Remove the Try-In Paste from the LUMINEERS andtooth surfaces with Tenure S. Only use Tenure S toclean up excess once the final Try-In Paste hasbeen selected.

f. Gently thin the Tenure S with air. A thin film will beleft on both the tooth and porcelain surfaces.

NOTE: There is no need to reapply PorcelainConditioner or Cerinate Prime if you are cementingdirectly after try-in.

5. BONDING LUMINEERS

a. Select the appropriate shade of Ultra-Bond® (Kit No.032599750). NOTE: If patient has dark stains, suchas tetracycline stains, we recommend the newBLOCK-OUT® opaque system by Ultra-Bond (Part No.032491510). If shade modification or basic opaqueadjustment is necessary, refer to this kit’s instructionsat this point. If not, proceed to the next step.

NOTE: After color selection is achieved, internal surfacesof LUMINEERS® do not need to be retreated.

b. Apply Ultra-Bond to the prepared LUMINEERS and putin place.

c. After placing, remove excess Ultra-Bond with a softbrush coated with Tenure® S or with Dab-Eze® sponges.

d. Tack LUMINEERS in place using a Sapphire® SupremePlasma Arc Curing Light fitted with a Ceri-Taper™2 mm tip for 2 seconds.

e. Continue to remove excess cement with a soft brushcoated with Tenure S.

f. Completely light-cure all LUMINEERS, exposing eachsurface for 5 seconds with a Sapphire Supreme PACLight fitted with a 9 mm tip.

6. FINISHING

We recommend the LUMINEERS® Finishing Kit(No. 033897100), which includes all the tools mentionedbelow. Reference Image-1 for bur images.

a. Clean up any leftover Ultra-Bond® resin cement usingbur G (or D).

b. Use Den-Mat’s Schure 349 instrument to remove thecured cement along the margins, as well as from theinterproximal area along both the facial and thelingual. You may also use bur G. This bur is anexcellent tool for cleaning up residual resin cementand also is great for finishing composites.

c. Use bur B to remove the porcelain ledge.

d. Trim/polish the margins of the porcelain veneer.Use bur C, an extra-fine diamond, to smooth outporcelain.

e. Using the interproximal fine diamond bur F, known asthe “mosquito” diamond, refine the margins along theinterproximal area.

f. Explore the tooth structure next to the CERINATE®LUMINEERS to check margins. There should not be acatch or bump.

g. If necessary, further polish the margins using theinterproximal fine diamond bur F again.

h. Check the bite with articulating paper. Use bur E, theAmerican football-shaped bur, on the lingual to finishthe incisal wrap and adjust high spots or length.

i. Place a wooden wedge interproximally (optional).

j. Use the CeriSaw to separate the teeth. Leave thewooden wedge in place.

k. Use the CeriSander (fine grit) to smooth theinterproximal surfaces.

l. Remove the wooden wedge.

m. Use the CeriSander (ultra-fine grit) to further smooththe interproximal surfaces. It is important that youuse each sanding blade only two or three times toopen contacts.

n. Check the interproximal surfaces with dental flossfor smoothness.

o. Polish the new smile with Porcelain LaminatePolishing Paste using polishing cup A.

BONDING LUMINEERS® TO A PORCELAIN SURFACE

TIPS:

• Placing LUMINEERS Over Crowns with Metal Margins: It is essential to use 4-power magnification and a small bur to remove the metal so thereis at least 1 mm of space between the LUMINEERS and the metal. This will allow for application of either TetraPaque® or Virtuoso® Flowable to fillthat area and make it look natural again.

• Placing LUMINEERS Over Pre-Existing Restorations: Whenever possible, it is best to place LUMINEERS on all units of a pre-existing bridge to getthe most satisfactory result. It is generally impossible to achieve a correct shade match when veneering one unit of a multi-unit restoration. Use ofthe Cerinate® Shade Modification Kit II is most helpful. It is important to know how to prepare all the surfaces in these types of cases. Again, this iswhere a LUMINEERS Placement Course can make the difference for long-lasting, beautiful restorations.

STEPS:

1. Sandblast (microetch) to roughen the porcelain surface.

2. Apply Porcelock® Porcelain Etchant (Kit No. 402060) to the porcelain surface for 2–3 minutes, then rinse and dry. Avoid soft tissue contact.

NOTE: Porcelock Porcelain Etchant is only for porcelain.

3. Apply Porcelain Conditioner to the porcelain surface for 30 seconds, then rinse and dry.

4. Apply Cerinate Prime® (Kit No. 1399) and gently air-dry.

5. Mix together equal portions of Tenure® A and B and apply 3–5 coats, or until glossy.

6. Use the Revitalize Modifier and Opaquer (a thin layer) on the metal to mask discoloration, if needed.

7. Apply Revitalize Resin Cement inside LUMINEERS and place LUMINEERS on the tooth surface.

8. Light-cure for 1–2 seconds using the Ceri-Taper™ 2 mm or 4 mm tacking tip and the Sapphire® Supreme Plasma Arc Curing Light. Removeexcess Revitalize Resin Cement from the margins by using a soft brush coated with Tenure S. Using a 9 mm tip and the Sapphire SupremePAC Light, completely cure all LUMINEERS, exposing each surface for 5 seconds.

9. Maximum bonding occurs in 24 hours. Final finishing should be performed at next visit.

10. Finishing—Adjust the occlusion and perform a routine finish on the margins.

intraoral bonds. PorceLock’s gel viscosity and needle tip dispensing allow you to place the gel only where

you want it. PorceLock is a buffered intraoral and extraoral use hydrofluoric acid gel that is both safe

and effective. Its formula contains 2.5% hydrofluoric acid.

PorceLock creates bond strengths three to five times greater than treatments using silane alone. It achieves

bond strengths approaching those of etched enamel after a 3–5 minute treatment.
