Clinical Experience

Out Patient Clinic : 4 sessions per weekWe have a busy outpatient clinic as our department covers a large catchment population. An average of 14 to 15 patients per clinic seen by me includes combination of new and review patients. My clinic schedule includes1. Joint Oral surgery / Orthodontics clinic - 2 sessions per month Consultation for a) Exposure of unerupted canines b) Removal of severely impacted canines, unerupted supernumerary teeth along with routine orthodontic extractions. c) Labial frenectomy 2. General clinic - 3 sessions per week Majority of referral covers wisdom teeth extractions followed by soft tissue oral lesions, medically compromised patients for dental extractions, complicated non wisdom teeth and failed extractions, management of complications of dental extraction, nervous patients requiring general anaesthetic, cyst management, second opinion on radiological findings and patients for apicectomy.Oral medicineA wide range of oral medicine patients are referred from Dentists & General practitioners to our department for management. As part of my preparation for the fellowship exam in Oral surgery which includes oral medicine, I have developed my interest in the diagnosis and management of patients with oral lesions. With the knowledge gained by constant updating and preparing for my lectures I have improved my diagnosis accuracy and appropriate management of these patients.Treating medically compromised patients As increasing number of medically compromised patients being referred to the secondary care for dental extractions and their management. The wide range of exposure and experience has given me full confidence in management including pre-operative precautions and anticipated post-operative complications following oral surgical procedures. Facial pain and temporomandibular disorders- 1 session per weekAn average of 12-14 patients with facial pain /TMJDS will be seen by me in the clinic and I have developed interest and skills in diagnosing and managing these condition.

Minor oral surgical procedures performed independently under local anaesthesia, IV Sedation and general anaesthesia.

o Third molar and non third molar extractionsPage 4 of 14

o Management of complications of extraction including closure of oro antral fistula, tuberosity fracture, retrieval of root from sinus etc.

o Coronectomy of third molars

o Apicectomy with retrograde root filling +/- granuloma / cyst removal

o Management of odontogenic and non odontogenic cyst

o Management of odontogenic and other oral infections-Incision and drainage of dental abscess, debridement of infected wound etc.

o Incisional / excisional biopsies of oral mucosal lesions

o Labial gland biopsies

o Management of salivary gland conditions - stone retrieval and duct dilatation

o Cryotherapy

o Exploration of hard and soft tissues

o Management of dentoalveolar trauma patients including wiring and splinting the teeth etc.

o Alveloplasty

o Exposure and bonding of canine teeth

o Surgical removal of impacted and supernumerary teeth

o Extraction of deciduous teeth including submerged teeth

o Labial and lingual frenectomy

Previous Experience : SHO level - 6 years

I have been exposed to a wide range of Oral and maxillofacial conditions during my SHO training which includes Head & Neck pathology , facial deformity congenital defects and facial trauma. I have also been involved in fracture reduction & fixation, abscess drainage, and management of patients with acute maxillofacial emergencies under guidance.

I have assisted in Major Head & Neck surgical procedures including neck dissection, micro vascular reconstruction, using free flaps, salivary gland operations, repair of cleft lip & palate with iliac bone graft, osteotomy and temporomandibular procedures.During my period in the Leeds teaching hhospitals and Leeds Dental Institute I had opportunity to work under cconsultants with specific interests in the above field. I have also developed my knowledge and improvised my skills in the dentoalveolar surgeries with the help and support from my senior colleagues including procedure under local anaesthetic and intravenous sedation.During my career I have learnt to prioritise my duties, understand my limitations, seek help when appropriate and improved my communication skills and record keeping. I acquired knowledge and experience in treating patients with complex medical problems and dealt with medical emergencies in oral surgical setup.

Additional Activities:

Clinical supervisor and tutor for junior trainees In our department, I am actively involved in supervising and guiding the junior trainees in order to help them to gain confidence in treating oral surgery patients. I also conduct minor oral surgery and clinic sessions with junior trainees under the instruction of consultants and work together on patients under local and general anaesthetics to help them to gradually develop confidence and improve the surgical skills.

I organise and deliver weekly tutorials to the junior members and nursing staffs in the view of helping them to prepare for the membership exams and to discuss current thinking and evidence based approach.

Clinical governance activities

Regular attender of clinical governance meetings and work together with my colleagues to improve quality of service and ensure high standard of care to the patients treated in our department

So far my contribution towards achieving the above goal includes regular course attendance including mandatory training, involve and support junior trainees in audit projects, to implement evidence based approach, identify and support risk management in our working environment.Since 2008, I have developed number of protocols and policies for our department in the view of providing standardised care to our patients in treating a particular condition.

1. Antibiotic prophylaxis before dental extraction to prevent infective endocarditis-October 2008

2. Steroid cover before Dental extractions - October 2008

3. Antibiotic prophylaxis before dental extraction for patients on bisphosphonates - November 2009

4. Follow-up policy in outpatient clinic - March 2010

5. Surgical safety checklist for minor oral surgical procedures-May 2014

Teaching Experience

I have developed the teaching programme in our department from August 2008 for the SHO/DF2’s with weekly lectures session aimed to help them to prepare for the membership exams and ensure they were kept up-to-date with the current developments in oral surgery.

The teaching session covers a wide range of oral surgery topics, including discussions of papers and journal club meetings under the supervision of consultants.

I have lectured along with my consultant to the local dentists organised by the Deanery, and planned to continue delivering periodic lectures in the future. I took every opportunity to seek advice and suggestions from my senior colleagues, and always wanted to give my best input to deliver precise and informative lectures in order to reach Individuals with varied levels of exposure in the field of oral surgery.Due to my academic interest and passion towards teaching, this opportunity and responsibility has given me encouragement to update regularly with a view to prepare for these lectures.

I have also attended relevant courses in relation to teaching and mentoring to develop my skills.
