REPORT OF THE COMMISSION ON MEMBERSHIP AND MEMBER SERVICES 1 Author: Lesca Hadley, MD, Chair 2 Date: April 8, 2017 3 4 The following members of the commission were present for the meeting: Lesca Hadley, Sarah Samreen, 5 Dale Crawford Allison, Emily Briggs, Bruce Echols, Terrance Hines, Graciela Moreno, Mary Nguyen, 6 Sherri Onyiego, Karen Smith, and Ikemefuna Okwuwa. 7 8 The following guests and staff were present for the meeting were present: K. Ashok Kumar, Troy 9 Fiesinger, Linda Siy, and staff liaisons Jonathan Nelson and Kathy McCarthy. 10 11 A regular meeting of the Commission on Membership and Member Services was held on Saturday, April 12 8, 2017 at the Austin Hotel at Renaissance Hotel in Austin, Texas. The Chair called the meeting to order 13 at 2:15 p.m. 14

15 The Committee took action at its meeting that resulted in the following recommendations for 16 consideration by the Board of Directors: 17 18

Item 1. That the Texas Family Physician of the Year Award be given to Jorge Duchicela. 19 20 BACKGROUND 21 Having requested and reviewed nominations for the 2017 Texas Family Physician of the Year 22 Award, the Commission selected Jorge Duchicela to receive the award. 23 24 Item 2. That the TAFP Physician Emeritus Award be given to Dale Moquist. 25 26 BACKGROUND 27 Having requested and reviewed nominations for the 2017 TAFP Physician Emeritus Award, the 28 Commission selected Dale Moquist to receive the award. 29 30

The Committee took action at its meeting that resulted in the following recommendations for staff 31 implementation or referral to other commissions and committees. These items do not require Board 32 approval. Should staff find that the time needed for implementation exceeds five hours, the item 33 should be referred to the Board for approval before proceeding. 34 35

1. That in 2018 the commission accept the resubmission of the nomination of Tricia Elliott for 36 the Texas Family Physician of the Year Award. 37

38 The following items were discussed at the meeting and are presented for information and further 39 distribution to members. 40 41

1. The Commission heard a report from a representative of the Section on Special Constituencies, 42 Bruce Echols. 43

2. The Commission discussed implementation of TAFP’s Member Communities. The Early Career 44 Physician Member Community was attended by six members including two residents, and the 45 group discussed MACRA and physician burnout, among other topics. The IMG group was 46 attended by only two members. The Solo and Small Group community discussed practice 47 management, ACOs, chronic care management and more. Members stated the networking and 48 discussion were cathartic. Commission members suggested staff record the names of attendees 49

for future marketing of these events. The members will also plan to encourage colleagues one-on-1 one to attend in the future. 2

3. The Commission reviewed Texas Family Physician. Advertising revenue exceeds the production 3 costs for most issues. The Commission reviewed the editorial arch for the magazine and its focus 4 on educating members about MACRA, physician burnout, and behavioral health, among other 5 topics. 6

4. Jonathan Nelson discussed the TAFP website. 7 5. Kathy McCarthy discussed TAFP membership statistics, the re-election report, and resident and 8

student recruiting. She presented data showing member retention for TAFP (94.6%) and new 9 member retention (90.6%) compare very well to AAFP and to other large states. As of January 10 2017, Active membership had risen to nearly 4,900. Student membership is up to 2,803. Active 11 member retention efforts are underway as staff works to get members to renew by paying dues 12 and maintain membership by reporting CME credits before the May 1 deadline. 13

c. Discuss opportunities for leadership and participation coming up, including NCCL 2017 representative positions, Special Constituency position on the TAFP Board of Directors, and Special Constituency delegates for the TAFP Member Assembly in 2018.

d. Appointment of the selection committee that will select the five official representatives to NCCL 2018. The proposed committee should include the Section chair, the Section vice chair, and the five NCCL representatives from the previous year. Others may volunteer to serve on the selection committee.

Commission on Membership & Member Services Item 4.B. Member Communities

October 2016

We debuted our Member Community groups last year at Annual Session and added one at Interim Session. We are adding two more and we’ll see how these do. The purpose is for members to connect with each other and help each other – building community. The groups are not deliberative bodies taking action, but they can ask a committee or a commission to discuss an issue at a future meeting. The schedule is below. I hope you’ll participate in one or more of the groups and provide feedback as we continue to develop this idea. April 12-13, 2018 | Team Leadership and Interpersonal Skills

The inaugural group session will be held along with TAFP’s C. Frank Webber Lectureship and Interim Session. Participants will discuss the importance of personality types in teams, how to lead change, negotiation, and motivational skills. They will embark upon a yearlong project, the result of which will be presented at the program’s conclusion.

June 9, 2018 Communications and Organizational Leadership

Communications skills are an essential piece of the toolkit good leaders rely on. Spend the day in Austin polishing your skills and learning how to communicate more effectively.

• Public speaking and media training.

• Running a meeting, governance, and organized medicine.

• Presentation skills.

August 24, 2018 | Advocacy & Influence

Participants will gather in Austin to learn about leadership in advocacy.

• Understanding policy issues, state and national, that affect health care and family medicine.

• Role playing scenarios: How to have successful meetings with legislators and how to give persuasive testimony to a legislative committee.

November 9 2018 | Sharing Your Knowledge and Graduation

The group will graduate from the program at TAFP’s Annual Session and Primary Care Summit. Participants will be asked to present their projects during the FMLE final session.

PROGRAM GUIDELINES

• Applicants must be active TAFP members and should be new physicians, mid-career physicians, or residents.

• A maximum of 25 members will be selected to participate in the program.

PROGRAM STRUCTURE

Over the course of one year, participants in the Family Medicine Leadership Experience will attend four in-person sessions lasting one to two days and presented in a variety of formats including didactic lectures, small group

discussions, and interactive learning exercises.

Participants must attend all four sessions to graduate from the program, but the curriculum will repeat annually, so a participant could pause and resume the program if he or she were to have difficulty making all four events in one year.

FEES AND EXPENSES

There is no tuition fee, but scholars are required to pay their travel expenses and some housing expenses. TAFP provides meals and program materials during training.

The application asks for a brief description of your leadership and management experience as well as a statement describing why you would like to pursue advanced leadership training.

The TAFP Leadership Development Committee will review all applications to compose the FMLE class of 2018.

Following are several charts that show our membership growth. AAFP dropped the members who had not paid dues or been re-elected in May, as they did last year. This year we lost more members (411 last year, 481 this year). At this point 309 of those dropped have not rejoined or been reinstated. 1. The first shows that the number of Active members. As of September 30, there were 4,935

Active Members in TAFP. That is up 139 year to year. 2. The second graph shows the total membership. As of September 30, there were 8,572

members. That is up over 300 from last year. That is driven by higher student membership.

On May 3, 2017, 481 memberships were cancelled for either non-payment of dues or lack of CME requirements. TAFP sent an email on May 9, 2017 to 481 dropped members with information on how TAFP and AAFP are here to help busy physicians. In 2016, we mailed 411 dropped members a letter about being dropped, 157 reinstated. To date, 169 members have reinstated their membership. In 2018 we will return to mailing a letter along with the dropped email to increase our reach to the dropped members.

Each year TAFP solicits award nominations through a general e-mail to all members and to local chapters. Recipients are honored at Annual Session. As you talk to colleagues at this meeting and in other places, be sure to promote TAFP awards as an opportunity to highlight and celebrate the best of family medicine. Family medicine is the ideal specialty in which to answer that call, whether medical students know it or not. By signing up to be a preceptor in the Texas Family Medicine Preceptorship Program, you can help make sure they know it. The Texas Family Medicine Preceptorship Program is designed to match pre-clinical medical students with a skilled family physician for a two- to four-week period during the summer so they can gain real-life exposure to the broad range of clinical medicine and the special relationships family medicine has to offer.

Want to become a preceptor? All you have to do is register for the program at www.tafp.org/preceptorship. Click the button to enter the Preceptorship Portal,

and create a new physician preceptor account.

What do you need to know about being a preceptor?

» Filling out an application puts your name on a list as a possible preceptor for two to four weeks during the summer each year. If a student selects you from our list of potential preceptors, TAFP will contact you to see if their request works with your schedule.

» You can earn up to 60 prescribed credits per three-year cycle for your AAFP membership.

» We will host two FREE clinical teaching skills workshops each year, each including an ethics credit. These workshops could count toward faculty development requirements if you have a current faculty appointment.

» You will be giving future physicians their first real impressions of the joy and fulfillment of family medicine!

Eligibility requirements

» You must have completed a three-year family medicine residency. » You must be board-certified.

» You cannot have any current board actions against you by the Texas Medical Board.

The 2017 fiscal year of the Texas Family Medicine Preceptorship program saw significant improvement in student participation, due in part to continued state funding, more aggressive marketing efforts and a greater understanding of the program experience.

In total, we received 189 applications from students across the lone star state. While the marketing and awareness challenges we faced in 2016 were largely avoided, 2017 brought a new set of challenges to overcome.

Preceptor employer challenges

Many of our volunteer preceptors faced internal challenges with their employer when it came to accept medical students for rotations. Some employer locations were willing to accept medical students into their facilities, but required several prequalification – drug testing, criminal background checks, vaccine records, etc. – to be completed prior to signing off on the experience. Often these requests delayed the specifically requested start times for the medical student and caused the rotation to be canceled.

Affiliation agreements

Affiliation agreements were another hurdle facing TFMPP. Several medical facilities required an affiliation agreement between themselves and the medical schools of students to facilitate the rotation. This also caused several matches to be cancelled.

Plenty of students for certain locations

Because we had an influx in medical student participants, we faced the obstacle of having a surplus of students requesting specific locations. Those popular locations did not always have enough available family physicians and again, often had number of restrictions for medical student rotations.

With these new challenges in mind we have begun dialogues with medical schools, individual practice administrators and physicians, to avoid these issues during the 2018 preceptorship program. TAFP remains committed to the opportunity to expose as many medical students to the family medicine specialty as possible.

ADDITIONAL FUNDING EFFORTS Texas Academy of Family Physicians continues to support the Texas Family Medicine Preceptorship Program in various ways. Multiple TAFP staff members provided their expertise to support and promote the program without billing the preceptorship program for their services.
