1. Purpose. To provide instructions for preparation and distribution of the DD 214 Certificate of Release or Discharge from Active Duty, DD 214C Certificate of Release or Discharge from Active Duty, Continuation Sheet, DD 214WS Certificate of Release or Discharge from Active Duty Worksheet and DD 215 Correction to DD 214, Certificate of Release or Discharge from Active Duty. This instruction is a complete revision and should be reviewed in its entirety.

2. Cancellation. BUPERSINST 1900.SC.

3. Background

a. Per reference (a), Department of Defense (DoD) prescribes the DD 214/DD 214C/DD 214WS and DD 215 for use by all military services. Reference (b) provides separation/discharge reasons.

b. Enclosures (1) through (3) are used for the preparation and distribution of the DD 214/DD 214C and DD 215. The NAVPERS 1900/2, Separation Travel Orders, will be issued to enlisted personnel upon separation and marked "For Official Use Only".

c. The DD 214 is des igned to provide a short- form origi nal which excludes ~special Additional Information# and a long form copy with additional i nformation selectively i ncluded based on the recipient's needs .

d. DD 214 provides military services with information necessary for administrative pro~ess i ng and enl ist.rnent/ reenlistment determination as well as data required by separatees for persona l use.

4. Intent of the DD 214. The DD 214 is a brief, c l ear-cut record of a period or t erm of active mi l itary service tha t provides:

a . Mil itary services with a readi l y available source of i nformation used to determine el.igibil ity for enl istment or reenl i stment;

b. Service members with a brief record of act i ve service; and

c . Appropriat e governmental agenc ies with An Authoritative source of i nformation which they require in the administration o f Federal and State laws.

5. Control of the DD 214/00 214C/DD 2l dWS and DD 215. These forms are documents containing sensitive information and are vul nerabl e to fraudulent use . OQD has directed the following procedures to prevent unauthorized use of the form:

a . Command Re sponsib i l ities. Each command will establ ish i nt ernal accountability procedures for use and destruction of forms . The commanding officer/officer in charge (CO/OIC) will a ppoint, i n writing , a n individual of grade &7. GS- 7, or above to control , and i ssue the DD 214, DD214C. DD 214~1S and when a ppropriate, t he DD 215. The CO/OIC of designated separation <tctivities l ist ed i n reference (b) 1910 - 8 12 may authorize an ES, GS•S, or above to serve in this capacity. if so designated in writing. Le t ter of appointment :shal l not be sent to Navy Personnel Command (NAVPERSCOM), but sha l l be maintained in the command files.

b. When a command i s supported by a Personnel support li.cti vi ty Detachment ( PERSUPPDET) , responsibility for issuance of the DD 214 lies with the PERSUPPDET.

c. Security, The following procedures for security of electronic forms are provided as follows:

(1) co·s must ensure appropriate security measures are in place to safeguard unauthorized use of the electronic PDF fi l e DD 214 /DD 214C/OO 214WS and OD 215 by placing the form on a separate dir·ectory, using l ocal area network (LAN) security and 1:-estricting QC::cess only t o qua l ified personnel creating the actual form.

(2) Assign a serial number in block 18 of the DD 214 (bl ock 5 of the OD 215) and maintain these serial numbers in either an e lec t ronic or paper log . The serial number consists o f f our elEMnents (14 spaces}, to i ncl ude command uni t i den tification code (U IC), two digit calendar year, four digit consecutive number, and initials of the preparer (e . g . , SER: 62981• 00-0001- DWC) .

(3) Log consists of . a t~ min~mum, serial number, Name o f Person Discharged/Separated, and Effective Date of Discharge/ Separation . conunands may add additional elements as desired.

d. Destruction of Forms. Obsolet e, blank, partially complet ed, and reproduced copies of the OD 214, OD 214C. DD 214WS and OD 215 to be discarded, wil l not remain intact but will be des troyed; i . e . , by shreddi ng or burning. No form will be discarded intact .

6. Records Management. Records created by this i nstructi on, regardless of media, will be managed per SECNAV Manual 5210 . l of November 2007.

7. Forms

a. Authorized separation acti v ities wi l l be provided current electronic versions of the DD 214/214C/214WS and 215.

1 . Criteria for I ssuance of DD 214. The DO 214 i s prepared to cover periods of service on active duty, active duty for training {AD'l'}, full - time train i ng duty, active duty for special work (ADS\oJ), certain periods termi nat ed by a change o f status not concurrent with separation from active service, and release from a status that is l egal ly determined t o be void. The DO 214 will be issued to members i n conjunction with their release from active duty servi ce and prior t o final departur e from their command on permissive temporary additional duty, terminal l eave and associated travel.

a. Eligible Personnel . The DD 214 wil l be issued to the fol lowing personnel:

(1) Personnel Released from Active Duty. The DD 214 wil l be prepared for each member at the time of separation from a pe.riod of active naval service as specified below or a per iod of service det err.tined to be void. The DD 214 will be provided t o officers dismi ssed under the sencence of a general court ­martial or dropped from t he rolls, (NOTE: Members who are placed on appellate l eave are NO'!' released from active duty. ttppellate leave i s a statut orily auth6rized non- pay l eave status . Do not, in such cases, i ssue a DD 214.)

{2> Personne l Released from ADT, Full-Time Training Duty ; or ADSlfl. The DO 214 will be prepared for personnel being separated from a peri od of ADT, ful l-time training duty, or ADSW when t he period of service was 90 days or more. The DD 214 a l so will be prepared for personnel being separated for cause or for physical disability, regardless of length o f t i me served on active duty. In the case o f disability, the OD 214 wi l l s how active duty for only the period for which the member was ordered to AOT. The period of hospitalization subsequent t o that period may be s hown i n the "'Remarks" block.

(a) Reserve Compone.n.t personnel ordered to active duty for a contingency operation. wi l l be i ssued a OD 214 r egardless of the number o f days served on acti ve duty .

(bl The requirement shall a l so apply to recalled retirees reverting t o retired status, regardless of the period o f active duty served.

(3) Released from Acti ve Duty Following Callup, Partial Mobilization or Full Mobi l ization. Personnel ordered t o active duty in time of National Emergency declared by either t he ?resident or Congress, or war dec l ared by Cong~ess wi l l be provided a 00 214 upon release from active duty . The DD 214 will be issued regardless of the l ength of time served on active duty.

(4) Personnel Continu ing or\ Active Duty. The DD 214 will be prepared and furnished to members while serving on active duty when they have a change of status or cor;1ponent as follows:

{a} Enl isted Personnel. Discharged to accept permanent appointment to either warrant or commissioned status for continued active duty.

(b ) Officers

1 . Temporary appointment terminated to accept a permanent warrant or commission in the Regular Navy or- Navy Reserve.

2, Appoi ntment terminated to accept appointment i n Qnother branch of the Armed Forces .

3 . upon termi nation of temporary appointment when enlistment contract expires concurrently.

4 . Officers retired and continued on active duty in a retired status will not be issued a OD 214 until t:he actual da te of separation (e.g., an officer placed on the retired list of the Navy on 30 J~ne 2004 and continued for 2 years, will not be issued a DD 2:14 unt i l 30 June 2006, the actual date of separation).

(5) Enlisted Personnel Released to Become Midshipmen or Cadets. The DD 214 will be issued for enlisted personnel on active duty who fal l under one o f the foll owing categories:

(a) Appointed midshipmen, U.S. Navy.

(b) Appointed midshipmen, U.S. Navy Reserve, or accepted as col lege program students i n Naval Reserve Officers Training Corps (NROTC) ?rogram.

(c) Upon acceptance of commission.

(6) Midshipmen and Cadets . The DD 214 will be prepared and issued at time of release to those midshipmen and cadets, including t hose se.rving concurrently as enlisted members, at the Naval Academy, Air Force Academy, Coast Guard Academy. or the Military Academy, released per reference {b ) 1910-184 and 1531-020.

b. Inel igib le Personnel . The DD 214 wi l l not be prepar ed or issued i n the case of personnel :

{1} Who are discharged for i mmediate enlistment or r:eenlistment on ac.tive duty;

(2) Who are found physic.ally disqual ified upon reporting for active duty and who do not perform active duty under orders;

{3} Who die whil e on active duty, active duty for training, full - time traini ng duty, or active duty for special work;

(4) Who are relea$ed from A.DI' of less than 90 days un less separated for physical d i sability or from a period of special AOT under a call for mobi l ization;

(5) Who receive a temporary officer appoi ntment in the naval service, or when temporary officer status is terminated and reverted to enlisted status to comple t e an enl i stment contract;

(6) Who are midshipmen, U.S. Navy, (Naval Academy), accepting a permanent commission in the Navy or Navy Reserve;

(7) Who are midshipmen, Naval Reserve Officer Training Corps;

(8) Who are removed from the Temporary Dis4bility Ret i red List (TDRL);

(9) Who are enl isted personnel transferred to the Pleet Reserve and who are concurrently retai ned on active duty;

(10) Who are discharged from Fleet Reserve i nactive status; or

(11) Who are officer per,sonnel retired and who are c oncur rentl y retained on active duty.

c. Times of Issuance. The no 214 will be compl eted for eligible personnel at time of re lease, transfer, change of status, or discharge except as s ·tated below.

(1) Personnel in ~Awaiting-Ordersw Status . I n the case of personnel who have appeared before a physical evaluation board (PEB) and have been p l aced i n an awaiting-orders status pending fina l action by the Se cr~tary of the Navy (SECNAV) . the c ol'M\and responsible for admini stering the member's records and accounts will complete blocks 1 through Ba; 10, 1 1 , 12a, 12i, 13 , 14, 19 through 22, and 30 prior co the departure of the member. The DD 214 wil l not be distributed until f i nal action is direcced by the NAVPERSCOM, Career Progression (PERS-8). I n t hose cases where retirement or discharge is directed . the r emaining blocks will be complet-ed and all copies of the form distributed promptly per i nstruc tions set forth in paragraph 4 . If the member is returned t o active duty instead of being r eti red or discharged, the original and all copies of the DD 214 will be destroyed .

(2) Personnel Transferred to Department of Veterans Affai rs Medical Center (DVAMC} , When a member is transferred to a DVAMC for f urther treatment pe:nding final action on the report of the PEB, the naval acti v ity affecting such t ransfer will c omple t e as much of the DD 214WS as possible with the information avai l able prior to the member's transfer. The worksheet will be forwarded t o the PERSUPPDET serving the area in which the designated OVA.MC is located. When retirement or discharge is directed by NAVPERSCOM (PERS - 8), the PERSUPPDET will complete the OD 214 and dis tri bute a ll copies promptly. A locally reproduced copy of the copy number 3 will be forwarded t o t he DVAMC in which the member i s a patient .

(3) Personnel Retired or Transf erred to the Fl eet Reserve and Retai ned on Active outy. 'tlhen an officer or

enl isted member is retired or transfe-rred to the Fleet Reserve, but retained on active duty, the OD 214 will not be issued until final separat ion from active service is accomplished , The service record wi l l not be forwarded to NAVPERSCOM {PERS-312} but will be retained at t he conu:nand until the member compl etes t he period of active duty.

(4) Members Separat ed Wh.ile on Separation Leave. Fo r chose members whose separation will become effective while on separation leave authori zed per reference (b) 1050-120, the DD 214 will be prepared and delivered prior to the member's departure. The remainder wi l l be distributed as i ndicated in paragraph 4.

(5) Void Enl istment . A DD 214 will be provided to enlisted members bei ng rel eased froro a status which has been j udicial l y determi ned to be void . Co1nplete the DD 214 per paragraph 3 except:

(a) Enter ' N/ A" i n !>locks 9, l l, 13 . 14, 16, and 17;

b . Accuracy and Compl eteness. The DO 214 i s an important r ecord of service. It mu-st be a<:curate and complete . Only au thorized items may be entered.

{3) Compl etion of Items. Block-by- b lock i nstructions for the en tries made on the DD 214 are conta i ned i n paragraph 3 below. No block wil l be left completely blank and no marginal entries will be made. When i nformation is not a vailable or more space is needed, the fol lowi ng instructions apply:

c. Legibility . Each copy of the DD 214 must be legible and will be printed with a reproduci'ble screen tint using appropriate security ink on Blocks l . 3, da , 4b, 12, and 18 t hrough 30 . Blocks 1, 3. 21, and 22 of the DD 214C And bl6~ks 1 , 3 . 5, 6 . and 8 of the DD 215 will be simi l arly printed to make a l terations readily discern i ble. No corrections wi l l be permitted in the screened areas.

d. Errors and Alterations. The use of the DO 2l4WS should prevent errors from appearing on the DD 214 , No changes or corr ect ions may be made to the shaded blocks of the form. Any unavoidable corrections or changes made i n the unshaded a r eas of t he DD 214 during preparation wi ll be neat, legible, and i nitialed on all copies by the a·uthori~ing o f ficial. The recipient will be informed t hat :making any unauthori zed change or alteration of the OD 214 will render it void.

3 . l nstructions for Completing the DO 214

a. Block 1 . NAME. Enter member ' s name : last name (in capital l e tters} , first name , and middle name. Do not i nclude spaces, hyphens or apostrophes in last name. Type a conuna after the l ast name . If the member does not have a middle name, indica t e by "NMN. • If the member uses an initial instead of a first or middle name, indicate by enclosing the initial in

quotation marks: e.g., "'J" or "K". Al so include, when a pplicabl e, Jr . , Sr., I I I , etc., following the member's middle name .

f. Department of Veterans Affairs State Director Copy (Number 6) . For personnel released from active duty, discharged . retired, or transferred to the Fleet Reserve, forward to the appropriate OVA S tate Director (see enclosure (3) ), if the member so reque$ts by checking "YES"' in block 20 of DD 214 . The member must specify the State. If the member does not request that the copy be mailed, copy number 6 will be destroyed.

g. Service Copy (Number 7). copy number 7 will be destroyed.

h. Servi ce copy (Number 8) . Issuing command must file a hard copy in an electronic format for 2 years from date of separation. After this period, copy number 8 wi l l be destroyed or deleted from the electronic f.ile. In the event the member should request an additional copy of the form, a copy is availab le from the issuing command.

i. Advance Issuance of DD 214 for Demobilizing Reservists. The 00 214 will be prepared and signed by the member and authorized official up to 120 days i n advance of the member's separation . Provide member copy (number 4) to demobilizing reserve personnel who were mobilized in support of a contingency operation. This advanced copy will be provided to the member during the demobilization proces.s prior to the member departing the Navy Mobilization Processing Site (NMPS) on separation leave and is provided solely for the purpose of pre-registering Sailors for Veteran's Administration medical/dental benefits at NMPS and i s not to be provided to any other command for any reason .

5 . Corrections to DD 214. No changes, strikeovers. erasures, or corrections may be made in the shaded blocks of the DO 214, (bl ocks l, 3. 4, 12, and 18 thro-ugh 30). Any other changes or correc tions entered on OD 214 may also render the form void unless they are initialed by the authorizing official . The member will be advised that any unauthorized change or alteration will render the form void. The member to whom the OD 214 is issued wil l be informed of the proper procedure for having official corrections made when errors are discovered after receipt .

a. Of ficial Chan9es and Corrections. Once the 00 214 has been issued and copies distributed, official corrections may be made by issuance of DD 215, exce·pt as noted i n paragraph 6 below. The OD 215 will be distributed i n the same manner as the ori ginal DD 214 which it corrects. Since the information contained in blocks 25, 26, and 27 (SEPI\RATION .~UTHORITY., SEPI\RATION CODE, AND REENTRY CODE, respect ively) has limited distribution, correction to these items on a DD 215 requires the l i mitation on distribution of the DD 215 . I f other items as wel l as blocks 25, 26, and 27 are corrected, issuing authorities must assure that copies of the DD 215 issued to agencies not entitled to recei ve blocks 25, 26 , and 27 information, receive DD 215 copies which omit that information .

b. Request for correction. A correction request must contain the full name, rank/rate·, social security number, periods of service, and current mailing address of che member c oncerned. The correction requested must a l so be identified, and if necessary. supporting documentation provided. If a copy of the 00 214 for which the request i s being made is avail abl e, it should be forwarded with the reques t to one of the following locations:

b. Reissuance. NAVPERSCOM (PERS -312) may reissue or d i rect reissuance when:

(1) Directed by appropriate appellate authority, Executive Order. or SECNAV.

(2) A DD 214 cannot be corrected by the i ssuance of a DD 215 or the correction would require the issuance of more than t wo DD 215's .

(3) There is a change directed by competent authori ty vice a correction of administrative error to Character of Serv ice in block 24 .

c. Request for Issuance or .Reissuance. Requests for issuance or reissuance should be addressed as i ndicated in Sb {l) or Sb(2) .

8. Repl acement of the OD 214

a . General . I n order to obtain a copy of a DO 214, a signed r equest mus t i nclude adequate i dentifying data ; i.e. , full name, rank or rat e, social security number, Navy service number{s), dat es o f service, and a current return address.

b. Individual Request. I n response to an individual's signed request for a copy of the DD 214, and based on the date o f i ssue o f the OD 21 4, the indi v i dual will receive the f ol l owing:

(1) In t he case of a DD 214 issued on or after l October 1979, one copy of the DD 214 inc luding the Special Addi tional Information (blocks 23 through 30).

(2) In the case of a DD 214 issued be fore 1 October 19?9, one copy of the OD 214 from which the Authori ty for Separation, Narrative Reas on for Separation, SPD. and Reentry Codes have been del e ted, and one copy o f the OD 214 containing ~he Authority for Separa tion, SPD, and Reentry Code. If specifical ly requested, t he individual may a l so be provided a p l ain language ~Narrative Statement« of the reason for the separation.
