1. The Applicant acknowledges and certifies that within 90 Calendar Days of the date of

the Corporation letter of preliminary award, the Applicant must submit:

(a) All site control documentation as stated in Part I. Item A of Exhibit D*; and

(b) All Ability to Proceed documentation as stated in Part I. Item B of Exhibit D.

THIS PROCESS OFTEN TAKES LONGER THAN ANTICIPATED. APPLICANTS ARE

STRONGLY ADVISED TO BEGIN THIS PROCESS AS EARLY AS POSSIBLE.

An invitation to credit underwriting will not be issued until these requirements are met.

*As stated on Part!. Item A of Exhibit D1 failure to demonstrate site control by the stated deadljne shall result in the withdrawal of the Corporation letter of preliminary award.

2. The Applicant acknowledges and certifies that the following information will be provided by the due date outlined below, or as otherwise outlined in the invitation to enter credit underwriting. Failure to provide the required information by the stated deadflne shall result in the withdrawal of the invitation to enter credit underwriting.

a. Within seven (7) Calendar Days of the date of the invitation to enter credit

underwriting, the Applicant must respond to the invitation. The Corporation will

then submit the credit underwriting fee and deduct the expense from the Maximum Eligible Funding Award Amount as outlined in Section Four, J. above.

b. Within 14 Calendar Days of the date of the invitation to enter credit underwriting, Applicants shall submit IRS Tax Information Authorization Form 8821 for all Financial Beneficiaries to the Corporation.

c. By April 29, 2016 the Applicant must provide a Transaction Screen Process (TSP) report in accordance with ASTM Practice E 1528 standards for the entire Development site as further explained in Part 11 Item C.3 of Exhibit D.

d. The credit underwriting report must be approved by the Board by the first Board meeting on or after August 31, 2016, unless a written extension of time has been approved by the Corporation as explained in Part I. Item C.4. of Exhibit D. In the event that the extension is granted, extension fees will be assessed as outlined In the fee section of Section Four, J.

e. All grant funding must close by November 30, 2016. Applicants may request

one (1) extension of up to 3 months as explained in Part I. Item C.5. of Exhibit D. In the event that the extension is granted, extension fees will be assessed as outlined in the fee section of Section Four, J.

f. All grant funding must be expended by November 30, 2017. Applicants may

request one (1) extension of up to 3 months, as explained in Part I. Item C.6. of Exhibit D. In the event that the extension is granted, extension fees will be assessed as outlined in the fee section of Section Four, J.

g. Other items that must be submitted during the credit underwriting process are

set forth in Part I, Item D of Exhibit D.

3. By submitting this RFA, the Applicant acknowledges and certifies that all requirements

of the RFA and commitments made by the Applicant will be provided for the proposed Development and its Residents. Failure to do so shall result in the withdrawal of the invitation to enter credit underwriting:

a. All requirements set forth in the RFA and all commitments made by the

Applicant will be met;

b. The iniformatTion set forth in Exhibit D will be provided w'thin the timeframes prescdbed by the Corporation and/or the Credit Underwriter;

c. The Applicant aclknowledges that any funding preliminarily secured by the Applicant ~s conditioned upon any independent review, analysis, and verification that may be conducted by the Corporation of all information contained in the Applkation and/or subsequently provided, the successful completion of credit underwriting, and all necessary approvals by the Board of Directors, Corporation

or other legal counsel, the Credit Underwriter, and Corporation Staff;

d. If preliminary funding is approved, the Applicant will promptly furnish such other supporting information, documents, and fees requested or required by the Corporation or Credit Underwriter;

e. All awa1rdees must provide a properly completed and executed Accessibility form at the end of construction certifying that the completed Development includes the applicable accessibility, adaptability, Visitability and universal design features required by the Corporation and proposed by the Applicant;

f. As a condition of the acceptance of funding, all awardees may be required to cooperate with the Corporation or any contractors affiliated with the

Corporation in the evaluation of the effectiveness of Permanent Supportive Housing provided through this RFA. The Corporation is interested in collecting evidence to demonstrate the extent to which these Developments meet expected outcomes;

g. All awardees may be subject to compliance monitoring visits during the affordability period;

h. The Applicant developing Community Residential Homes shall commit to license

the Community Residential Home for 6 or fewer Residents for the entire affordability period. The Applicant shall submit a hard copy of the facility's license to the Corporation within 30 calendar days from the date the license is issued by the licensing agency. The Applicant shall also submit all subsequent

renewal Community Residential Homes facility licenses to the Corporation with each annual Continuing Program Compliance self-certification submitted. A current license shall be available on-site for compliance monitoring visits during

the affordability period;

i. The Applicant has read all applicable Corporation rules and provisions governing

this RFA and has read the instructions for completing this RFA and will abide by the applicable Florida Statutes and the credit underwriting and program

provisions outlined in the RFA;

j. When eliciting information from third parties required by this RFA and/or included in this Application, the Applicant has provided such parties' information that accurately describes the Development. The Applicant has reviewed the third party information included in this Application and, to the best of the Applicant's knowledge, the information provided by any such party is

based upon, and is accurate with respect to, the Development as proposed in

th~s Application;

k. The Applicant's commitments will be included in the Restrictive Covenant and Grant Agreement and must be maintained in order for the Development to remain in compliance, unless the Board approves a change; and

I. No more than two (2) Applications from the same Applicant entity can be submitted by the same Applicant entity. The Applicant understands that if more than two (2) Applications are submitted from the same Applicant entity, all

Applications submitted from the Applicant entity will be considered withdrawn, even if the Application(s) had been selected for funding and has entered into credit underwriting.

retarded wherever they may be. The term "retarded children"

as used herein shall include all mentally retarded persons

regardless of age, degree, or type of retardation and regardless

of whether or not they attend public, private~ or religiousi

schools, residential or day, or whether they reside at homes

or in institutions, or regardless of race, creed or color.

(b) To foster development of programs in behalf

of the mentally retarded.

(c) To encourage research related to mental retarda-

tion.

(d) To advise and aid parents in the solution of

their problems of mental retardation, and to coordinate their

efforts and activities.

(e) To develop a better understanding of the problems

of the mentally retarded by the public.

(£) To cooperate with all public 9 private, and

religious agencies and professional groups in the furtherance

of these ends.

(g) To associate with and support financially the

State and National Association to promote the common cause,

(h) To serve locally as a clearing house for gather­

ing and giving out information regarding the mentally retarded.

(i) To solicit and receive funds for the accomplish­

ment of the above purposes.

ARTICLE III

Membership shall be open to all parents, relatives

and guardians, including foster parents of retarded children

and to other parents or persons sympathetic to the purposes

of this corporation. Membership shall be obtained on application

to the Chairman of the Membership Committee as provided in

the By-Laws adopted by this Corporation.

ARTICLE IV

Citrus County Association for Retarded Children,

Inc., (A Corporation Not For Pecuniary Profit)~ shall exist

perpetually or until such time as the same becomes dissolved

or merged.

Kevin is always the man to get the job done, said Suzanne Green, Arelys Leon and Ed Shaw, representing Homo­sassa Walmart. They laughed when they related that Kevin's trademark reply when co-workers and customers say hello is, "Hi. I'm really very busy."

Kevin is an inspiration to ev­eryone in the store, Green said. "He does the hard work and never loses that wonderful attitude."

Whether it's his jobs at Nature Coast Screen Printing and the Adult Day Training (ADT) Key Training Center workshop, his apart­ment and friends, his church or his acting and singing, he knows who he is, what he's about and where he's headed.

Davidson, 48, talked about his life.

"I was born in New Orle­ans, Louisiana, but I lived in Philly," he said. "My mom and dad came here in 2005 and lived in Dun­nellon and I moved to Ocala. I worked at Easy Street, where they have arcade games - I did the ticket taking and gave out prizes. I came to the Key Center in February 2011 and I've worked here at Nature Coast Printing since October 2012."

He lives in his own stu­dio apartment, and he and his fellow Key Center neighbors, about eight of them, all go out together on Friday nights to a movie or out to dinner.

On Sundays, he serves as assistant to the pastor at Pleasant Grove Church, the newly formed congre­gation at the Key Training Center Inverness campus, the former First United Methodist Church.

"Every Sunday, I make all the announcements about . Vacation Bible School or other activities like Wacky Wednesday or Bible Study and pizza," he said.

If you ask him about his print shop job, Davidson will tell you that he han­dles everything - clean­ing the screens, putting

He calls himself an "all­around" person.

"I've been to college at Philadelphia Community College, I've done com­puter classes, Zumba, cooking, math, science and I learned about Flor­ida panthers," he said.

The only thing missing, he said, is his dream to open his own supermarket with food and clothing for sale. • . "It's been my dream liince 1988," he said. "I just wish my. mom could be here to see how far I've come. She passed in Octo­ber. ... But coming to work makes me feel good. I've got my personal lifu, I've got my church, I've got my job. What more can I ask for? Well, there's one thing people keep throwing at me. They say, 'You should run for president!'"

"Troy works harder than anyone I know," Mauldin said. "He's always smiling, and he tries really, really hard."

His job coach, Laura Pridham, called him a "take-charge" guy and a leader who could run the town if he had a chance.

That's not a foreign idea to Davidson. He's thought about running for office, and when people ask him if he'll run he tells them no, that he's young enough but his priorities are his friends and co-workers at the Key Center, helping them and serving them.

"I'm a fun-loving per-. son," he said. "I make sure

everybody has a smile on their.. race every day. I'm here ·to help the other guys and· boost their morale. Sometimes on the bus theY'll be complaining and they say, 'Can you tell us a stocy?' I say, 'Sure, I've got one in my head.' I make up

Example: Tylenol 325 mg. 2 tablets for fever greater than 101 degrees. Notify physician if fever is 102 degrees or greater or if fever persists beyond 3 days.

A:ll"medicationn:equiFe.fabels including over-the-counter medications. 1) Creams, lotions, eye and nose drops, ointments and others that are dispensed one

container inside another container; need to be labeled (a bottle or container of medication that comes in a box). If the label is on the outside container (the box), the medication is to be kept inside the container for storage. If the inside container is labeled, the outside container can be discarded.

2) Sample medications cannot be used because there is no label on them. 3) When pouring liquid medications, place the labeled side against the palm of your

hand to avoid staining the label. The label must be legible at all times.

Staff brining individuals to doctor and dental appointments must advocate for the individual if they are not deemed responsible to dispense their own medications. Staff is to ensure any prescriptions written by the physician are written within the above policies guidelines. Attached is an over-the-counter medication sheet that was created to assist you in describing what is needed on a prescription. The individual's doctor or dentist can complete, sign and date this in order to use the medications listed. It must then be copied and the original given to the pharmacy for labeling over-the-counter medications when you purchase them. If the doctor chooses to use this form, thev must fiii in the dose for each medication. After that all they need to do is circle the frequency the medication is given, when to contact the physician and any other special considerations. This works best to have this signed by the doctor at the same time the annual physical is being done. It should be up-dated during an)' physician's visit and it will oniy be good for one year after the date it is signed by the physician.

We hope this will assist you in obtaining over-the-counter medications and will send any additional forms when the new Medicaiion Poiicy is sent from Tallahassee.

Description of Intended Construction, Rehabilitation and Renovation

The Citrus County Association for Retarded Citizens, Inc. ("CCARC"), which is locally known as and d/b/a The Key Training Center, as Applicant, proposes to develop along with the assistance of co-developer, N Vision Communities, Inc. ("N Vision"), a Community Residential Home to be known as Detmer Place, to provide additional permanent supportive housing to persons with developmental disabilities, as previously described in Exhibit A -Question B, herein. Established in 1966, CCARC was founded to unlock and open the door of opportunity for people with developmental disabilities. As a designated 501(c)(3) organization, N Vision was founded to create, preserve, expand, rehabilitate, and otherwise enhance the quality of housing supply available to persons with special needs or disabilities, the elderly, and/or those of low or moderate income. Detmer Place will be a single-family home style that blends in well within the surrounding neighborhood.

Detmer Place will be a Community Residential Home featuring individual bedrooms with private bathrooms for up to six (6) intellectually disabled residents. CCARC currently has fifteen (15) prospective residents on its waiting list for residential services/housing. Detmer Place will offer these in need of such housing a wonderful house to call their home and where they can be safe and thrive in a loving and caring environment.

This newly constructed home will have a similar layout as that of other successful Community Residential Homes that CCARC developed in the 1990s, and continues to own and operate, and will be modeled directly after the Community Residential Home known as "Country Walk," as approved for grant funding under FHFC RFA 2013-005 that completed construction in in July 2015 located in the City of Inverness. Each resident will enjoy their own private bedroom and bathroom, and they will be able to enjoy a large family room with separate kitchen and dining room areas, allowing for spacious and comfortable living, and allowing the residents to feel '"At Home" while promoting independence as well. This family atmosphere will encourage social interaction with other residents, yet each will still have their private bedroom for privacy. Detmer Place will be located on property that CCARC will acquire that will be located in a quiet neighborhood and close to amenities such as parks, shopping, and recreational activities.

As referenced above, this new home will offer private bedrooms for each of the six intended residents of Detmer Place by providing six individual, fully-furnished, bedrooms. All of the bedrooms (100%) will be fully accessible in accordance with the 2012 ADA Accessible Design. Instead of providing just the required number of bathrooms, Detmer Place

will feature private six (6) individual bathrooms for each resident, twice the number required, in order to promote independence and quality of life among the residents.

The kitchen area is often the center of conversation at certain meal times. Residents will receive three nutritious meals each day with snacks, as appropriate. All menus are developed in conjunction with a registered dietitian, and the dietitian must provide written approval for each menu. While the meals for the residents will be prepared in Detmer Place's kitchen by the Resident House Manager, residents will be encouraged to participate by learning basic cooking skills, table-setting techniques, and dish washing. Not only does this teach important life skills, but it opens the door to increased social interaction between the residents. At the center of the kitchen will be the required full size range and oven. In addition, Detmer Place will feature a full-sized refrigerator. All appliances will be Energy Star qualified.

Detmer Place plans to incorporate various energy saving features in addition to those Green Building features committed to in this application. Such additional features include a programmable thermostat, energy star qualified lighting in all open and common areas, and low-E vinyl windows.

As with all CCARC group homes, Detmer Place will be wired for a fire alarm system throughout the house, including all bedrooms and bathrooms. Detmer Place will have a house fire sprinkler system in efforts to reduce risk of resident fatalities and injuries.

Detmer Place will be a smoke-free house, with no smoking in the house or on the premises. We want our residents to be able to live in a clean, fresh, well-ventilated home promoting healthy living.

In addition to health matters, life safety is one of the major concerns of CCARC. Each licensed facility meets the standards of its licensing agency by incorporating ease of ingress and egress function into the design, and which also includes annual fire inspections, health inspections, fire drills and hurricane/tornado drills. Residents are required to participate in these drills. Each residence has a written evacuation plan, first aid kit and fencing around pool areas, if any. A Comprehensive Emergency Management Plan (CEMP) approved by the Citrus County Sheriffs Office is fully operational and updated on an annual basis. Detmer Place will be added to the CEMP as filed with the Citrus County Sheriffs Office. This, and participation in the safety drills, will aid in overall health, safety, and stability for all residents of Detmer Place.

Additional safety and security will be provided by decorative fencing along the property lines. Decorative plants and shrubs will be added along the fence to increase curb appeal, not only for the surrounding neighborhood, but especially for the enjoyment and serenity of the residents of Detmer Place.

Cleanliness is highly valued and all interior areas will be kept neat and orderly with all appliances in working order. Residents must keep their rooms neat and may assist in light household chores. This will enable the residents to feel a sense of pride in their surroundings and enhance everyone's quality oflife. Maintenance and storage areas will be kept clean and

orderly. Maintenance needs will be documented with written work orders for the Maintenance Department, for tracking and follow-up.

A Laundry Room with Energy Star qualified full-sized washer and dryer will be included for the convenience of residents at County Walk, adding to their level of independence. This laundry room will be conveniently located to an area utilized for social interaction by the residents where they can enjoy games, working out, or other recreation. In addition, Detmer Place will feature a storage area providing for a safe, clean, and dry place to store bicycles and other personal property of the residents that may be too large or too numerous to keep in individual bedrooms.

All Community Residential Homes are nicely furnished and reflect a warm, family-style setting. All homes encourage group activities to allow the residents to feel like they are part of this new family in their new home. Social interaction will be encouraged among the residents of Detmer Place. Inside the house, an area will be set up for the playing of board games or cards, and electronics will be available for listening to music. The living area will contain a large flat-screen television and comfortable furniture for all residents to enjoy and encourage guests and family members to visit with them. As we have found many of our residents in our other Community Residential Homes like to have their own computers or tablets, Detmer Place will offer internet Wi-Fi that will be incorporated throughout the house.

Socialization will be further enhanced at Detmer Place with the inclusion of a back-yard patio, complete with BBQ grill/pit, picnic area, and attractive patio furnishings. Family and other guests will be encouraged to visit and participate in the many activities the residents have to choose from.

Each resident will have the opportunity to choose the color of their room, and be able to decorate the room as they wish so, allowing the residents to feel a sense of ownership. The interior of the house will be decorated with specific colors that are geared toward promoting comfort, health, the feeling of security, and overall promoting the quality of life. Research has shown that certain colors promote different types of behavior, and the behavior that we are striving for is the one that promotes calm, happy feelings. We have a team of counselors that have studied environmental behavioral promoters that can affect the well-being of a person's mood, most importantly people with developmental disabilities. We have taken into consideration lighting throughout the house, as well, as some residents may be sensitive to certain types of lighting that can promote headaches, blurred vision, and an uncomfortable feeling. We will use soft lighting throughout the house that will promote a healthy and relaxed environment for the residents.

In adding to the supply of units that serve persons with developmental disabilities, Detmer Place will provide a home for its intended residents that will be well-built, will meet all 2012 ADA building codes and standards, and will contain additional features and amenities as described herein that will enhance the residents' health, safety, stability, socialization, level of independence, and quality of life. In evaluating the cost to develop Detmer Place, as described herein, CCARC estimates the requested Florida Housing Finance Corporation grant to be in the

approximate amount of $375,000. Additionally, CCARC anticipates the following Qualifying Financial Assistance for the development of Detmer Place:

Detmer Place will be a single-family home style that blends in will within the surrounding neighborhood that is chosen for the home. The property that will be acquired for Detmer Place will be done with the intent of providing a community residential home that the residents will enjoy, take great pride in, and compassionately call "their home".
