COR Status Report Workflow

1. The COR creates the status report and upon submission of the report SPM generates an email to notify the CO/CS/QAPOC/ACO. 2. A Reviewer (CO or CS) reviews and signs the report by clicking the Review button. 3. Once the Review is complete, the CO or CS then approves or rejects the status report in the Surveillance Document section. 4. An email is sent to the COR after approval or rejection.

Steps to Smart Form Access

Step 1: After selecting COR Surveillance and Oversight from the SPM Menu the following page will display. Enter in the search criteria, the user will click the Search button. The search results will be restricted by role, agency and DoDAAC.

Step 2: A list of relevant contracts will be returned.

Step 3: After selecting the Contract Number link, the following page will be displayed. Each contract will have the following sections of information: Contracting Information, Contractor Information, Contract Place of Performance, QA Surveillance Plan/Waiver, COR information, Supervisor/Commander Information, COR Certifications, Supervisor/Commander Certifications, Contracting Office Certifications, Designation Letter, Termination Letter, Workflow History, and Surveillance Documents.

Smart Form Selection

To create, edit of view Smart forms, the user will navigate to the Surveillance Documents section. The user will click on Add Smart Forms and select the appropriate Smart form.

COR Status Smart Form

The required fields are: Report Month, Report Year, and all the fields after Surveillance Method.

Review by the CO/CS

Once COR submits the COR Status Report, the CO/CS receives an email with a notice to take action. The CO/CS will navigate to the Surveillance Document section and find the COR Status Report to complete the Review. Once the CO/CS completes the Review they click on the Review button. The COR Status Report will now appear in the Surveillance Document section with an Approve and Reject button.

Approval/Rejection

The CO/CS will now have the option to Approve or Reject the COR Status Report. If the report is approved, this completes the process. If it is rejected the CO/CS will enter a comment and the report will be routed back to the COR.

If the COR Status Report was rejected the COR can now see the rejected document in their Surveillance Document list. Click on the Edit button and make the necessary changes.

The COR can now click on Resubmit to send the COR Status Report through the approval process again.
