Preparing the Report

Mentor prepares report about the clergy’s progress and experiences in the mentoring process.

Clergy reads and requests any modifications to the report.

Clergy and mentor reach consensus and both sign the report.

Content of the ReportA brief description of clergy’s participation in the

mentoring process.The ways the clergy has reflected on their own

ministry. Ways clergy has used process in developing

understanding of self and ministry. Important influences in progress toward

effectiveness.Other insights or comments that may enable the

dCOM or BOM to understand the clergy better.

Confidentiality

The clergy’s conversations and files are confidential, and any information from them is to be released only with the clergy’s written consent.

The dCOM/BOM may not request information from candidacy/mentor sessions that are not contained in the mentor report.

ConfidentialityThe clergy voluntarily includes any

information he/she chooses.

The dCOM/BOM may ask for clarity and clergy may add whatever he/she deems necessary.

According to state law, when another person (particularly someone vulnerable) or the clergy is at risk, confidentiality does not apply.
