EXECUTIVE SUMMARY

The McGill Division of Orthopaedics is based at the Montreal General Hospital, which houses one of Quebec’s busiest Level I Trauma Centres, a Supra-Regional Centre for Musculoskeletal (MSK) Oncology and serves the professional athletes of three world class sports teams. More than 200 beds dedicated to adult and paediatric orthopaedic care are distributed between the Montreal General, Montreal Children's, Shriners, Jewish General and St. Mary's hospitals.

In 2010, a core administrative staff coordinated the activities of 31 orthopaedic surgeons, 9 fellows and 36 residents with a support staff of 23 clinical and 12 research coordinators. The clinical and research activities in Arthroplasty, MSK Oncology, Paediatrics, Spine, Sports and Trauma are complemented by translational research in labs across the McGill campus that include the Joe Miller Lab (MGH), the JTN Wong Lab (740 Penfield), the Orthopaedic Research Labs (RVH, LDI) and the Shriners-MCH.

From 2009-2010 these laboratories trained 43 individuals in projects ranging from fundamental mechanisms of bone development to advanced technology for bone repair. In the same period, members of the Division held funding from peer reviewed sources including AO Foundation, CIHR, US Department of Defense, FRSQ, Leukemia & Lymphoma Society, NSERC, Orthopaedic Trauma Association and others and published more than 70 clinical and 50 basic scientific papers.

SECTION 2: HIGHLIGHTS from the DIVISION In November 2010, the McGill Medical Simulation Center hosted the 5th Annual McGill / Université de Montréal Arthroscopy Course. This one of a kind collaboration between Montreal’s two major academic institutions is an ever expanding 2-day course, organized by Dr. Mark Burman (MGH), Dr. Monika Volesky (JGH) and Dr. David Blanchette (UdeM). This unique collaborative effort is structured to offer arthroscopic training in a didactic and hands on manner to community orthopedic surgeons from across Quebec, the Maritimes and Eastern Ontario. The second day is dedicated to resident teaching, offered to Orthopaedic residents from all 4 Quebec medical teaching programs. Aside from a strong group of local McGill and U de M faculty, we welcomed representation from other Quebec teaching programs and were honored to have Dr. Freddie Fu as the special Invited Guest Faculty. Dr. Fu is the Chair of Orthopaedics at the University of Pittsburg and a world renowned leader in the field of Sports Medicine and Anterior Cruciate Ligament Reconstructive surgery. Dr. Fu shared his thoughts on Anatomic ACL reconstruction with course participants and residents during a much appreciated special Grand Rounds followed by a surgical cadaveric demonstration. Drs Burman, Volesky and Blanchette are already at work putting together the next course for November 2011. In October 2010, the Division hosted Mininder S. Kocher MD, Associate Professor of Orthopaedic Surgery, Harvard Medical School and Associate Director of the Division of Sports Medicine, Children's Hospital Boston as the 25th Annual Eugene Rogala Visiting Professor. Dr. Kocher is the head team physician for Babson College and the Boston Public School Sports Medicine Initiative and serves as physician for the Boston Ballet, the USA Figure Skating and Ski Teams, USA Track & Field and the Boston Marathon. He is the recipient of numerous awards for clinical excellence including the 2005 American Academy of Orthopaedic Surgeons (AAOS)/ OREF Kappa Delta Award/ Clinical Research Award, which is the highest research honour in orthopaedics. As the newly appointed Chief of Paediatric Orthopaedic Services for the Shriner’s Hospital for Children – Canada, Dr. Reggie Hamdy will provide leadership during the transition from the 85 year-old facility on Cedar Ave. to the Glen Campus facility, slated for completion in 2014. The Shriner’s revealed their plans for a new hospital at the Glen Campus adjacent to the Montreal Children’s Hospital (MCH) of the McGill University Health Centre (MUHC) in July 2010. The clinical facilities with four operating rooms, 22 single patient rooms and a rehabilitation facility will share services with the MCH and be supported by fundamental and translational research activities in 25,000 square feet of dedicated space. Dr. Jean Ouellet, appointed as Deputy Chief of Staff in December 2010, will work closely with Dr. Hamdy to ensure the timely and effective transition of clinical and research activities from the existing location on Cedar Ave. to the Glen Campus. The Division of Orthopaedic Surgery welcomes Dr. Neil Saran to the Montreal Children’s and Shriner’s Hospitals. Dr. Saran joined the Paediatric Orthopaedic Surgery team in September 2010 after completing fellowships in Paediatric Orthopaedics & Scoliosis at the Texas Scottish Rite Hospital for Children (Dallas, Texas) and Paediatric Spine & Hip Surgery at the BC Children’s Hospital (Vancouver, BC) and a concomitant MHSc in Clinical Epidemiology. In addition to his strong interest in Evidenced-Based Medicine, he has joined the expanding group of surgeon scientists in the Division. Dr Saran brings a multitude of much needed new skill sets to the McGill unit including hip realignment procedures such as the Ganz periacetabular osteotomy and surgical hip dislocations for adolescent and young adult patients with hip dysplasia or femoroacetabular impingement to prevent early onset osteoarthritis of the hip. In addition, he has a keen interest in minimally invasive spinal surgery to improve post-operative management and outcomes of patients undergoing spinal surgery. Congratulations to Dr. Saran on performing the first minimally invasive adolescent idiopathic scoliosis corrective surgery in Canada outside of British Columbia.

SECTION 3A: SUMMARY of CLINICAL RESEARCH ACTIVITIES Arthroplasty: The arthroplasty surgeons based at the MUHC-MGH and SMBD-JGH are conducting clinical research on the benefits of computer-assisted navigation in total knee arthroplasty and randomized trials on the long term outcome of metal-on-metal hip arthroplasty and plasma ion levels in these patients. Other work conducted in collaboration with colleagues across Canada and in Europe involves the evaluation of clinical outcomes including pain assessment in patients with novel hip implants and assessment of a new class of anticoagulants in patients undergoing surgical procedures. Oncology: The MUHC led the largest study ever conducted across Canada on outcomes following treatment of myxoid\round cell liposarcomas, as well as a study sponsored by the Public Health Agency of Canada and the Quebec Ministry of Health investigating the incidence and nature of post-transplant adverse events following allograft bone transplantation. The later study precedes the establishment of a national surveillance system of adverse events following tissue and organ transplantation. Collaboration continues with the University of Toronto and McMaster for banking sarcoma tissue and clinical data collection on a prospective study examining the benefits of surgery for long bone metastases. Paediatrics: The expanding group of physicians providing orthopaedic care at the Shriners and MCH sites are aligning their clinical research efforts in the surgical reconstruction of the hip, limbs and spine with fundamental work on cartilage and bone regeneration. Osteogenic proteins have been investigated to increase bone formation and botulinum toxin A to decrease pain in children undergoing limb lengthening to correct for discrepancies with the Ilizarov technique. Other work is investigating minimally invasive surgical approaches for the correction of hip dysplasia and novel growing rod implants for guided spine growth that will significantly improve the quality of life of young children with scoliosis. Spine: Much of the clinical research effort in spine effectively crosses the boundaries of that classified under Paediatrics and Trauma. The research efforts that fall in the adult spine domain include collaborative work with international consortia that addresses spinal stenosis and cord compression and the development of advanced instrumentation and technology for the management of fractures of the spine. Sports: In keeping with their role as attending physicians to some of Canada’s top sports teams and elite athletes, the research by MUHC surgeons is focused on the development of advanced 3D MRI for the planning of ACL reconstruction, the use of patient controlled analgesia following ACL reconstruction and the advantages of using different arthroscopic portals for minimally invasive procedures. Trauma: The orthopaedic trauma surgeons are involved in many ongoing studies with colleagues across Canada under the umbrella of the Canadian Orthopaedic Trauma Society (COTS). The MUHC group has played a leading role for more than a decade in prospective, randomized orthopaedic trials on a wide variety of topics including open reduction and internal fixation of fractures in the elderly, operative vs non-operative treatment of displaced calcaneal and clavicular fractures and closed reduction with external fixation. The COTS model for collaboration amongst surgeons and clinical research coordinators for the development of a research protocol is widely recognized as key to the success and productivity of the group.

SECTION 3B: SUMMARY of TRANSLATIONAL RESEARCH ACTIVITIES

Joe Miller Lab: The main areas of research past and present include orthopaedic biomaterials and biomechanics, non-cemented implant fixation, porous materials, tissue response, bone remodeling response to total joint prostheses, drug delivery from implants for fixation enhancement, de-mineralized bone matrix for fixation enhancement, implant retrieval analysis, implant design, implant stiffness material testing (static and fatigue), stability measurements of knee and hip implants, hip simulator testing of bearing materials with emphasis on hard/hard bearings such as cobalt-chrome/cobalt-chrome and ceramic/ceramic and highly cross-linked polyethylene.

JTN Wong Lab: Current research is focused on skeletal regeneration and reconstruction using small animal models of implant infection and failure, fracture mal-union and repair, steroid-induced bone disease and genetic-based models of disorders of skeletal development and bone metabolism. The lab supports the research programs of clinician scientists from Trauma (Harvey, Henderson, Berry), Paediatrics (Ouellet, Saran) and Sports (Martineau) and from Haematology/Oncology (Seguin). Current trainees include one post PhD fellow, five post MD MSc or PhD candidates, two post BSc MSc candidates and five undergraduate students from high school, biotechnology and MDCM programs.

ORL-LDI: Efforts to characterize disc degeneration using gross morphology, current MR imaging, and biochemical techniques have been disappointing. Thus, one of the long term goals of our research activities is to develop an objective, accurate, non-invasive diagnostic tool (Quantitative MRI) in the detection and quantification of matrix biochemical composition and integrity and biomechanical changes in early disc degeneration. The inception of such a diagnostic tool will be a foundation for future work in diagnosing and treating spine pathology. A second area of research is focused on intervertebral disc regeneration using autologous mesenchymal stem cells embedded in a support scaffold.

ORL-RVH: The McGill Orthopaedic Research Laboratory was founded in May 1993. The Laboratory is a multidisciplinary research facility dedicated to basic and applied spinal research, focusing on the functional spinal unit, and the single motion segment of the spine. Fundamental and clinician scientists in the field of biomechanics, biochemistry, morphology, and electrophysiology, combine to form a fully integrated multidisciplinary laboratory. The group intends to provide a comprehensive understanding of spinal disorders that will lead to an improvement of current diagnostic and therapeutic tools of spinal pathology.

Shriners-MCH: Current research activities remain focused on the molecular basis and treatment of heritable disorders of the appendicular and axial skeleton including disorders of bone and mineral metabolism, limb length discrepancies, hip dysplasia and scoliosis. The work includes development of novel nanoparticle-based delivery systems for biological repair, cloning and characterization of components of the vitamin D signaling pathway, correlation of genotype-phenotype in non-lethal osteogenesis imperfect, molecular biochemical analysis of scoliotic intervertebral discs and the development of novel surgical implants and approaches for the correction of hip and limb length discrepancies and spine curvature.
