If you are eligible to access the NDIS, a planner from NDIS will arrange to meet at a time and place that is convenient for you where you will discuss things that are important to you, and what you would like to include in your Individual NDIS Plan. Your Plan will determine which supports will best meet your needs and help you achieve your life goals. Great news for the South Western Sydney area and Bankstown, the NDIS is rolling out on the 1st July 2016. It is

expected to bring lots of positive changes to you and your family.

You will be contacted soon by one of the Local Access Coordinator (LAC) to discuss if you meet NDIS disability or

early intervention access criteria. If you are eligible to access the NDIS, a planner from NDIS will arrange to meet at a

time and place that is convenient for you where you will discuss things that are important to you, and what you would

like to include in your Individual NDIS Plan. Your Plan will determine which supports will best meet your needs and

help you achieve your life goals.

To help you think about and identify your needs, what you would like to be included in your plan, and assist your

FAQS Frequently Asked Questions

What Creating Links’ Disability Services?

Creating Links offers a variety of programs for children

and adults aged 65 who have a permanent disability.

The programs this term are:

Link & Play Club (Bankstown): 5 – 12 years old;

Youth Links (Bankstown): 13 – 17 years old;

Learn and Play Club (Fairfield): 5 – 12 years old;

Social & Community Activities: 16 – 65 years old.

If I enrol into a program, do I need to attend all

of the activities?

No, if you enrol into any of our programs, you have the

freedom to pick the activities that you would like to

attend.

How do I pay for the activities?

At the start of the term, Creating Links will provide you

with an invoice listing the activities you have been

booked into for the term. Prior to the commencement

of the term we ask that you pay all fees. You may issue a

cheque to Creating Links or direct bank deposit the fees.

Alternatively, cash payments will be accepted at the

Creating Links office in Bankstown.

What do I do if I’m running late?

If you are running late, please call the contact number

provided in this newsletter. Please note that due to

group needs, we can only wait for a maximum of 15

minutes after the arranged meeting time

If you have any additional questions, please contact Gissell or Cassandra on 02 8713 7700.

How should I prepare for an activity?

Refer to the calendar to identify where you are going for

the day. For example, if you are going to the swimming

pool bring swim clothing and towel. The calendar also

lets you know whether to bring or buy lunch. Please

remember to keep yourself hydrated by bringing water.

Please wear comfortable closed in shoes to all of our

activities.

What happens if it is raining?

Our programs go ahead, rain or shine! However, please

be mindful that due to weather conditions the activity

may change.

Will Creating Links cancel an activity?

Activities go ahead given we meet the minimum booking

requirements for the day. If we have less than 5

participants booked in, we will inform carers that the

activity is cancelled.

What do I do if I can’t attend the activity?

If you are unable to attend an activity, please let us know

24 hours beforehand. This will allow someone else on

the waiting list the opportunity to participate in an

activity.
