INTRODUCTION

Dear valued customer,

Welcome to BPFK QUEST 3 Submission system.

The Quest3 Online Submission System has many build in security measures. It is an integrated and computerized system which enables Product License Holder, Manufacturer, Importer, Re-packer, Wholesaler and other related users to conduct secured online transactions such as registration, variations, re-registration along with limited changes in stored data such as change of manufacturing site addresses etc. and other transactions including licensing and surveillance activities.

There are several steps you need to follow before start using QUEST 3:-

a) Register your QUEST membership online

b) Purchase your User Digital Certificate that comes in USB token.

Once you have obtained and installed your Digital Certificate USB token to your

compatible*Personal Computer (PC) that is Internet* Ready, then you will be able

to conduct your transactions in Quest3.

*To ease the registration process, please make sure you are using compatible hardware and software.

PC SPECIFICATION

To conduct transactions in Quest3, you must conform to the following pre-requisite:

1) Have an Internet-Ready PC. Minimum requirement:

Intel Pentium II 400MHz/ Intel Pentium IV 1.8GHz or above 128MB RAM/ 256MB RAM or above 10GB free disk space or above 56kbps/128kbps Internet Connectivity. WIFI 10Mbp or above

2) Have Microsoft Internet Explorer 8 (IE8) installed. If you are using other versions of Internet Browser, please go to http://www.microsoft.com/windows/internet-explorer/worldwide-sites.aspx to download the IE8.

3) Have a scanner and/or digital camera installed to scan related documents for attachments during submission of application forms.

4) Have at least 1 unused USBport on your PC for the connection of the Digicert USB token.

Please take note that QUEST 3 system DOES NOT support MAC book or any APPLE Machines.

1) Log on to National Pharmaceutical Control Bureau

Quest3 website. http://quest3.bpfk.gov.my/QUEST3/

2) Click on "Registration" menu to register your

membership.

5) Fill in all compulsory fields

All fields are compulsory. Please fill in all fields and

attachments. Empty field will result in difficulties to re-

enter the same company name and registration number

For company registration number, kindly remove any

spaces, and or dash (-)

6) Attach scanned images/picture (in JPEG or PDF format,

with size below 2MB) of Company Registration Form

with Suruhanjaya Syarikat Malaysia (SSM)

For Sdn. Bhd. and Berhad companies, please include the

Memorandum Article of Association (MAA) attached

together with Form 9 (Company Registration Certificate).

7) Click “Save” and proceed to the next page

11) Fill in company details such as full address,

telephone number, fax number, email address.

12) Click “Add Other Address” to if you wish to declare

other business address (e.g. product owner)

13) Click Next to proceed to the next page.

14) Fill in relevant business details if you had clicked on

“Add Other Address”

15) Click “Save”, then “Next” to proceed to the next

16) Fill in details of authorized master.

Please take note that name of authorized master MUST BE EXACTLY THE SAME as stated in

the identity card.

17) Attach scanned images/picture (in JPEG or PDF format, with size below 2MB)

of Company Authorization Letter

(This is an Official letter of authorization by your company to BPFK authorizing the

authorized person to conduct transaction on behalf of the company)

18) Attach scanned images/picture (in JPEG or PDF format, with size below 2MB)

of Authorized Person Identity Card.

An individual can only be the authorized person for one company. Subsequent

application made using the same IC number will be rejected and no refund will be

given.

In the event of rejection, you need to

27)Your USB security token (Digital Certificate)

will be sent to you via POSLAJU at the

company address stated in the membership

registration form upon receipt of payment

(within 7 working days).

You can manage your membership subscription in the QUEST 3 system.

1) Log in to your QUEST 3 account by your user name and

password at http://quest3.bpfk.gov.my/QUEST3/

It is advisable to apply for membership renewal

while you are still able to log on QUEST 3 system,

preferable 1 month before the expiry.

You will not be able to use the USB token to access

to QUEST 3 once it is expired.

Fill in the details; choose either package A or B then

select ‘proceed’.

FAQ ABOUT QUEST MEMBERSHIP

1) Who can apply for QUEST 3 membership?

Please be advised that for membership application the company

must be registered with Suruhanjaya Syarikat Malaysia (SSM)

and the scope of business related to pharmaceutical and

cosmetic fields. For further information, please refer to the Drug

Registration Guidance Document (DRGD).

For Sdn. Bhd. and Berhad companies, please attach the

Memorandum Article of Association (MAA) and Form 9 (Company

Registration Certificate) together with the application

2) How to write a company authorization letter?

3) How do I reprint my membership registration slip (NOT

for renewal)

i. Go to QUEST 3 webpage

ii. select “print slip membership registration here”

iii. Fill in username, IC number@ passport number

iv. Answer security Questions

v. Hit „submit‟

4) I can‟t generate and print the payment voucher.

i. Make sure you are using internet browser IE8

ii. To avoid congestion of network, it is recommended printing

payment voucher in morning before 8a.m; or during lunch

hour; or after office hour.

5) The figure printed on payment voucher is incorrect.

Please contact our helpdesk for the correction.

6) Can one user have more than one membership account?

No, one user with his/her unique identity card can only register

for one membership account.

7) I can‟t register membership due to existing “authorized

person” name or “company name” in the system

Please contact our helpdesk 03-78835400 Ext 5560/61/62 for

assistance.

FAQ ABOUT USB SECURITY TOKEN

1) Can a company have more than one token?

Yes, a company can have more than one token.

The first token will be for the principal user account (master) and

subsequent application(s) will be considered as for supplementary

account(s) (member).

Supplementary account(s) can only be applied through the principal

account on the membership utilities page.

2) Is there a limit to the number of supplementary

accounts a company can purchase?

No, a company can possess an unlimited number of supplementary

accounts.

3) How much does the USB Token costs? To whom shall I

pay the charges?

All payment regarding USB token must be made payable to

Digicert Sdn Bhd. The charges are as follows:

4) When will I receive the token upon payment and

submission to Digicert Sdn Bhd?

If all the required documentation and payment is made

successfully, you should expect to receive your token from

Digicert Sdn Bhd within 7 working days.

However, if you are still not receiving beyond the date, please

contact Digicert at 03-8992 8888 or email at

[email protected]

5) How to install the USB Token into my computer?

Once plugged in to the computer the application will auto run,

please follow instruction of the software to complete installation.

6) My USB token is still valid, but I can‟t log on to QUEST 3

system.

i. Make sure you are using internet browser IE8

ii. If problem persist, please contact Digicert Sdn Bhd at 03-

8992 8888 or email at [email protected]

7) I've lost my USB Token, how do I get the replacement

set?

Please report to BPFK at 03 78835560/5561 and contact Digicert

Sdn Bhd at [email protected]

Replacement cost will be imposed. Please refer to the charges

table

8) I've lost (forgotten) my Pin Code, Username or

Password, what should I do?

To retrieve user/ login name &

password:

Please call BPFK office at 03-7883

To reset Pin Code please return your USB Token to

9) My USB Token pin number is locked, what should I do?

If the USB Token pin code is locked, please return your USB

Token to Digicert Sdn Bhd for resetting process.

10) How do I know if my USB token already expired?

11) My USB token has expired, what do I do?

Contact Helpdesk to re-print Renewal Slip. It is advisable to

always keep track on the validity and do renewal 1 month before

expiry.

12) I want to change the name of cardholder for my USB

token, what should I do?

i. Logged in to your account and click on „COMPANY

MEMBERSHIP‟ option

ii. In the membership utilities on right hand side, choose

“Change Authorized Person”.

iii. Fill in the online form. Once complete, you will have to print

the membership slip and apply for a new USB token.

iv. Please make sure that the new authorized person does not

have any existing membership with the QUEST system.

13) Are all my transactions and registration matters

recorded inside the USB Token? What if I lose the USB

token? Do I have to re-do all my work?

No. The transactions and registration matters are not recorded

onto the USB Token. The record is stored in the BPFK servers

and you will not need to re-do your registration matters if you lost

your USB token.

14) How do I change or modify the company data in my

BPFK account (e.g. company name, company new

address, email address etc.)?

To change or modify the company name, company address and

company registration number, please contact the Regulatory Co-

ordination Section. User‟s email address can be changed directly

from the Membership Utilities.
