CHOICE Regional Health Network is a nonprofit collaborative of health care leaders in a five-county region that in-
cludes Grays Harbor, Lewis, Mason, Pacific, and Thurston counties. The organization was founded in 1995 with the mis-
sion to improve community health in Central Western Washington through the collective planning and action of health
care leaders. The organization’s board of directors consists of CEO-level executives from hospitals, community health
centers, public health, and behavioral health agencies, and the region’s leading medical home. We have partnered for
years with the region’s health care providers, local governments, and a large number of social service organizations on a
wide range of community health improvement initiatives.
CHOICE works in three core program areas, which include:
1. improving access to care and consumer education;
2. care coordination and integration; and
3. health system planning and transformation.
A key strategy of CHOICE is to serve as a neutral convener to bring together leaders in our region to identify issues, de-
velop innovative solutions, and implement agreed-upon actions. CHOICE provides skilled facilitation and project man-
agement services to drive the work forward and keep projects on track, ensuring active engagement of stakeholders and
a focus on outcomes. CHOICE has two decades of experience leading successful community engagement and regional
health improvement efforts.

Navigator Network Lead for Second Open Enrollment Period
The Washington Health Benefit Exchange contracted CHOICE as a Lead Organization for a second year of health insur-
ance enrollment assistance services. Between October 1, 2013 and September 30, 2015, CHOICE’s seven-county network
of Navigators enrolled over 30,000 individuals into affordable health plans. (See page 9.)

Accountable Community of Health Designation
CHOICE serves as the backbone organization for the Cascade Pacific Action Alliance (CPAA), which was designated as
one of two pilot Accountable Communities of Health by the Washington State Health Care Authority in January 2015.
As part of the pilot designation, the CPAA began a pilot project that identifies youth with behavioral health challenges,
and connects them with community-based interventions and treatment services. In July 2015, the CPAA was fully desig-
nated as an Accountable Community of Health and began its plans for long-term sustainability.

Outreach for Prenatal Oral Health Begins
CHOICE’s Access to Baby and Child Dentistry (ABCD) program launched an outreach initiative to encourage pregnant
women to get dental care and treatment for themselves and their babies to foster preventative habits that will improve
their babies’ long-term health. (See page 7.)

In 2015, CHOICE increased ABCD outreach by launching a bi-
monthly e-newsletter that goes out to partners and providers
in both counties.
CHOICE staff conducted 73 outreach events for ABCD in 2015,
including library story times, where children learned about
how to brush their teeth. A total of 337 children were enrolled
in the program in 2015.
CHOICE administers the Lewis and Thurston County chap-
ter of this statewide program, which provides oral health
education and connection to dental providers for families
with children six and under who are enrolled in Washington
Apple Health (Medicaid). The goal is to encourage Apple
Health clients to make full use of the preventative care that
their insurance provides, while promoting healthy habits.

CHOICE serves as the backbone organization for the Cascade
Pacific Action Alliance (CPAA), which was designated an Account-
able Community of Health in July 2015 by the Washington State
Health Care Authority. The Alliance is made up of a broad cross-
sector group of stakeholders from a seven-county region including
Cowlitz, Grays Harbor, Lewis, Mason, Pacific, Thurston, and
Wahkiakum counties. The goal is to improve health in our communities through aligned action on a shared agenda of
regional priorities.
The CPAA began the Youth Behavioral Health Coordination Pilot Project in 2015, which aims to identify children with
behavioral health challenges (mental health and chemical dependency) as early as possible in both education and health
care settings, and connect at-risk youth with community-based interventions and treatment services. Four pilot elemen-
tary, middle, and high school sites were selected to implement the program. By decreasing the number of school-aged
youth with unmet behavioral and physical health needs through improved care coordination, this pilot project seeks to
increase school attendance and
academic achievement.

Since 2013, CHOICE has been one of ten regional Lead Organizations for the
Washington Health Benefit Exchange’s In-Person Assister (IPA)/Navigator
program. In 2015, CHOICE’s seven-county navigator network* enrolled over
5,000 eligible individuals into Qualified Health Plans and Washington Apple
Health (Medicaid) through Washington Healthplanfinder, an online market-
place, by providing in-person assistance to potential insurance clients. As a
result of enrolling residents in health insurance, the uninsured rate in the sev-
en-county region decreased significantly from 17% in 2013 to 9% in 2015.

Thurston County Safety Net Council
In 2015, CHOICE continued to bring together health care providers and community partners to support the health needs
of the county’s most vulnerable individuals. The Council serves as the Health Hub of the local Asset Building Coalition,
in an effort to better coordinate health and social services across sectors.
Regional Care Transitions
This project promotes a sustainable and coordinated approach to improving care transitions using patient-centered in-
terventions. In 2015, CHOICE held several shared learning sessions about post-acute transitions of care and medication
management in Grays Harbor, Lewis, Mason, Pacific, and Thurston counties.
Emergency Department Consistent Care
Working with area hospitals, CHOICE continued to reduce emergency department visits and costs through the Emer-
gency Department Consistent Care Program
(EDCCP) in 2015. CHOICE staff provided regional
data management and analysis, and facilitated
multi-disciplinary care teams to promote im-
proved care coordination for high utilizers of
emergency services. The program created 2,587
care guidelines for referred clients between 2010
and 2015, when the program was transitioned to
our hospital partners.
