To the Residents of DeKalb County:

We are pleased to present our Fifty-Third Annual Report which highlights the ways in which the DeKalbCounty Health Department works each and every day to improve health outcomes in our community.

Our most recent Community Health Improvement Plan was adopted by the Board of Health at the end of2018 and in 2019 we moved to the action phase of the Mobilizing for Action through Planning andPartnership (MAPP) process. Throughout the year, we worked collaboratively with our partners toimplement strategies identified in the Community Health Improvement Plan which aligned with ourestablished health priorities. We have highlighted some of our successes within this report and look forwardto continued collaboration with our valued partners.

At the beginning of this year, DeKalb County joined the Intergovernmental Personnel Benefit Cooperative(IPBC) for employee health and dental insurance. One of the benefits of joining the Cooperative is that theCounty became eligible to receive incentives for the development of a county-wide wellness program.Beginning in June of this year, the Health Department kicked off the DeKalb County Wellness Initiative byconvening the DeKalb County Wellness Committee. Over the course of six months, the Committee workedto plan and implement the annual biometric screening event, implement an employee wellness interestassessment and develop an action plan for wellness activities moving forward.

We continued our focus on improving operational efficiencies this year by implementing a new electronictimekeeping system. With the new system, we have been successful in eliminating paper time sheets aswell as several other spreadsheets used for payroll. In addition, we were able to streamline the tracking ofaccruals and upgrade our employee badging procedure. The Health Department, as the pilot site, went livein December of this year, with the remaining County Departments to follow suit in 2020.

In November 2019, we initiated the first phase of strategic planning which included a review and update ofour current mission statement and the development of a new vision statement. We believe that the revisedmission statement emphasizes our focus on community collaboration and the newly developed visionstatement articulates our focus on the health of the entire community. Moving forward, our Department willcontinue to work with our community partners to prevent disease and injury, protect the environment andpromote health and towards our vision of a community where all can achieve optimal health where they live,work and play.

We hope this report not only highlights the achievements in 2019, but also demonstrates our commitmentto improving outcomes in our community. We would like to thank our Health Department staff, ourcommunity partners, the elected and appointed officials and the residents of DeKalb County for theirongoing support and commitment to public health.

FDA Food CodeImplementationOn January 1, 2019 the Health Departmentimplemented the 2017 FDA Food Code to meet Staterequirements. Most food safety guidance andrequirements remained the same, but there werenoticeable changes including:

The old inspection form was a simple 45-pointchecklist, the new form is a 58-point checklist thathas two main sections "Foodborne Illness RiskFactors/ Public Health Interventions" and "GoodRetail Practices".Establishments used to receive a score based onhow many violations they had, now there is noscore and each item (violation) is simply marked asIn/Out of Compliance.Each item(violation) is broken into three groups:Priority Item, Priority Foundation Item, and a CoreItem. Each group has a different time  frame forcorrection.Inspections are taking about 30% longer and theperson in charge during the inspection is requiredto engage in more dialogue consisting of food flow,allergens, HACCP concepts, etc.

Approximately 300 carswere served during theevent.A second ElectronicWaste Collection eventheld on September 7,2019, served  over350  residents recycling45,000 pounds ofelectronic waste.

Family Case Management Program  Women, Infants and Children(WIC) Supplemental Food &Nutrition ProgramHelps meet the nutritional  needs of income-eligible  women who are pregnant, breastfeedingand  infants and children under 5  years oldthrough  health and nutrition assessments  andeducation, food vouchers to participating  storesand breastfeeding support. Many working familiesand those who receive SNAP benefits also qualifyfor WIC services.  Walk-ins are welcome.

Assists income eligible pregnant women, infants andyoung children to obtain the healthcare services andother assistance  they may need to have a  healthypregnancy  and  promote the child's  healthydevelopment.  We continue to encourage pregnantwomen to seek our services early in their pregnancy.

Adult & Travel ImmunizationsProvides vaccines for preventable diseases toadults, including flu vaccines. Travelimmunizations visits include a nurseconsultation for recommended vaccines andcountry specific information.

On February 22, 2019, DeKalb County Health Departmentconducted an emergency preparedness drill by having aPersonal Hygiene Collection Exercise. The exercise allowedstaff to simulate receiving supplies from the StrategicNational  Stockpile (SNS). When SNS is requested  for aDisaster  in a real situation, the Health Department has to beprepared to receive, sort, inventory, and push out to setlocations.  from the SNS. The exercisenot only to prepared staff for a publichealth emergency, but also helped tostock DeKalb County food pantrieswith personal hygiene necessities.

Community Lab DrawThe Community Lab Draw (CLD) program launchedon August 5, 2019.  CLD provides discounted labservices with a healthcare provider's prescription forages 18 and up. Laboratory testing services areprovided by the University of Illinois HospitalReference Laboratory. This is a cost effectiveprogram that is perfect for uninsured, under-insured and high deductible insurance plans.

Healthcare Enrollment Assistance Program

Provides assistance  with enrollment in the HealthInsurance Marketplace and Medicaid. Funding for thisprogram is made possible by the  DeKalb CountyCommunity Mental Health Board.

Initiative: Basics DeKalb CountyDCHD applied for and was awarded funding from the Northwestern Medicine Community Benefit Grant todirectly support activities under the Maternal and Child Health Priority area of the Community HealthImprovement Plan. A portion of the funding was allocated to create an interactive wall that promotes Basics

Initiative: Vaping Education

Early in 2019, DCHD adopted a Trauma-Informed Care Organizational Framework and began taking stepstowards becoming a trauma informed organization. The ultimate goal of this initiative is to evolve ourpractices to promote a culture of safety, empowerment, and healing. In June, DCHD staff attended their firsttrauma informed training and participated in several other trauma-informed trainings throughout the courseof the year. In August, DCHD convened an internal Trauma Action Team that focused on gaining a workingunderstanding and knowledge of the Substance Abuse and Mental Health Services Administration's traumacomponents and principles and on assisting Health Department programs with the implementation oftrauma-informed practices. We were fortunate to have received support from the DeKalb County MentalHealth Board to secure consultation in the development of trauma specific training for staff and supportthrough observations and coaching for Community Health and Prevention staff.

Initiative: Trauma Informed System of Care

DeKalb County. Basics DeKalb County, based on theBoston Basics, is a set of five evidence-basednurturing practices that help all infants and toddlersthrive cognitively, socially and emotionally. The wallincorporates all five Basics with interactive elementsincluding sensory wall panels and book shelves withboard books that clients are able to take home.Remaining funding was utilized to promote safesleep and care seat safety for infants througheducation during home and office visits.

Priority Highlights

DCHD responded to the e-cigarette or vapingproduct use-associated lung injury (EVALI) outbreakthat was  first recognized in August 2019 by theCenters for Disease Control (CDC). In light of thesevere  respiratory illnesses  seen in many teensacross the nation, DCHD staff promoted theadoptions of the MD Anderson ASPIRE program toeducate youth and targeted populations aboutvaping. DCHD collaborated with regional healthdepartments to create an anti-vaping campaignthat ran in 2019 to educate on the dangers ofvaping.
