PART I . DESCRIPTION OF 22 MONTH SURVEY

The background to the study

The experimental evidence that malnutrition Inanimals during the period of brain cell proliferation canlead to a permanent reduction in the number of brain cellshas suggested that malnutrition In human Infants might have apermanent effect on their subsequent physical and mentaldevelopment (Cruise, 1973; Davies and Davis, 1970, Dobblng,1964, Fltzhardlnge and Steven, 1972; Thomson, 1970; Wlnlck,1969). Prolonged malnutrition 1s rare during postnatal llfeamong babies In this country, but It maY occur prenatally.Damage from fetal malnutrltlon, If Irreversible, could beexpected to have a depressive effect on the subsequentdevelopment of the child.

The Identlflcatlon of babies who suffered fromfetal malnutrition Is not easy. Babies of low blrthwelghtare not a homogeneous group and Include many whose weight 1swlthln normal llmlts for their length of gestation. Somebabies are genetically small, some have abnormalities orillnesses which affect their growth, while others areretarded due to unfavorable conditions In utero. Thesocial, biological and medical factors wh~ch led to the fetalmalnutrition may In some cases cont~nue to operate and,therefore, have a direct effect after birth on the progressof the child. Despite these considerable llmltatlons, It wasfelt that there were three groups of babies which were mostl~kely to include substantial numbers who had suffered fromfetal malnutrition. The f~rst group were the multlpleblrtbs, because It was assumed that the primary, but not theonly, reason for fetal malnutrition might be competitionbetween the fetuses for available nourishment. The secondgroup were the postmature babies, because placental growth lSsaid to cease from the 39th week of pregnancy (Gruenwald,1963) and thus the likelihood of fetal malnutrition occurring1s Increased In any baby born after term. The third groupwere the small-for-dates babies, by virtue of the fact thatthey showed evidence of fetal retardation at everygestatlonal age.

Plan of Investlgatlon

Method of sampllnK. During the week beglnnlng 5 April 1970,a detailed survey of all births which occurred In the UnitedKingdom took place under the auspices of the N.stlonalBlrthdav Trust Fund and the Royal College of Obstetrlclansand Gynaecologlsts IBrltlsh Births, 1970). A stratifiedsample was drawn from th~s cohort of 16,955 llveblrths asfollows.

(1) A 10% random sample of all singleton and multlplebirths.

(11) A1l multlple births, i.e. twins. (The one set oftriplets died In the first week of llfe).

(111) All singleton chzldren who were born at or later than42 weeks gestation whose mothers were recorded as beingcertain of the date of their last menstrual period andwere not recorded as having taken the contraceptive PI1lfor at least 15 months before childbirth.

(IV) A1l singleton children who were on or be~ow the 5thcentlle of blrthwelght for their length of gestation,allowlng for their sex and maternal parity, butregardless of whether their mother was certain or not ofher last normal menstrual period or had taken thecontraceptive pill. the children were ldent~fled usingTanner and Thomson’s (1970) charts for blrthwelght andlength of gestation.

All the children of mothers who stated that theywere unmarried, divorced, separated, or widowed at childbirthwere then excluded, because of the dlfflcultles lnvol~ed intracing and examlnlng them.

The total sample came to 3,471 children, of whom 96were known to have died since birth. Some of the children,of course, belonged to more than one of the above categories.A multlple birth could also be In the random group, asingleton child could be In the random group, In thepostmature, or small-for-dates group, or any combination ofthese .

Examlnatlon of children. The survlvlng children were tracedand the examinations arranged by health vlsltors and otherstaff of the local health departments. Practically allexamlnat~ons were carried out by cllnlc medical officers, afew children were seen by general practitioners and, w~th theassistance of the Mlnlstry of Defence, the children ofmembers of the armed forces who were llvlng abroad wereexamined by service doctors. All the health departments InWales and Northern Ireland took part, but three In Englandand two in Scotland were unable to do so because of staffshortages, this accounted for non-exam~natlon of 5.4% of thechildren. Seven percent of the ch~ldren were not seenbecause the parents refused or defaulted their appointments,while 11.1% were untraced or were traced too late forlncluslon In this first survey at 22 months. A further 2.9%were In hospital, on hollday, or had left the country. Thus ,in all, 73% of the children from the orlglnal sample wereexamined or were known to have died. Table 1 shows theproportions examined, these dld not vary slgnlflcantly In thethree study groups from the random sample.

The results were recorded on a standard form. Thefirst part conta~ned a llm~ted number of questions about thefamily background. most of th~s Information was already knownfrom the Brltlsh Births Survev. The second part kas a recordof a developmental screening test. The third Indicated theheight, length, weight, and head circumference of the child,together with information on the past medical history and anyabnormallt~es noted on examination. The need for accuracywas stressed and In the maJorlty of Instances these
