Men With Belly Fat At Risk For Osteoporosis

 According to the National Center for HealthStatistics, more than 37 million American menover the age of 20 are obese. Obesity is associated

 with many health problems, including cardiovas-cular diseases, diabetes, high cholesterol, asthma,sleep apnea and joint diseases. Researchers arenow saying that visceral, or deep belly, obesity is a risk factor for bone loss and decreased bonestrength in men.

"It is important for men to be aware that excessbelly fat is not only a risk factor for heart diseaseand diabetes, it is also a risk factor for bone loss,"Miriam Bredella, M.D., radiologist at Massa-chusetts General Hospital and associate profes-sor of radiology at Harvard Medical School inBoston was quoted saying. "Most studies on osteoporosis have focused on

 women. Men were thought to be relatively pro-tected against bone loss, especially obese men,"Dr. Bredella said. Not all body fat is the same. Subcutaneous fatlies just below the skin, and visceral or intra-abdominal fat is located deep under the muscletissue in the abdominal cavity. Genetics, diet and

exercise are all contributors to the level of vis-ceral fat that is stored in the body. Excess visceralfat is considered particularly dangerous, becausein previous studies it has been associated withincreased risk for heart disease. Dr. Bredella and her team evaluated 35 obesemen with a mean age of 34 and a mean body mass index (BMI) of 36.5. The men underwenta CT of the abdomen and thigh to assess fat and

muscle mass, as well as very high resolution CTof the forearm and a technique called nite el-ement analysis (FEA), in order to assess bonestrength and predict fracture risk. "FEA is a technique that is frequently used

in mechanical engineering to determine thestrength of materials for the design of bridges orairplanes, among other things," Dr. Bredella wasquoted saying.

"FEA can determine where a structure will bendor break and the amount of force necessary tomake the material break. We can now use FEAto determine the strength or force necessary tomake a bone break," Dr. Bredella continued. In the study, the FEA analysis showed that men

 with higher visceral and total abdominal fat hadlower failure load and stiffness, two measures ofbone strength, compared to those with less vis-ceral and abdominal fat. There was no associa-tion found between age or total BMI and bonemechanical properties. "We were not surprised by our results that ab-dominal and visceral fat are detrimental to bonestrength in obese men. We were, however, sur-prised that obese men with a lot of visceral fathad signicantly decreased bone strength com-pared to obese men with low visceral fat butsimilar BMI,” Dr. Bredella concluded. 

 The results also showed that muscle mass waspositively associated with bone strength.

 with increased risk for acute myocardial infarc-tion (AMI, heart attack).

Many adults in the United States are affected by the strain of employment instability, but little isknown about the cumulative effect of multiple

 job losses and unemployment on the risks for AMI. Matthew E. Dupre, Ph.D., and colleagues fromDuke University, Durham, N.C., examined theassociations between different dimensions of unemployment and the risks for AMI in 13,451U.S. adults’ ages 51 to 75 years in the Health andRetirement Study with biennial follow-up inter-

 views from 1992 to 2010. "Results demonstrated that several features of one's past and present employment increasedrisks for a cardiovascular event. Although therisks for AMI were most signicant in the rst

 year after job loss, unemployment status, cumu-lative number of job losses and cumulative timeunemployed were each independently associated

 with increased risk for AMI," the authors werequoted saying. 

 The study group (median age 62 years) had1,061 AMI events (7.9 percent) during 165,169person-years of observation. In the study group,14 percent of the individuals were unemployedat baseline, 69.7 percent had one or more cumu-

lative job losses, and 35.1 percent had spent timeunemployed, according to the study. Statistical analysis indicated that AMI risks

 were signicantly higher among the unem-ployed (hazard ratio [HR], 1.35) and that risksincreased incrementally from one job loss (HR,1.22) to four or more cumulative job losses (HR,1.63) compared with no job loss. The risks for

 AMI also were "particularly elevated" within therst year of unemployment (HR, 1.27) but notthereafter, according to the results. "We found that the elevated risks associated

 with multiple job losses were of the magnitudeof other traditional risk factors, such as smoking,diabetes mellitus and hypertension," the authors

 were quoted saying. "In the context of the current U.S. economy andprojected increases in job instability and unem-ployment among workers, additional studiesshould investigate the mechanisms contributingto work-related disparities in AMI to identify
