    Diabetes experts have proposed a simple

    standard for the diagnosis of the disease :a fasting blood sugar (FBS) of 126 mg/dLor higher, providing the result isconfirmed on the second test.

    To be accurate , the blood should beobtained after eight hours without anycaloric consumption.Fasting blood sugars between 100 and125 indicate impaired glucose tolerance, amilder condition that may lead to diabetes;levels below 100 are officially normal.

    Any blood sugar level above 200 suggests thediagnosis, even if its obtained after eating.

    To be officially diagnosed as diabetic,however, people with random sugars of 200or higher should also have symptoms of thedisease, such as increased thirst and urinationor unexplained weight loss.

    A third way to evaluate diabetes is to see what% of oxygen carrying hemoglobin moleculesin a persons red blood cells has glucoseattached to them.

    A single glycosylated hemoglobin (HbA1C)levels reflects a persons average blood sugarduring the preceding 30 days.

    Normal is 5.0 and 7.0 indicates diabetes.

    The more of these risk factors you have,

    the greater your chances of gettingdiabetes :- Youre over age 45 -You have a family history of diabetes.Your Body Mass Index (BMI) is 25 or more.(The risk of diabetes starts to climb at aBMI of 22.)You dont exercise regularly. You blood pressure is 140/90 or higher.Your HDL (good) cholesterol is 35 or lower.

    Complications and prognosis

    Acute complications1. Diabetic ketoacidosis (DKA)is an acute and

    dangerous complication that is always a medicalemergency.

    2. Hyperglycemia hyperosmolar state - (HNS) is an acute

    complication sharing many symptoms with DKA, but anentirely different origin and different treatment.

    3. Hypoglycemia - or abnormally low blood glucose, is anacute complication of several diabetes treatments.

    4. Respiratory infections - The immune response isimpaired in individuals with diabetes mellitus.

    Chronic complications1. Vascular disease - Chronic elevation of blood glucose level leads to

    damage of blood vessels ( angiopathy )

    The damage to small blood vessels leads to a microangiopathy , which cancause one or more of the following:

    1. Diabetic retinopathy 2. Diabetic neuropathy 3. Diabetic nephropathy 4. Diabetic cardiomyopathy

    Macrovascular disease leads to cardiovascular disease, to whichaccelerated atherosclerosis is a contributor:

    1. Coronary artery disease , leading to angina or myocardial infarction ("heart

    attack )2. Stroke (mainly the ischemic type)3. Peripheral vascular disease , which contributes to intermittent claudication

    (exertion-related leg and foot pain) as well as diabetic foot.4. Diabetic myonecrosis ('muscle wasting')
