INSTRUCTIONS
Using a #2" or soft lead pencil, and gOing from top to bottom, fill in your medical education 'number (ME#) and quiz identification in the spaces and scanning targets. (Scanning targets: "0' "1" "2" "3" "4" "5" ). The example below demon~ strates the correct procedure for entering your medical education number. The six digit Quiz Identification Number, which may be found at the beginning of the Dermatology Quiz, should also be marked from top to bottom. The method for marking the scanning targets is the same as shown in the Recording Methods section of the answer sheet.
