Creating and Developing a Portfolio
• Definition: A portfolio is a compilation of materials that exemplifies your beliefs, skills, qualifications, education, training, and experiences. It provides insight into your personality and work ethic. All in all, it is a representation of your professionalism.
Clark University
Helpful Tips on Keeping a Portfolio If you already have a professional portfolio there is no need to maintain a separate one.
Creating a portfolio is the simplest way of keeping everything together. It will:
• help you know what you still need to complete
• bring clarity to your discussions with your Confirmer
• Help you be prepared if the NMC need to see your supporting evidence to verify the declarations you have made in your application
It must demonstrate:
• outcomes of your learning (knowledge/skills)
• how you have applied your knowledge and skills in the workplace
• your competence and confidence as a nurse or midwife registered with the NMC
Portfolio Checklist
Your portfolio must include the following elements:
Practice hours - maintain a record of practice hours
Continuing Professional Development (CPD) – maintain accurate and verifiable records of your CPD activities
Practice-related feedback – notes on the content of the feedback and how you used to improve your practice
Reflection and discussion – five reflective accounts that explain what you have learnt from the CPD activity or feedback
PDD Partner declaration
Third party confirmation – a signed confirmation form
Confidentiality The Code - Prioritise people
Respect people’s right to privacy and confidentiality
Under no circumstance should any information be recorded in your portfolio that can identify any individual, either patient or colleague.
Documentation
• The following section provides guidance on how you can record the different elements required for revalidation.
• The NMC have produced templates to assist you but if you already have a system that works for you there is no reason to change
• Just ensure that the presentation is both clear to you and can be easily understood by your confirmer

Third Party Confirmation
Confirming Evidence in the Portfolio
Confirmation increases professionalism by making registrants more accountable for their performance and improvement. It also encourages those registrants who work in isolation from other colleagues to discuss their professional development and revalidation with a third party.
Confirmation from a third party form
Confirmation in practice
The NMC requires:
• A declaration that the nurse or midwife has demonstrated to an appropriate third party that they have complied with the revalidation requirements using the online form
• The Confirmer must provide their name, NMC Pin or other professional identification number (where relevant), email, professional address and postcode of the appropriate third party.
Group Exercise Consider the two cases studies submitted in a portfolio and decide:
• Has the registrant completed the requirements for revalidation?
• Are the reflections linked to the Code?
• What feedback would you provide?
