    AbstractObjectivesThis report presents preliminary U.S. data on

    deaths, death rates, life expectancy, leading causes of death, andinfantmortalityfortheyear2004byselectedcharacteristicssuchasage,sex, race,andHispanicorigin.

    MethodsData in this report are based on a large number ofdeathscomprisingapproximately91percentof thedemographic fileand90percentof themedicalfile foralldeaths in theUnitedStatesin2004.Therecordsareweightedto independentcontrolcountsfor2004. For certain causes of death such as unintentional injuries,homicides, suicides, and respiratory diseases, preliminary and finaldata differ because of the truncated nature of the preliminary file.Comparisonsaremadewith2003 finaldata.

    ResultsThe age-adjusted death rate for the United Statesdecreasedfrom832.7deathsper100,000populationin2003to801.0deaths per 100,000 population in 2004. Age-adjusted death ratesdecreasedbetween2003and2004forthefollowingmajorcausesofdeath:Diseasesofheart,Malignantneoplasms,Cerebrovasculardiseases, Chronic lower respiratory diseases, Accidents (unintentionalinjuries), Diabetes mellitus, Influenza and pneumonia, Septicemia,Chronicliverdiseaseandcirrhosis,andPneumonitisduetosolidsandliquids. Rates increased between 2003 and 2004 for the following:AlzheimersdiseaseandEssential(primary)hypertensionandhypertensiverenaldisease.Lifeexpectancyatbirthroseby0.4year toarecordhighof77.9years.Introduction

    SourcesandMethodsDatasources

    Preliminary data in this report are based on records of deathsthat occurred in calendar year 2004 and were received and processedbytheCentersforDiseaseControlandPreventionsNationalCenterforHealthStatistics(NCHS)asofSeptember12,2005.Aftercomparisonwithindependentcontrolcountsofthenumberofdeathsfor 2004 that were received from state vital statistics offices,demographicinformationwasavailableforanestimated90percentofinfantdeaths(under1yearofage)andabout91percentofdeathstopersons aged 1 year and over (TableI). Medical or cause-of-deathinformation was processed separately and was available for anestimated 90percent of infant deaths and 90percent of deaths topersonsaged1yearandover.

    Toproducethepreliminaryestimatesshowninthisreport,recordswereweightedusingindependentcontrolcountsofinfantdeathsanddeathsaged1yearandoverfor2004receivedinthestatevitalstatisticsoffices. Two separate sets of weights were applied to the death

    Theproportionofrecordsprocessedbyeachstate isshown inTableIintheTechnicalNotes.Changesindeathratesbetween2003and2004anddifferencesindeathratesacrossdemographicgroupsfor 2004 were tested for statistical significance. Unless otherwisespecified,reporteddifferencesindeathratesarestatisticallysignificant.Detailed informationonthenature,sources,andqualificationsofthepreliminarydata isgiven in the TechnicalNotes.

    Causeofdeathisnotalwaysavailablewhenthepreliminarydataaresent toNCHS,but isavailable laterfor finaldataprocessing.Asaresult,estimatesbasedonpreliminarymortalitydatamaydifferfromstatistics from the final mortality data. Such differences may affectcertaincausesofdeathwherethecauseispendinginvestigation,suchas for Assault (homicide), Intentional self-harm (suicide), Accidents(unintentionalinjuries),andSuddeninfantdeathsyndrome(SIDS),orconditionsthatarehighlyseasonal,suchasInfluenzaandpneumoniaand Chronic lower respiratory diseases (see section Nonsamplingerrorin the TechnicalNotes).

    In addition to national and state estimates of total deaths anddeathrates,thisreportincludespreliminarystatisticsonlifeexpectancy,infantmortality,andcausesofdeath.Dataareshownforthefollowingraceandethnicgroups:white,non-Hispanicwhite,black,non-Hispanicblack, American Indian or Alaska Native (AIAN), Asian or PacificIslander (API), and Hispanic populations. Tabulations by race andethnicgrouparebasedontheraceandethnicgroupreportedforthedecedent.RaceandHispanicoriginarereportedasseparateitemsonthedeathcertificate.DeathratesforAIAN,API,andtoalesserextent,forHispanicsareknowntobetoolowbecauseofreportingproblems(seesection RaceandHispanicoriginin the TechnicalNotes).

    Age-adjusteddeathratesarebetter indicatorsthancrudedeathratesforshowingchangesintheriskofdeathovertimewhentheagedistributionofthepopulation ischangingandforcomparingthemortalityofsubgroupsof thepopulation thathavedifferentagecompositions.Allage-adjusteddeathratesarestandardizedtotheyear2000population (see section Computing rates and percentages in theTechnicalNotes).Twomeasuresof infantmortalityareshown, theinfant death rate and the infant mortality rate (see section Infantmortality in the Technical Notes). These measures typically aresimilaralthoughtheycandifferbecausethedenominatoroftheinfantdeathrateincludesacombinationofinfantsbornthepreviousyearwhohave not reached their first birthday and infants born before thereferencedateforthepopulationestimate,whereasthedenominatorofthe infantmortalityrate isallbirthsoccurringduring thereferenceyear. For example, the preliminary numberof livebirths for2004 isabout1.0percentgreaterthantheinfantpopulationin2004.Aninfantmortality rate isabetter indicatorof theriskofdyingduring the firstyearof life.

    Thisreportincludesdatafor12areas(California,Idaho,Michigan,Montana,NewHampshire,NewJersey,NewYorkCity,NewYorkState,Oklahoma,SouthDakota,Washington,andWyoming),which implementedthe2003revisionoftheU.S.StandardCertificateofDeathby

    2004, and the remaining states and the District of Columbia thatcollectedandreporteddeathdatain2004basedonthe1989revisionof the U.S. Standard Certificate of Death. The 2003 revision isdescribedindetailelsewhere(10,11).Inthisreport,reviseddataarecombinedwithunrevisedbutcomparabledata.Moredetailsonproceduresusedtocombinerevisedwithunreviseddataonracearegivenin the TechnicalNotes.

    The populations used to calculate death rates were producedunderacollaborativearrangementwiththeU.S.CensusBureauandarebasedonthe2000censuscounts.Exceptfor15states(California,Hawaii,Idaho,Maine,Michigan,Minnesota,Montana,NewHampshire,NewJersey,NewYork,Oklahoma,SouthDakota,Washington,Wisconsin,andWyoming),thedeathcertificatedatabyrace(thenumeratorsfordeathrates)arecurrentlyincompatiblewiththepopulationdatacollectedinthe2000census(thedenominatorsfortherates).Inordertoproducenationaldeathratesfor2003and2004,itwasnecessarytobridgetheincompatiblereportedpopulationdatatobeconsistentwith the1977OfficeofManagementandBudget (OMB)single racecategories (12), that is, to bridge multiple-race categories back tosingle-racecategoriesandreportthedataforAsianpersonsandNativeHawaiiansorotherPacificIslandersasacombinedcategory,APIs,andtoreflectageasof thecensus referencedate (13).Theproceduresusedtoproducethe bridgedpopulationsaredescribed inseparatepublications (14,15).As the remaining 35 states and the District ofColumbiagraduallybegintocollectdataonraceaccordingtothe1997OMBguidelines (16), it is expected thatuseof the bridgedpopulationscanbediscontinued.

    Readersshould keep in mind that the population data used tocompile the death rates by race shown in this report are based onspecialestimationprocedures.Theyarenot truecounts.This is thecaseevenforthe2000populationsthatarebasedonthe2000census.The estimation procedures used to develop these populations aresubjecttoerror.Smallerpopulationgroupsareaffectedmuchmorethanlargerpopulationsby thismeasurementerror,especiallyAIANs(14).ResultsTrends innumbersandrates

    Thepreliminarynumberofdeaths intheUnitedStatesfor2004was 2,398,343, representing a decrease of 49,945 from the 2003total. The crude death rate of 816.7 per 100,000 population was3.0percent less than the rate of 841.9 per 100,000 in 2003. Theestimatedage-adjusteddeathrate,whichaccountsforchangesintheagedistributionofthepopulation,reachedarecord lowof801.0per100,000 U.S. standard population, 3.8percent lower than the 2003rateof832.7(TablesAand1)(4).Theage-adjusteddeathrateintheUnited States has generally trended downward since 1900 withoccasional increasesusuallyduetooutbreaksof influenza,themostrecent being from 1998 to 1999 (17,18). From 2003 to 2004,age-adjusteddeathratesdecreasedby3.9percentformalesandby3.7percentforfemales.Allthesex,race,andHispanicorigingroupsdescribed in this report showed significant decreases in the age-adjusteddeathratebetween2003and2004.Therelativemagnitudeof thesechangesbysexand raceandHispanicorigin (Table1) are as follows.

    Among the major race and Hispanic origin groups, the lowestmortalitywas reported for theAPIpopulation (44percent lower thanthenon-Hispanicwhitepopulation), theHispanicpopulation (27percent lower than the non-Hispanic white population), and the AIANpopulation(19percentlowerthanthenon-Hispanicwhitepopulation).In contrast, the age-adjusted death rate for the non-Hispanic blackpopulationwas30percenthigherthanthatforthenon-Hispanicwhitepopulation (Table1). However, mortality for races other than whiteand black may be, in some cases, seriously understated due tounderreporting for some race groups and Hispanic origin on deathcertificates(1921).Statisticallysignificantdecreasesinmortalitybetween2003and2004wereregisteredacrossallagegroupsexcept514years,asfollows:Under1year(2.5percentseealsosectionon Infantmortality)14years (4.8percent)514years(2.4percentdecrease,notsignificant)1524years (2.8percent)

    Thedeath rate for Under1yearshownabove isbasedonapopulation estimate and is different from the infant mortality rate,whichisbasedonlivebirths(seediscussiononinfantmortalityratesinsectionon Infantmortality).

    Thepreliminaryestimateof lifeexpectancyatbirth for the totalpopulationin2004reachedarecordhighof77.9years.Thisrepresentsanincreaseof0.4yearrelativeto2003(seeTablesAand6).Record-highlifeexpectancieswerereachedforwhitemales(75.7years)andblackmales(69.8years),aswellasforwhitefemales(80.8years)andblack females (76.5 years). The gap between male and female lifeexpectancywas5.2yearsin2004,downfrom5.3yearsin2003,and5.4 years in 2002. The difference between male and female lifeexpectancyatbirthhasbeengenerallydecreasingsinceitspeakof7.8years in1979(22).Lifeexpectancyformales increasedby0.4year,reachingarecord75.2years.Femalelifeexpectancyincreasedby0.3year,increasingfrom80.1yearsto80.4yearsbetween2003and2004.

    Bystateofresidence,Hawaiihadthelowestmortalityin2004withan age-adjusted death rate of 623.6 deaths per 100,000 standardpopulation.MortalitywashighestforMississippi,withanage-adjusteddeathrateof998.2per100,000standardpopulation.Causesofdeath

    The15 leadingcausesofdeath in2004(TableB)remainedthesame as in 2003 with the exception of two causes that exchangedranks. Alzheimers disease, the eighth leading cause in 2003,became theseventh leading cause in 2004,whereas Influenza and

    pneumonia, the seventh leading cause in 2003, became the eighthleadingcausein2004.The15leadingcausesofdeathin2004wereas follows:1)Diseasesofheart;2)Malignantneoplasms;3)Cerebrovasculardiseases;4)Chronic lower respiratorydiseases;5)Accidents (unintentional injuries);6)Diabetesmellitus;7)Alzheimersdisease;8) Influenzaandpneumonia;9)Nephritis,nephroticsyndromeandnephrosis;10)Septicemia;11) Intentionalself-harm(suicide);12)Chronic liverdiseaseandcirrhosis;13)Essential(primary)hypertensionandhypertensiverenaldisease;14)Parkinsonsdisease;and15)Pneumonitisdue tosolidsand liquids.

    Theage-adjusteddeath ratedeclinedsignificantly for10of the15 leading causes of death. From 2003 to 2004, the preliminaryage-adjusteddeathratefortheleadingcauseofdeath,Diseasesofheart, decreased by 6.4percent. The age-adjusted death rate forMalignantneoplasms decreasedby2.9percent (seeTablesBand2). Deaths from these two diseases combined accounted for morethan 1.2 million deaths, 50percent of all deaths in 2004. Althoughheartdiseasemortalityhasexhibitedadownward trendsince1950,cancermortalityhasdeclinedonlysince1990.

    Thepreliminaryage-adjusteddeathratealsodecreasedbetween2003 and 2004 for Influenza andpneumonia (7.3percent),Cere-brovascular disease (6.5percent), Chronic liver disease and

    cirrhosis (5.4percent), Pneumonitis due to solids and liquids(5.1percent),Diabetesmellitus(3.6percent),Chroniclowerrespi-ratorydiseases (3.5percent),Septicemia (3.4percent), andAcci-dents (unintentional injuries) (1.9percent). Although there weredeclinesinthedeathratesforkidneydisease,suicide,andParkinsonsdisease, thesedeclineswerenotstatisticallysignificant.

    AlthoughHuman immunodeficiencyvirus (HIV)diseasewasnot

    among

    the

    15

    leading

    causes

    of

    death,

    itis

    still

    amajor

    public

    health

    concern. The preliminary age-adjusted death rate for HIV diseasedeclinedsignificantlyby6.4percentbetween2003and2004(Table2).From 1987 to 1994,HIVdisease mortality increased an averageof16percent annually. In 1995, the rate for this disease reached aplateau,notdifferingsignificantlyfromthe1994rate.Subsequently,therate decreased an average of 33percent from 1995 to 1998, and3percentfrom1999to2002.HIVdiseaseiscurrentlythesixthleadingcauseofdeathforallracescombinedfortheagegroup2544years.It is also the 9th leading cause of death for the age group 1524yearsanupwardmovementintheleading-causerankingsrelativetoits2003positionof10thplace for thisgroup(Table7). In2004,HIVdiseaseexited the listof top10 leadingcausesofdeath for theagegroup4564years.Increasesoccurred forEssentialhypertensionandhyperten-siverenaldisease(2.7percent)andAlzheimersdisease(1.4percent).The lattermoved fromeighth toseventhplace in the leading-cause list.

    Between2003and2004,thepreliminaryage-adjusteddeathratedeclinedby4.9percentforFirearminjuries,2.9percentforAlcoholinduceddeaths,and2.0percentforDruginduceddeaths(Table2).Theage-adjusteddeathrateforInjuryatworkdecreasedby4.5percentbetween2003and2004.

    The preliminary infant mortality rate for 2004 was 6.76 infantdeaths per 1,000 live births. The slight decrease of 1.3percentrelative to the rate in 2003 was not statistically significant (seeTablesA and 4). The neonatal mortality rate for 2004 decreasedsignificantly by 2.6percent relative to the rate in 2003. The mostdramatic decrease in neonatal mortality rate was observed for theblackpopulation (down4.6percent from2003;seeTable4).

    The infantmortalityratehaseitherdecreasedorremainedleveleachsuccessiveyearbetween1958and2001(2,6,23).Theincreasein2002wasconcentratedamongneonates,andmostoftheincreasein the infant mortality rate that year was due to the change in thedistributionofbirthsbybirthweight,particularlyanincrease ininfantsbornweighing less than750grams(24).

    The2004preliminary infantmortality rate forblack infantswas13.65infantdeathsper1,000livebirths,morethandoubletherateforwhite

    infants

    (Table

    A).

    Because

    of

    inconsistencies

    in

    the

    reporting

    of

    someracegroupsandHispanicoriginonbirthanddeathcertificates,infantmortalityratesforsomegroupsarelikelytobeunderestimated(20,21).Thelinkedbirth/infantdeathdatasetprovidesabettersourceof data for infant deaths and mortality rates by maternal race andethnicity(25).

    Althoughtheinfantmortalityrateisthepreferredindicatoroftherisk of dying during the first year of life, another measure of infantmortalityisalsoshowninthisreport;theinfantdeathrate(seesectionInfantmortalityintheTechnicalNotes).Thetwomeasurestypicallyaresimilaryet theycandifferbecause thedenominatorof the infantdeathrateincludesacombinationofinfantsbornthepreviousyearwhohave not reached their first birthday and infants born before thereferencedateforthepopulationestimate,whereasthedenominatoroftheinfantmortalityrateisallbirthsoccurringduringthereferenceyear.Forexample, thepreliminarynumberof livebirths for2004 isabout 1.0percent greater than the infant population in 2004. Thisdifferenceinthedenominatorsofthetwomeasuresofmortalityislargeenoughthatthedecreaseintheinfantmortalityratebetween2003and2004isnotstatisticallysignificant,whereasthedecreasemeasuredinthe infantdeathrate for thesameperiod is.The10 leadingcausesof infantmortality for2004were:

    1)Congenitalmalformations,deformationsandchromosomalabnormalities;

    2)Disorders related toshortgestationand lowbirthweight,notelsewhere

    classified;

    3)Sudden infantdeathsyndrome(SIDS);4)Newbornaffectedbymaternalcomplicationsofpregnancy;5)Newbornaffectedbycomplicationsofplacenta,cordand

    membranes;6)Accidents(unintentional injuries);7)Respiratorydistressofnewborn;8)Bacterialsepsisofnewborn;9)Neonatalhemorrhage;and

    10) Intrauterinehypoxiaandbirthasphyxia.Theonlydifferencesbetween the leadingcausesasshown for

    2003(4)andthosefor2004concernthedroppingoffofDiseasesof

    thecirculatorysystemfromthelist(itwaspreviouslythe10thleadingcause of infant death) and the incorporation of Intrauterine hypoxiaandbirthasphyxiatothe2004listasthe10thleadingcauseofinfantmortality.

    Theinfantmortalityratedidnotchangesignificantlybetween2003and2004fortheleadingcauseofinfantmortality,Congenitalmalformations, deformations and chromosomal abnormalities. There was,however,asignificantdecreaseof4.0percentinthemortalityrateofthesecond leadingcauseof infantdeath,Disorders related toshortgestationandlowbirthweight,notelsewhereclassified(Tables5and8).Deaths from these twoconditionscombinedaccounted formorethan10,000deaths,37percentof infantdeaths in2004.

    Amongtheleadingcausesofinfantdeath,asignificantdecreasein the infantmortality ratewasalsodocumented for the fifth leadingcause,Newbornaffectedbycomplicationsofplacenta,cordandmembranes.The ratedropped10.0percentbetween2003and2004.

    Deathsdue toSIDS,currently the third leadingcauseof infantdeath, have been declining since 1988 (8). The slight decline of3.2percentinthemortalityrateofSIDSbetween2003and2004isnotstatistically significant. The general consensus concerning recentdeclinesinmortalityduetoSIDSisthattheymostlyreflectachangeinthewaySIDSisdiagnosedandreportedbymedicalexaminersandcoroners (26).
