Message from

2004 was a milestone year for Hospice of Green Country. In just a few short months, the staff, along with strong board leadership, turned the financial status of the organization from vulnerable to solid. Dedicated volunteers and employees of our organization pulled together and implemented several cost-saving measures; all without sacrificing the quality of patient care HGC is renowned for providing.

Hospice of Green Country clinical staff and volunteers assist our patients and families to make every moment count; this past year, we realized the value of bringing the same philosophy into our business practices. Every dollar spent must count toward facilitating the organization in living the mission of providing compassionate and quality end-of-life care to patients and families, regardless of their ability to pay.

As Hospice of Green Country moves into the future, we humbly thank all of our donors and friends for their support. It is only through such generosity that the staff and volunteers experience the privilege in assisting patients and families in making every moment count.

Hospice of Green Country is dedicated to providing compassionate and quality end-of-life care to patients and families—regardless of their ability to pay.

Hospice cares for the whole family—not just the patient. Patients’ medical, emotional and spiritual needs are supported in the comfort of their own homes or home-like settings. This allows patients to be surrounded by their loved ones during their remaining days—making every moment count.

Hospice of Green Country is the only community based, non–profit hospice, supported by the Tulsa Area United Way. Currently, more than 40 hospice organizations provide services in our 50-mile radius service area. The majority of them are either

affiliated with a particular hospital system, have a specific religious focus or are for-profit companies.

Hospice of Green Country is one of four hospices in northeastern Oklahoma to achieve the rigorous standards of accreditation by the distinguished Joint Commission on Accreditation of Healthcare Organizations (JCAHO). Our 501 c (3) organization also is licensed by the State of Oklahoma and is Medicare approved.

For most patients, the full cost of hospice care is covered by Medicare or private insurance. In 2004 Hospice of Green Country provided Medicare and private insurance reimbursed hospice care for 272 patients.

Palliative care refers to managing the physical, emotional and spiritual care needs of people who are diagnosed with incurable, progressive illnesses. The goal of palliative care is to achieve the best possible quality of life through pain management, symptom control and restoration of functional capability. Sensitivity to patients’ personal, cultural and religious values, beliefs and practices is observed.

Since its founding in 1985, Hospice of Green Country continues to be northeastern Oklahoma’s leading hospice in meeting the needs of the poor through its Courtesy Care services.

In 2004, Hospice of Green Country provided $373,546 in end-of-life care to indigent, terminally ill Oklahomans at an average cost of $104.05 per patient per day, which is down from $116.42 a day in 2003. Through careful management and

effective cost–cutting measures, our dedicated leadership, staff and volunteers helped control costs without compromising patient care.

Many of the unemployed and those living below poverty level have general health coverage under the Oklahoma Medicaid benefit. However, Oklahoma is one of six (6) states in the country without hospice coverage under that state supported health benefit.

Hospice of Green Country’s Courtesy Care program enabled a 46-year-old dying man to be reunited and lovingly cared for by his brother and sister-in-law, during the final weeks of his life. Michael (not his real name) had been estranged from his brother and other family members for many years until one sad October day when he found himself homeless, alone, blind and dying from the ravages of uncontrollable diabetes. Hard luck and a series of life crises contributed to Michael’s being without his life-sustaining insulin for too long a period of time. When Michael finally returned to Oklahoma, so he could die surrounded by his family, both he and his family members requested hospice care from our team of qualified and compassionate professionals.

Thanks to generous supporters of our Courtesy Care program, quality end-of-life care is available to patients like Michael and families, regardless of their ability to pay. Our dedicated hospice team ensured that Michael’s medical, physical, emotional and spiritual needs were met during his final weeks of life. Hospice of Green Country provided care for Michael with daily visits from registered nurses qualified in pain control techniques and a social worker who helped with nursing home arrangements, and provided emotional support to family members. And in his last hours, when Michael was actively dying, our compassionate chaplain was doing everything possible to ensure Michael’s spiritual wishes were honored.

The Live Alone program allows patients, who live alone or have limited access to a caregiver, to maintain control of their lives by residing in their own home and having access to special services that support their safety and quality of life.

In 2004, Hospice of Green Country cared for 36 Live Alone patients, with an average cost per patient of $856.75. To ensure peace of mind and immediate access in an emergency, security and Life Line services are available for Live Alone patients.

Without access to end-of-life care terminally ill patients, who do not have hospice benefits, will likely die in pain. During a crisis, those patients often seek pain relief and symptom control in hospital emergency rooms and are then admitted to acute healthcare settings. Hospitals are a more costly environment for treatment, the price of which is often offset by taxpayer dollars or cost-shifted to the insured.

Meeting Community Needs

Live Alone Program When Hospice of Green Country began taking care of Mr. R, an 84-year-old man with end-stage respiratory failure, his wife of over 50 years was his loving and compassionate caregiver. Tragically, one night his wife was severely burned in a freak accident at home. She was immediately rushed to the hospital and died a few days later.

Mr. R. not only lost his wife, he lost his caregiver and faced an immediate transfer to a nursing home. During this difficult time, Hospice of Green Country was able to care for him at home through the Live Alone program. He received specialized nursing care, visits from a social worker and chaplain and remained in the comfort of his own home.

Grief is a journey or a process and everyone’s journey is personal and unique. Understanding grief, especially anticipatory grief, is important because it affects both the dying person and his or her family.

Upon learning that a patient’s disease is terminal, a family begins the anticipatory grief experience. A bereavement counselor helps family members become aware of their own needs, improve communication and better express feelings. Hospice also helps facilitate closure for patients

by helping families understand and meet patient’s goals, desires or last wishes.

After the death, our grief assistance services include personal follow-up and individualized counseling for 13 months with the patient’s family or caregivers, helping them learn to grieve, heal and live again.

In 2004, 328 family members and loved ones received grief assistance through our Bereavement department.

Studies reveal that, given the choice, 80% of people choose to die at home or in other familiar settings. Without the support of specialized hospice care, patients often will die in a hospital setting, in unfamiliar surroundings and without family support.

Hospice of Green Country encourages everyone to discuss their personal wishes regarding healthcare crises and end-of-life care, and to complete a signed and witnessed Oklahoma Advance Directive or Living Will.

End–of–Life Choices

A Grieving Process

Trained volunteers, along with our professional, medically directed staff, provide a unique and specialized approach to healthcare for patients and their family members during the difficult time of coping with a terminal illness. Volunteers provide transportation, run errands, make deliveries, provide relief for caregivers, provide emotional support or companionship for patients and/or family members, assist the family with grief issues, help document a patient’s life history and provide clerical support.

In 2004, more than 80 Hospice of Green Country volunteers contributed 7,138 hours of service, which included: making 2,843 contacts with patients, driving 65,201 miles on behalf of patients and fulfilling more than 2,002 requests from our patients and staff. Substantial cost-savings were realized for the organization thanks to our team of dedicated volunteers, who contributed volunteer time to our agency.
