How we’ll use the information
• To provide this information back as groups continue to work to improve care
• To conduct further research as to what models of integration “work” and for whom
• Outcomes important to patients with mental health concerns and their families
• Outcomes important to those who deliver care in health systems and the community
• Outcomes important to those committed to the health, education, and safety of the public
What do we mean by “what models work”?
Incentives to participate
• Include the voices of your organization and those it serves in improving care
• Keep the tablet or receive $250 for your organization
• Help the 1 in 4 people of Dane County who are affected by mental illness
Stay informed and stay in touch!
