Opened in 1994 Satellite clinics – Lincoln, MT and God’s Love Shelter Primary Health & Dental Care, Mental Health, Case
Management & Medication Assistance Program 6 Medical Providers, 1 Mental Health Provider, 1 Case
Manager, 1 Diabetes Educator Typically serve 6000 individuals per year 20,000 encounters per year EMR implemented in May 2009 Telemedicine by end of 2010

Our Vision
Integrate Mental Health & Chemical Dependency Services into Primary Health Care
Received ORHP Grant in 2006
Hired staff, and developed a Community Consortium. The goal of the consortium
was to increase patient access to psychiatric & CD services.

Fulltime LCSW and Case Manager hired in 2006 LCSW provides brief, focused, cognitive behavioral
therapy on site Case Manager provides care coordination, resource &

Received New ORHP Funding 2009!
This allowed us to continue and expand our Mental Health Program
We are now targeting CHC patients with co-morbid Diabetes & Mental Health Diagnoses to improve mental health and diabetes control.
