CLOTHING REQUEST FORM INSTRUCTIONS1. The Sharing Shelf provides clothing to infants, youth and teens (ages birth to

19) from families in financial need where purchasing new or used clothing would be a burden on the family’s resources. We do not provide clothing to adults.

2. This form is a Word document. Please download to your computer and keep a blank version for future use.

3. Type in ALL requested information at the top and throughout the form. 4. Please provide accurate sizing information (ie: youth sizes are 6, 7, 8, 10, 12,

14. There is no size 9). We suggest you consult a Gap size chart online when in doubt. Inaccurate information may delay your order or result is clothing that is the wrong size. For teens, provide a NUMERIC PANT size (ie: 0, 1, 3, 5 for girls or 28, 30, 32 etc… for boys).

5. For ALL teens , please provide approximate height since jeans for teens, especially boys, have varying lengths.

6. For teen girls, please include information in the comment section regarding their styles tastes.

7. Save your changes as either as a .doc or .docx then attach and email the form back to: thesharingshelf@fsw.org. We do not accept handwritten requests or forms submitted as a pdf or fax.

8. You may put more than family or client per form and you can add numbers to the form. We prefer if you consolidate your orders.

9. It generally takes 2-3 weeks to process and fill orders. In busier periods, it may take 4-6 weeks.

10. You or one of your colleagues must be pick-up at our warehouse, located 47 Purdy Avenue in Port Chester. Your clients may not place nor pick up orders.

11. If this is an order for emergency clothing (ie: fire, emergency removal), PLEASE indicate on your form and we will expedite your order.

12. We will hold your order for up to 3 weeks once we have notified you that it is complete. After that time, we may redistribute it to another waiting child.

13. We strive to serve all children and teens but our inventory is dependent on donations. Unfortunately, we DO NOT have plus size clothing or clothing for teens above a women’s 10/11 bottom or L/XL top for teen girls or a men’s 38 or 2X for teen boys.

14. Baby clothing is prepacked in 4 sizes: 0-6, 6-12, 12-18 or 18-24m . Choose one of these 4 categories when ordering. (6-9 and 9-12 will be in 6-12 and 0-3 and 3-6 are in 0-6).
