Daughters of Miriam Center/The Gallen Institute

Dear Friends,

We are pleased to present the 2013 ad journal. This ad journal is proof that we are a community who understands, values and supports this 92-year-young continuum of care senior facility. Despite this challenging economy, the 2013 ad journal raised a total of $210,000.

Daughters of Miriam Center/The Gallen Institute distinguishes itself by seeking and achieving accreditation from the Joint Commission on Accreditation of Healthcare Organizations, a private nongovernmental agency that establishes guidelines for the operation of health care facilities, conducts accreditation programs and surveys, and encourages the attainment of high standards of institutional medical care in the United States. Of the 314 long-term care facilities in the State, approximately 12% took those extra steps necessary to seek accreditation, a voluntary process. DMC is proud to be included in this elite group. In addition, we proudly note that we are the only Jewish long-term care facility in the entire State of New Jersey to meet the Commission’s high standards of care, far exceeding government guidelines.

DMC achieves such excellence because of its people. From the nurses, staff and administration, to the Board of Trustees and volunteers, the Center is filled with people who create a community of caring, dignity and respect, following Jewish traditions and values. Our parents and grandparents who live here depend on the staff and we, the donors, have many reasons to be proud of our support.

Renovation is ongoing as we strive to remain competitive in this ever-changing healthcare landscape. Naming opportunities still exist which offer a very meaningful opportunity for special donors and/or families. We are available to discuss these and other possibilities with you at any time, and invite you to tour the Center to see all these exciting changes.

We, the Board of Trustees, will continue to work hard to ensure that this facility will always be here to serve our Jewish community, providing these very necessary services for our seniors and fulfilling our responsibility and obligation to future generations.

To those of you who participated, now more than ever, we thank you. We look forward to continued and, possibly, increased support in the years to come. Let us all be proud that in these difficult times of government cutbacks our gifts are making a difference.

For 92 years, Daughters of Miriam Center/The Gallen Institute has served the senior community of Northern NJ. In this ever-changing healthcare marketplace meeting those senior needs is more challenging than ever before. Changes at the federal and state level, as well as influences much closer to home, are affecting how the Center does business. It is a constant balancing act to effectively respond to the current market while maintaining the long-range vision needed to insure our viability.

The country’s healthcare has experienced a radical shift. The implementation of the Affordable Care Act is sending ripples through the entire system, while its very future is in question as many in Congress seek its repeal. Funding is in question as federal and state lawmakers try to close budget gaps, looking to make cuts and find cost-saving measures. The increased financial pressure from an aging population is taxing an already stressed system. For Daughters of Miriam Center, staying abreast of these changes and finding a way to work within the system is a must.

In response to these changes, the Center joined the Care Associates Network. Founded in 1998, the Network was established to negotiate payer contracts with managed care companies. The Network also provides assistance with clinical development of other business relationships that

may benefit the Network. Each member has the autonomy to decide whether or not to accept the payer agreements or contracts as well as any other business relationships through the Network, which is owned by the members. The advantage for the Center is the ability to obtain higher reimbursement rates, better contract terms and assistance in proper payment of managed care claims.

At the state level, a major change will take place as NJ moves to a managed care system for

Medicaid long-term care. This could have a serious impact on the Center since Medicaid represents 67% of our revenue. This will be an experimental model, which gives the state more flexibility to provide services, improve delivery systems, improve care and reduce costs. In 2014 all new and existing Medicaid long term care residents will be enrolled and processed via managed care organizations. Currently, the Center receives $215/day, a rate set by the State for Medicaid residents. In the near future, insurance companies will establish the rates they pay facilities. What is not clear at this point is whether facilities will be able to negotiate with these managed care companies regarding reimbursement rates. If they are, our inclusion in the Care Associates Network will prove to be a strategic advantage.

One plan which is being considered would have a set reimbursement based on a diagnosis. For example, a hip replacement would be paid a flat rate. Hospitals and rehabilitation facilities would then have to split the reimbursement, knowing that there is finite amount of funding and that any expenses beyond that cap are not reimbursable. It is a shared risk model. If the hospital and rehabilitation facility can keep the expenses under the reimbursement, they will profit, but there is always the risk, especially high with seniors, that expenses will exceed the reimbursement. In this model, the synergies that Daughters of Miriam Center has developed with area hospitals will be critical. The seamless transition from acute to subacute care will be essential to managing any potential complications and keeping expenses in check.

Nursing stations are open and positioned directly opposite the dining rooms to allow for a more efficient operation.

Physical and occupational therapies allow the subacute patient to return home after relearning how to do activities of daily living.

The Center already has a long-standing, close relationship with St. Mary’s Hospital in Passaic, which is the primary source for referrals into The Gallen Institute for Subacute Care. The sale of this hospital to the California-based company, Prime Healthcare Services, is pending approval at the State level. Prime Healthcare Services is a hospital management company which owns and/or operates 25 acute care hospitals across the country. They are known for transitioning non-profit facilities into for-profit models and, while the purchase contract states that they must maintain St. Mary’s non-profit status for five years, it is likely that they will transition to a for-profit business once the contract expires. The Center has maintained close contact with the hospital leadership during this takeover.

While we must respond quickly to the current changing marketplace, it is still important to take a far seeing approach to ensure our long-term survival. To maintain that balance, the Board of Trustees has re-established the Strategic Planning Committee. This committee, under the chairmanship of Board of Trustee member Len Thaler, is looking to the future of the Center. Their mission is to explore all possibilities and present the Board of Trustees with options. This includes looking at how to best utilize our physical plant as well as ways in which to tailor our

services or create strategic partnerships to meet the needs of the emerging market.

One of the ways the committee is looking to solidify our position in the local market, is by strengthening our relationships with other acute care facilities. We have a great deal to offer in the high quality of services we provide. One of the key factors which both managed care providers and Medicaid monitor for hospitals is the percentage of patients who are readmitted after

release to the community or another facility. There are reimbursement penalties for the hospitals when patients are readmitted within a 30 day time frame. The Center’s low readmission rate is an advantage to acute care facilities which are looking to avoid those penalties. While the penalties are currently low, there is a strong likelihood that they will increase as government and insurance payers look to further contain costs. This increases the value of our services as we move forward in establishing strong relationships.

Government and insurance payers are not the only agencies looking to contain costs. The Center has worked very hard to keep its own expenses in line. The union contracts were up in 2013 and the Center successfully renegotiated the contract with a 2% salary increase, balancing the need to maintain costs with the desire to compensate our employees fairly. A consistently strong census combined with careful monitoring of expenditures has kept the Center on budget for the year, allowing us to maintain our current reserve funding as insurance for the future.

While the times we face are more uncertain than ever, the leadership of the Center continually balances an ability to respond quickly to the current climate with clarity of vision for our future. The Board of Trustees and administration recognize that the services we provide are critical to our community and are committed to the Center’s mission and survival for generations to come.

The Rehabilitation Department is open seven days a week to ensure steady progress in patient recovery.

Private and semi-private accommodations are well-appointed and feature custom closets and comfortable furnishings.
