By signing this Entry & Release Form, I acknowledge that I have read and understand The Art Institutes Best Teen Chef Competition or The Art Institutes Culinary Scholarship Competition Official Rules (which I viewed at artinstitutes.edu/culinary) and agree to abide by and be bound by them and the decisions of The Art Institutes International LLC (“The Art Institutes”) and judges which are final, binding, and non-disputable in all matters relating to The Art Institutes Best Teen Chef Competition or The Art Institutes Culinary Scholarship Competition or (“Competition”).

I hereby irrevocably give my consent to The Art Institutes and to those it may authorize to photograph, film, and/or videotape me and/or my performance in any level of the Competition, and to quote and record statements made by me and to use, reproduce, publicly display, stream, distribute, sell, and/or create derivative works from my image, images of my performance in the Competition, and statements made by me (with or without my name) and my entry materials and all elements thereof and to identify me by name and/or with school and employment information, in all forms of media now known or later developed, including without limitation on the Internet, for any editorial, promotional, advertising, trade, commercial, or other purpose whatsoever, in perpetuity throughout the world, without additional compensation, notification or approval from me. I hereby release, indemnify and hold harmless The Art Institutes, those it has authorized, and their respective successors and assignees, from any and all claims and/or damages that may arise out of the use of any of rights granted herein, including any claims of defamation, invasion of privacy or infringement of moral rights, rights of publicity, or copyright. Enter either the high school senior (The Art Institutes Best Teen Chef Competition) or high school graduate (The Art Institutes Culinary Scholarship Competition) category and match your skills against others who share your passion. Show the world what you can do. You could earn a tuition scholarship toward an education where you explore foods from around the world. One first place winner will be named in each category from each school. A second place winner will also be named in the Best Teen Chef category from each school.

Entry requirements for The Art Institutes Best Teen Chef Competition and The Art Institutes Culinary Scholarship Competition: By Feb. 3:Complete the Entry & Release Form available for download at www.artinstitutes.edu/culinary or attached to this brochure and mail it to The Art Institutes school you plan to attend. By Feb. 24:Mail the following to the same school: • Your favorite recipe in standardized recipe format (standardized recipe form available for download at www.artinstitutes.edu/culinary) • An original photo of your favorite recipe prepared (optional) • A copy of your high school transcript • A short essay (maximum 250 words) legibly handwritten or typed in English about why you want to study culinary at The Art Institutes

Selected entrants in each category will compete against others in the same category in Local Cook-Off Competitions and prepare a predetermined menu issued in advance by The Art Institutes. Local Cook-Offs will be held on or around April 21, 2012.* Selected entrants will compete at the school where they entered the competition. * Certain participating Art Institutes schools may hold their Local Cook-Off Competition on a different date. Any such date changes will be announced to selected contestants as soon as reasonably possible.
