The Isabela State University (ISU), Ilagan City Campus is a globally renowned institution that provides student excellence and empowerment to young and talented professionals. In line with this, the SCHOOL OF MIDWIFERY commits to create COMMUNITY ORGANIZING PARTICIPATORY APPROACH AND ACTION RESEARCH (CO-PAR) in a diverse society for the community people to reach and deliver programs of the Department of Health (DOH) in partner with the Rural Health Unit (RHU) 1. The organization aims to: 1. Promote health promotion and wellness. 2. Raise community awareness with different problems concerning health. 3. Provide a Strategic plan through networking and linkaging with the different Government (GO) and Non-Government (NGO) agencies to address health related concerns. 4. Monitor health concern of individuals, family and community, especially the maternal and child cases. 5. Community empowerment through self-reliance and income generated program maximizing community resources. Furthermore, we would like to conduct an OCULAR SURVEY at Barangay__________ to start the process of this program. Your approval to this proposal shall give a positive result for community awareness and development. Thank you and God bless!
