Healthcare TransitionsThrough a grant-funded project, the Division ensured that
all nursing homes accessed the Delaware Health Information Network.
The Division is now working with the Post Acute Care Council and the Transitional Care Consortium to ensure seamless transitions between healthcare settings and to
reduce readmissions to hospitals.

• In 2012, the Division presented an Emergency Preparedness Conference for facilities in conjunction with the UD College of Health Services and contracted with a consultant to assist nursing homes in developing All-Hazards Preparedness plans.
• In 2013, All-Hazard Preparedness plans were completed for all nursing homes.

• We continue to approve curriculum and provide oversight of the testing/certification process
• The web-based educational tracking system regulates the 24-hour minimum training requirements
