Health System’s Needs:

Aging, sicker population:

85 and older fastest growing segment*

20% of Medicare beneficiaries have 5 or more chronic conditions**

75 cents out of every health care $ treats chronic conditions**

5% of beneficiaries who die each year take up 30% of Medicare budget***
