• Ghost Shift Change
• Controller Commute Time
• Definition of Safety-Related Points
• Emergency Response Training
Controller OQ Validation
• Controller Perspective on OQ Testing
• Could validate skills to operate pipeline equipment …
– how to open valve, how to start unit
• Better to test for …
– when or why to open valve or start unit
Controller OQ Depth
• How to perform Task
• Needed Information
• Related Responsibilities
• Impacts of action
• Accountability for Actions
• Abnormal Conditions
Fatigue Management Hard Targets
• Must per (d)(1)
– Opportunity for 8 hours of Sleep
• Must per (d)(4)
– Establish maximum hours of service
Fatigue Management Soft Targets
• Should consider – FAQ D.07
• 14 duty hours in any sliding 24-hour period
• 65 duty hours in any sliding 7-day period
• At least 35 continuous hours spent off-duty when any limit is reached
Fatigue Management Soft Targets
• Times of concern/increased Fatigue risk – Any shift duty hours worked after first eight
hours (day or night)
– Any hours worked between 2 AM and 6 AM (in general “dawn”)
– Any night shifts following three successive night shifts in a row
NTSB Safety Recommendation, P-12-7
Develop requirements for team training of control center staff involved in pipeline operations similar to those used
in other transportation modes.
• ADB-2014-02 : Team training among other lessons learned in Marshall, MI
• Crew Resource Management (FAA) • Bridge Resource Management (Coast Guard/Maritime) • API 1168 (2nd Edition Feb 2015) includes team training • PHMSA is considering to put team training in CRM
Section (h), Training
