POC-guiding catheter for the direct endoscopy of the biliary tract

ORDER-NO. 04205120, PaCkaGING uNIT 2 PIECEs

The POC-guiding catheter, consisting of a balloon catheter with three

extensions, a disposable syringe and a needle for refilling the POC

guiding catheter, was developed for the peroral cholangioscopy

(POC).

The POC-guiding catheter is positioned through the working channel

of a duodenoscope, and the balloon is inflated through the insufflation

lumen of the catheter until the balloon anchors behind the stenosis.

after the removal of the guidewire, the extensions are cut off from

the POC-guiding catheter at the green mark with a sharp side cutter.

Now the duodenoscope can be removed.

subsequently, the guidewire and the cholangioscope

or the nasal gastroscope are advanced through respectively over the

POC guiding catheter through the papilla into the desired area of the

bile duct.

Thanks to the blocked balloon, the cholangioscope or rather the

nasal gastroscope can be advanced through the stenosis.

In case of insufficient air inflation of the balloon and already

separated extensions of the catheter, or accidental deflation, it is

possible to refill the balloon with the corresponding needle.

Cyst drainage set

The cyst drainage set consists of a cystostome with an extended pusher and a 2-flap endoprosthesis.

The cyst drainage set was developed for the endoscopic puncture with HF-current. The endoprosthesis

for cyst drainage can be positioned without changing the instruments.

Removable connections

The puncture wire can be removed and replaced by a guide wire

(0.035 inches).

OTSC® System Está formado por un capuchón de aplicación con un clip incorporado, hilo y una manija para la liberación del clip.

Application System

EPI provides an innovative powder delivery system to enable endoscopists to spray the powder- G I

format agent directly to the designated site via endoscope. The innovative applicator includes a

delivery catheter and a specially designed powder/gas mixing chamber that is connected to the

powder container and an external gas source.

The delivery catheter is inserted into the endoscope and positioned towards the lesion in the gastro-

intestinal tract. Pressure coming from the gas compressor propels the powder through the catheter

directly to the designated area. The unique anti-reflux design prevents occlusion.

Polymer Solution for hemostasis

AMP® technology is a proprietary engineering process that modifies plant starch into biocompatible,

absorbable hemostatic polysaccharides.

AMP® particles have a molecular structure that rapidly absorbs water from blood, causing a high

concentration of platelets, red blood cells and coagulation proteins at the bleeding site which accel-

erates the physiologic clotting cascade. The interaction of AMP® particles with blood rapidly

produces a gelled matrix that adheres to and seals the bleeding tissue. AMP® particles are readily

dissolved by saline irrigation and are degraded rapidly by human enzymes.

AMP® particles have been widely used in open surgery and proved to be safe and effective in

achieving hemostasis.

EndoClot® PHS consists of Absorbable Modified Polymers (AMP®) and a powder delivery system. It

is intended for use as an adjunct hemostat to control bleeding from capillary, venous or arteriolar

vessels in the upper and lower GIT.

Hemostatic solution in upper and

lower GIT to control bleedings

EndoClot® PHS is safe and effective for controlling bleeding in GIT. It works particularly well for

large and oozing wounds. Additionally, it allows endoscopists to control bleeding when conventional

hemostatic techniques are impractical to apply. EndoClot™ PHS can be used to prevent re-bleeding

after endoscopic, invasive surgery.

The Endocuff is disposable and simply slides over the tip of the colonoscope distal to the bending section. lt does not interfere with normal colonoscope function. Each individual flexible arm is hinged at its base so that during anal intubation and forward motion they automatically retract into the EndoCuff casing to create a smooth low friction surface. The Endocuff takes colonoscopy to a new level of detection with its innovative, intuitive and easy to use approach. The Endocuff gently opens the colon for easy inspection, its unique array of flexible arms maintains the tip of the instrument in the centre of the lumen and retracts the mucosal folds to allow dynamic inspection as they move back into their resting position. Endocuff provides better tip control so that the endoscopist is able to withdraw without sudden slippage. The flexures can be inspected in greater detail and concertinaed tracts of sigmoid colon are opened as the arms gently separate the folds to provide a clear view of the mucosa behind them. Maintaining tip position during EMR and polypectomy is essential to ensure the procedure is performed safely and quickly. The Endocuff acts as a handbrake allowing optimization of tip position during therapy and retrieval and helps to prevent movement as instruments are fed through the biopsy channel. Endocuff brings better tip control and vision to colonoscopy.

The advantages of Endocuff assisted colonoscopy:

•There is no interference of vision even when using the latest 170 degree colonoscopes.

•The mucosal folds are retracted automatically during colonoscope withdrawal, enabling their upstream surface and the mucosa between the folds to be viewed.

•The movement of the folds provides a dynamic picture so even the smallest of polyps can be identified by the play of light across the mucosal surface.

•Fold retraction begins before de fold is viewed so there is no need to keep re-intubating to manipulate folds that are beyond the tip.

•Larger folds, folds ata flexure or those in a wider part of the colon can be manipulated with ease.

Black Eye is indicated for endoscopically marking lesions in the Gl tract when the

endoscopist anticipates the lesion will require surgical removal within 30 days.

Technological Characteristics and Substantial Equivalence

Black Eye is substantially equivalent to the predicate colon marker. The Black Eye and the predicate

device have same indications for use. Both are carbon black formulations that are used for

endoscopically tattooing or marking the colon.

The Black Eye Endoscopic Marking System provides a method of marking cancerous lesions on the

wall of a patient's stomach or colon during an endoscopic procedure. The mark provides a reference

point for subsequent removal of the tissue. The inexpensive packaging solution turned the

technology into a product, by creating a whimsical brand identity that uses color and simple

iconography that make it appear less threatening, thereby reducing user anxiety.

CLINICAL DATA

A recent clinical trial of 22 patients (16 pancreatic,

6 cholangiocarcinoma) was reported in

Gastrointestinal Endoscopy Journal (GIE). The

patients were treated with the EndoHPB catheter

between January 2009 and April 2010 and the

study was conducted to demonstrate safety and

report on 90 day biliary patency. Deployment of the

catheter was successful in 21 patients. Self

Expanding Metal Stent (SEMS) placement was

achieved in all cases after deployment of our

catheter (Steel et al, GIE 2011;73:149-153).

In another trial, the Imperial College team have also

observed a statistically significant improvement in

survival at 3 months and 6 months in patients

treated with intraductal RF ablation compared to a

retrospective cohort treated only with SEMS (pre-

publication data).

USAGE

The HabibTM EndoHPB can be used with a range of

commonly available RF generators. The purchase of

a corresponding adapter cable is required.
