Top Interventions Action Plan

6 Distribute pocket cards and laminated badge cards (Hour 1 Bundle of Surviving Sepsis Campaign).

7 Establish Rapid Response Team for inpatient care to recognize sooner.

8 Review data with clinical staff and include on quality scorecard.

9 Real-time chart review of patients with sepsis.

10 Monitor readmission and mortality rates associated with sepsis.

11 Provide earlier recognition/awareness at outside facilities and EMS.
