    ABOUT THISHANDBOOK

    We have created this handbook to answer any questions you may have about the SLIM STUDIO andCSA dance. It contains important yearly information on our policies and procedures; outlining ourphilosophies and expectations and offers dancers and parents a clear understanding of theirresponsibility and commitments to the CSA dance program. Our policies and procedures are here toensure that all students and their families are treated fairly and equally.

    CSAHISTORY AND GENERAL INFORMATION

    CSA HISTORYCENTER STAGE AMERICA was established in the 2001. It was located inthe small town of Ephraim, UT. In 2005, the dance studio moved to

    West Jordan, and this year after 5 more years of growth and success,the studio moved to its current location in Midvale. We attribute oursuccess as a result of providing our students with a solid, technicaldance foundation, running an organized business and an underlying beliefin the philosophies of our program.

    Center Stage America Dance is part of the SLIM STUDIO located in Midvale Utah. We areexcited to announce the expansion of our studio and are looking forward to a great turnout!

    Our professional staff is looking forward to helping children and adults find a love of dancewhile also pushing them to become stronger more advanced dancers! We at CSA offer a widevariety of dance styles and classes for all ages, varying from age three to adult classes. As partof SALT LAKE INSTITUTE OF MOVEMENT we will be offering competition dance classes as wellas more recreation dance classes for those who are not interested in the competitive side ofthings. We offer great classes at a competitive price and hope you will join our growing family!

Our staffconsists of professional dance instructors with various years oftechnical training, BFA degrees in the field of dance, collegiate andprofessional performing experience as well as years of teachingexperience. Under the direction of MaryAnn Champman, our faculty iscommitted to excellent technical training in classical ballet, jazz, modern,tap, hip-hop, ballroom, tumbling and more.

    CSA understands the importance of providing a professional studiospace for our dancers. All of our spacious studios have large mirrors,secure Barres and a professional floor. This combination helps protectour dancers from fatigue, pain and injury.

    CSA DANCE is located in Midvale just south of 5400 south and StateStreet. Our studio location offers easy access for drop off and pick upas well as plenty of parking.

    Please Remember

    Cooperation between all parents is expected.Showing respect for the other parents, along with the students andfaculty, makes an important impression on the children.

    You are a role model for your child in how to interact with others

    in a professional setting.Your childs presence at all classes is important.The spirit of teamwork and the lesson of dedication are a big partof our studios educational process.

    Parents and teachers may look at a childs learning from differentperspectives. However, they share a common goal: to assure thatevery child receives the best possible training, both physically and

    mentally. Mutual respect between our faculty and our dancers parents

    provides the children with the ultimate care and education.

    As a parent, the financial support of your student is important, but ofequalperhaps even greatervalue is your emotional support.

    Encourage your child to be the best that he or she can be without regardto what others may achieve. Dance is an individual art form; each childneeds to achieve at a pace thats comfortable for him or her. No twostudents will progress at the same rate, even if they experience the exactsame training.

    Some dancers who are placed in a higher level become discouraged,only to lose their passion for dance. Others respond to the challengeof being in a class with dancers who are more proficient by pushingthemselves to work harder. Placement is highly individual and the factorsthat go into the decision are complex.

    If you have questions or concerns about your childs placement ordance education, please do not hesitate to speak with the instructors ofthe class or Maryann herself.

    If you do request a conference, please listen carefully to what your childs

    We ask that all parents and those who are dropping students off and orpicking them up at S.L.I.M studio please do so in the East Parking lot,

    located behind the building. This lot can be accessed by turning at 6790 S(East Lester Ave). Parents please park in a designated spot and wait foryour child. Please DO NOT pull up and wait in front of the studio door.

    The parking spots located at the front of studio are reserved foremployees of the Salt Lake Institute of Movement and our businessesneighbors. Please be courteous and DO NOT park here for pick up anddrop off. This entrance is NOT the public entrance or exit and we ask that

    you please be sure to respect the parking policies of the studio.

    Parents are responsible for their children before and after class times.Please do not drop your child off more than 10 minutes prior to class.Prompt pick up (within 5 minutes) is also expected.

    Parents are welcome to watch class from the waiting rooms or hallwaythrough the observation windows. Only dancers who are enrolled in theclass are allowed in the classroom during class time.

    We understand at times this cannot be helped and we have provideda small waiting area room for those parents with children who arewaiting for their student.

    If a child is found to be using disrespectful, dangerous or inappropriatebehavior such as sitting or kneeling on lobby tables, jumping on and fromchairs, running, playing relay or hiding games, rough housing, throwingfood, clothing or other items, playing in the bathroom, distracting classesin session, defacing studio property, or other non-acceptable behavior,they will be given a written warning that will be sent home to you.

    If the child receives a 2nd

    written warning, you will be requested to havea conference with their teacher or the studio director before they canreturn to the studio for classes.

    If the child receives a 3rd

    written warning, they will be removed from allof their classes at Slim Studio/CSA DANCE.

    If the child is a sibling who is not attending classes, you will be asked torefrain from bringing the child to the studio while waiting for your dancer.

    Please know that this program is not instituted lightly, nor will it be usedto judge or inhibit your childs appropriate behavior at the studio. It will,however, be a strictly enforced program, especially when children are

    endangering themselves or others.

    Please take this opportunity to speak candidly with your children aboutthe appropriate behavior at the studio. Discuss in detail with them whatis not appropriate, and the consequences of receiving a warning from thestudio.

    We want to provide the best possible facility, teachers, programs andenvironment for your childs dance education. For this reason we askthat you help us by following the rules and guidelines we have set forthin this handbook. If you have suggestions or concerns regarding any of

    the information in this handbook, please contact the office manager, ormake an appointment to speak with me. We truly value your support ofthe UDA dance program. Thank you for your attention and dedication toyour childs dance education.

    DANCER ETIQUETTE AND GUIDELINES

    The first step to becoming a successful dancer is making a solidcommitment to your dance classes. Strong technique is a key ingredientand class is where you develop that technique. Come to each classprepared to learn and without personal distractions. Class time is yourchance to focus on yourself, so try to leave your concerns or worries atthe studio door. True progress is made when you look at each class as anopportunity to become better at what you love to do. Dance full out,stretch a little further, become more aware of your technique, and makethe most of every class.

    Respect for the faculty and teachers you work with are essential. Listento each correction given, whether its directed to you or another dancer.A correction is an honor; it shows you how much a teacher caresabout your progress as a dancer. Always say thank you when a teacher

    offers you constructive criticism. Nothing can stop those students whoapply themselves in every class and appreciate their teachersknowledge and experience.

    ATTENDANCE: Students are expected to be at all dance classes. It isimportant in the learning process that students are there so they donot fall behind or delay the progress of the class.

    DRESS CODE: Wear proper class uniform, shoes and hairstyle. (See

    Dress Code section for more details).

    ABSENCES: If a student is going to be absent or late, a phone calland or email should be made to notify the office or the studentsinstructor. This will relieve any concerns that we might have of whereyour child may be.

    BE ON TIME FOR CLASS: Students should arrive five minutes early to

    class to get themselves fully situated before class begins. If a studentarrives late to any class they should show courtesy and respect byasking the teacher permission to participate in class.

    RESTROOM BREAKS: Use the restroom before class begins.

    CLASS BEHAVIOR: The CSA Discipline Policy will be strictly enforced,

    and lack of adherence to its guidelines may result in removal from theprogram. Students are expected to manage their own behavior.

    CELLULAR PHONES: Students cell phones should never be turned on

    while in the studio where a class is being taught or lefton in the dressing room lockers. Constant ringing andvoice mail alerts are disturbing to other patrons in the

    lobby area.

    RESPECT THE TEACHER: All students are expected to follow the

    directions of the teacher and to show utmost respect. Students shouldrefrain from talking or visiting with classmates during class time. Thisis disrespectful to the teacher.

    RESPECT THE STUDIO:

    Students are also expected to contain their dance clothing and gear

    in a dance bag which should be stored in a cubby at all times. Itemsthat are left strewn throughout the studio and floor will be deposited intothe Lost and Found.

    o No gum, food and drinks are allowed in the dance studios.

    o While taking class students should never stop dancing during anexercise unless they become ill or injured.

    o

    Students should never sit down unless instructed to do so.o Students should refrain from leaning, hanging, sitting or swinging

    on the ballet Barres.

    o Students should always show respect for others in class by

    keeping their hands to themselves and staying in their owndancing space.

    o Students should never leave a classroom to get a drink, use therestroom, or any other reason without asking the

    teacher permission to do so.

    o Students should always end class by eitherapplause or a personal thank you to their teacher.

    Grooming is essential to our professional image. Dancers who come toclass looking and feeling like dancers behave accordingly.

    At CSA we follow a specific dress code to promote uniformity and asense of professionalism as well as structure and discipline in theclassroom.

    All new Pointe students (Pre-Pointe class) should NOT purchaseshoes prior to their first class. Students who already have themmay bring them.

    Pointe shoe fittings are highly individual, and will be discussed in detailin class.

    Your students instructors can helpgive you advice on options aboutwhere to buy etc.

    Bear paws and foot undies may be worn by the students for lyrical andcontemporary classes if teacher has okd them.

    Students in hip-hop classes may choose to wear sneakers to practice in,however we ask that these shoes be specifically for dance (please do notwear them anywhere but in the studio).

    Students who wish to participate in our ballroom classes may choose topurchase ballroom foot wear, but it is not required. (soft soled shoes are

    allowed)

    Students are allowed to use any tap or jazz shoes for class, however, all

    recital shoes will be ordered if necessary (if black and/or tan needed)

    Students are required to keep all of their belongings inside a dance bag.Please make sure that this dance bag is large enough to keep everythingin and that your child only brings the things that are necessary to danceeach week.

    A locker room is provided so that bags and belongings can be kept safelyinside.

    All dance items (shoes, leotards and leggings etc) should be markedwithyour childs first and last name before their first dance class. This will

    save us from headaches and tears

    There is a LOST and FOUND at the dance studio behind the main desk.Items that are left on the floor, in the studios, or loose in the lobby willbe deposited into the Lost and Found.

    Items not recovered from the Lost and Found within 90 days are donatedto Deseret Industries. If your child is missing any of their dance items,please check the LOST and FOUND before you purchase replacements.

    This year we will be offering the following dance clothing andmerchandise:

    Hot shorts

    CSA Tees

    Ballet Shoes

    Jazz Shoes & Pants

    These items will be available to order at registration and multipletimes throughout our year. All items require payment at time oforder placement.

    REGISTRATIONRegistering for our classes indicates commitment to the en tire d anc ey ea r as outlined in this handbook.

    All registrations must be by. The

    completed registration form, liability waiver, media release, andaccount contract must all be completed and signed by theaccountholder in the studio.

    Only siblings living in the same household may combine onone account. Multiple families, even those living in the samehousehold must set up separate accounts.

    The Liability Waiver Form must be signed waiving the right toany legal action for any injury sustained on CSA property resulting fromnormal class activity or any other activity conducted by students before,during or after class time.

    The following fees are due with all registration paperwork inorder to enroll any student: registration fee $25 per student ($35 perhousehold) First month free. No Refunds.

    Class drops made after the 1st

    of month will result in any tuitioncredit for that month. Class additions made during the month will result

    in a pro-rated tuition charge for that month.

    Any student withdrawing from a ll CSA classes must submit ato the office before the end

    of the tuition month. Without written notification,tuition will continue to be billed, even if the student isnot attending class. Withdrawals from single classesmust be submitted in written form prior to the lastbusiness day of the month. No credits will be givenfor untimely notification.

    Withdrawal notices submitted after the 1st

    of the month will

    result in a refund or credit for that month.

    Students wishing to withdraw by the end of the month must submit acompleted Withdrawal Notice by the 15

    th.

    Make-up classes are available with the following stipulations:

    The make-up class will be scheduled for the same or lower skill levelas the missed class. Students may not attend a HIGHER level classfor make-up purposes.

    , andare subject to change after recital preparation has begun. CSA reserve the right to remove the privilege of make-up classes, if

    that privilege is abused.

    CSA reserves the right to add or drop classes based upon enrollment, as

    well as change the class instructor if necessary. In the event that aregular instructor must miss a class, a qualified teacher will be brought inas a replacement.

    In the event of inclement weather, classes may be cancelled.CSA will cancel dance classes on days that schools areclosed due to bad weather. If you have any questionsabout classes being cancelled, you should call the studio tohear a phone message if classes have been cancelled.Notifications will also be up on the website for easy access.

    If classes are ever cancelled due to weather or for other reasons ofunseen circumstances, a make-up class will be offered andaccountholders will be notified by email of the upcoming make upschedule.

    THE SESSION.

    We at CSA have found it is easiest to have card information in our systemso that payments can automatically be taken out at the correct time each

    month payment is due. This helps us to receive payments on time andtakes away the stress of having to remember to get the payment in eachmonth. Accountholders are responsible for providing up-to-date creditcard information prior to all payment due dates (i.e. expiration dates,account changes, lost cards, etc). All card holders are responsible forcontacting the CSA office before the last business day of the expirationmonth, and providing the new expiration date for their card. Graceperiods will be applied to credit card expiration defaults.Accountholders who do not provide up to date expiration dates prior to

    the 1st

    of the month will be charged a non-refundable late fee.

    PAYMENTS RETURNED FORNON-SUFFICIENT FUNDS WILL RESULT IN CREDIT CARDCHARGES OF THE BALANCE OWED PLUS A $25.00 NSF FEE. ACCOUNTSEXPERIENCING TWO NSF FEES WILL HAVE THEIR ACCOUNT STATUS MODIFIED TOCERTIFIED CHECK, MONEY ORDER, CASH OR CREDIT CARD PAYMENT ONLY.

    All monthly tuition payments will be billed the 5th of each month. If the5th occurs on a week-end or holiday, credit cards will be billed on thefirst business day following the weekend or holiday. (Credit cards willbe automatically charged each month.)

    Refunds for overpayment of tuition and/or fees will be given only forapproved medical withdrawals.

    Invoices/statements will n ot be ma ile d m o n th ly . Statement copies may berequested by email or in the office on an individual, as-needed basis.

    o There are no refunds on missed classes.

    o Classes that are missed cannot be credited to the following monthor subtracted from the monthly tuition.

    o There is no tuition reduction or refund for classes that fall onscheduled holidays or breaks (see Studio Holidays).

    If your family experiences a financial emergency, please contact theoffice BEFORE your payment is due to make payment arrangements.

    Arrangements will be established on a case by case basis.

    A c c o un ts w ith balance s of 30 days or m ore w ill r e s u lt in s u sp en sion fr omthe progra m ; a notice will be e-mailed to you temporarily withdrawingyour child from classes until all fees are paid. No credits are given forclasses missed due to lack on on-time payment. This policy is stronglyenforced.

    Accounts having overdue balances will be assessed a non-refundable late fee of $25.00 or 5%, whichever is greater.

    PROGRAM FEES

    $25.00 per student or $35.00 perfamily (three or more students) This fee initiates your account,

    reserves your childs space in class and is non-refundable.

    $50.00 per account. CSAperforms their annual Spring Recital each June. The recital w i l l beperformed at Murray High School, 5440 S State Street. A non-refundableperformance fee of $50 is assessed to each account and is due and

    payable by October 1ST

    with October tuition.

    This fee covers Spring Recital expenses such asadministrative fees, insurance, building rental, ushers,janitorial fees, technician fees, programs, etc. This fee entitleseach family to FOUR complimentary reserved tickets to your choice ofone of three performances, and a rehearsal CD.

    Additional tickets will be available for purchase at the front desk. The priceof these extra tickets will be posted at the studio.

    Costume Fee; TBA

    These fees will be determined based on the costume that your childs

    instructor chooses. These costumes will be ordered prior to the recital dateand will be worn by your dancer for dress rehearsals and the finalperformance.

    Costs vary per student and program

    CSAs Fall/Spring: Monthly tuition remains thesame whether there are 3, 4 or 5 weeks in the

    month.The longer months, extra rehearsals and recitaltech rehearsal in the spring months compensatefor the shorter months, observed holidays andbreaks).

    All observed holidays and breaks have beenincorporated into the monthly tuition rates. (Please

    refer to the holiday list on the inside cover of thishandbook).

    The competitive program provides an avenue for dancers to learnteamwork and good sportsmanship, benefit from healthy competition,and develop stronger performing skills. Competition teams have theopportunity to compete/perform during the spring months atcompetitions, conventions and fundraisers.

    Dancers who are selected are required to sign up for a designatedschedule of fall classes. Students who are interested in auditioning forthis program should consult with the Studio Director or other instructors.

    Additional expenses for Company members may include, but are notlimited to: Choreography, costumes, accessories,make-up kit, jewelry,convention and competition fees, warm-ups and duffle bag.

    Families (on a single account) with two or more students enrolled inCSA classes will receive a family discount.

    The discount is calculated by multiplying $5 times the number ofadditional students. (The first student is not discounted) Thisdiscount is then applied each month to the families tuitionas long as the students are actively enrolled in classand tuition is paid on time.Family discounts may be revoked if tuition and all feesare not paid on time.

    SPRINGInformation

    Please plan to set aside the entire day for these events. These eventsare mandatory for all performance class students. Specific time scheduleswill be available after the first of the year.

    Successful recital preparation requires consistent attendance by all

    students. Students can, at the teachers discretion, be removed from anyor all recital pieces if repeatedly absent or not prepared due to lack ofrehearsal.

    COSTUME DISTRIBUTION DATESRecital costumes are distributed only to parents by the Retail Coordinatorduring designated hours. These dates and times are listed below:

    PLEASE PLAN TO PICK UP ALL OF YOUR STUDENTS COSTUMES DURINGONE OF THESE TWO SCHEDULED TIMES.

    Accounts with unclaimed costumes will be billed a $10.00 pick up fee toschedule an appointment with the Retail Coordinator to retrieve theircostumes.

    All accounts must be paid in full in order for students to take home theirspring concert costume(s) and participate in the annual spring concert.This includes all monthly tuition, late fees (if issued), concert fees,costume fees and uniform fees. There will be no exceptions made to thispolicy. Costume deposits are due November 5th, 2012.

    The deadline for withdrawal from CSA classes in preparation for the 2012Spring Recital is Friday, December 17, 2012.

    COMMUNICATIONCommunication between the studio and parents isessential. The foremost communication tool for the studioto communicate with parents is through email.Information is also posted regularly on the website, and

    through quarterly newsletters posted on this site andemailed to all accounts. Additionally, CSA posts calendars andnotifications on the bulletin boards, sends notes home with students, andmails letters to parents when needed.

    Parents are asked at registration to provide a valid email address so thatCSA may communicate time sensitive notices as well as ParentHandbooks, Newsletters and other important tools of communication.

    Parents are responsible for keeping apprised of studio schedule changes,events and notices through the timely reading of emails, quarterlynewsletters, and the UDA News bulletin board.

    Newsletter will be emailed to all active accounts on a regular basis.Please take the time to carefully read all information that is in thenewsletter as most of the important information, deadlines and eventdates you will need is contained there. This is our key component ofcommunicating with our parents.

    Throughout the dance year if your child receivesrecognition in dance or the visual arts, please inform us so that we canannounce it in our studio newsletter.
