Resection in saline (NaCl) is mainly used inurology for prostate and bladder resection.The procedure is also used in gynecology formyoma and endometrium resection as anadditional alternative to monopolar resection.The key difference is the use of saline (NaCl)instead of glucose solution.Saline is used as an irrigation medium andconductor, contributing to even greater pa tientsafety because a neutral electrode no longerneeds to be connected to the patient’s body.The current does not therefore flow throughthe patient’s body but is conducted to theelectrode instead.Resection benefits from the generation ofplasma to achieve outstanding cutting power.A short outline of the process is providedbelow:• An electric current heats the saline solutionand tissue via the loop

• A bubble is generated as the saline evapo-rates

• The current ionizes the gas bubble to gene-rate plasma

• The plasma ignites and the loop is illumi-nated bright yellow

• The tissue evaporates and the loop electro-de starts cutting

This method offers the following main advan-tages:• shallow coagulation • minimal in-depth thermal damage • no impairment of histologic quality• reduced irritation of the urethra (both ther-mal and mechanical)

Working elementsThe new Richard Wolf S(a)line Resecto s co -pes are compatible with all resectoscope sheaths from 22 Fr. or 24.5 Fr. (continuousirrigation resectoscope sheaths), 4 mm stan-dard telescopes from 0° to 30° can also beused. A new electrode guide permits the

combination of a working element with both0° and 30° telescopes.It is even possible to use the world’s narrowestcontinuous irrigation resectoscope shaft measuring 24.5 Fr.A special "easy-click" lock between the elec-trode and the working element makes chang -ing the electrode particularly fast and user-friendly.A neutral pin at the proximal end of the wor-king element takes over the function of a neu-tral electrode and hence conducts the currentthrough the bipolar cable.

ElectrodesSpecial electrodes for the bipolar resection ofthe prostate and bladder tumours are availa-ble both as disposable and reusable electro-des. For use in the prostate, we recommendelectrodes with a wire Ø of 0.3 mm with astrong haemostatic effect. Electrodes with awire Ø of 0.25 mm were designed speciallyfor working in the bladder where they achievean extremely sharp and smooth cut. The con-struction of the electrodes ensures that thecurrent flows from the loop to the electrodefork without causing any thermal injury in theurethra.

R. Wolf offers customers genuine 22 Fr. elec-trodes. The cutting characteristics of theseelectrodes combine with plasma formation topermit removal of much more material.

"BiVAP"Bipolar Vaporization ElectrodeA special shape of electrode head was specially developed for bipolar vaporizationtechnology. The specific geometry of the electrode surfacemeans that the high-energy plasma can beused in concentrated form for fast, effectivetissue vaporization with minimal loss ofblood.

Morever, the "stepped" gradation at the contact area of the electrode enables theenergy density to be kept as low as possibleand this avoids thermal damage to tissue.

No bridges are left behind during vaporizationwith our BiVAP electrode as a result of its planar bearing surface. A completely flatcompartment is created.

HF cable A special HF bipolar connecting cable (3 m or 5 m long) is naturally also providedfor use in resection. Apart from the appliance plug, the cable isalso equipped with a black instrument plugfor the HF connection and a grey instrumentplug for the neutral pin.
