Welcome to Fidelity! Thank you for consolidating your MPF accrued benefits to Fidelity. This handbook is a step-by-step guide to help you fill in the forms for accrued benefits transfer.

MPF(S)-P(P)

If you choose to transfer your Employer Choice Arrangement related accrued benefits to Fidelity, simply fill in an Employee Choice Arrangement (ECA) Transfer Election Form, or simply referenced asForm MPF(S)-P(P) (as marked at the top left hand corner of the form)

If you have your latest annual MPF benefit statement3, please check the front page of the statement for the Name of Trustee, Name of Scheme & MPF Account Number. You may then fill in Section B.

If you dont have your benefit statement, you may request for MPF account information from the Mandatory Provident Fund Authority (the MPFA). Just fill in a separate Request Form for Personal Account Details (Scheme Member) which is referenced as Form PA-SM4 and submit it to the MPFA by one of the methods5 shown on page 14. Once you receive information on your MPF accounts trustee, you may call them to get further details like Name of Scheme and MPF Account Number for filling in Section B.

For those of you who would like to transfer the accrued benefits derived from mandatory contributions in the account stated in Section B into the Contribution Account with your current employer that is with Fidelity :

This section gives instruction to terminate your MPF account with no residual balance.

If your MPF account stated in Section B (1) has no residual balance upon transfer of the full accrued benefits to Fidelity, an instruction will be given to the original trustee to terminate your relevant MPF account.
