SUMMER CAMP PROPOSAL

“Children of the Most High-Accredited Private School” presents

A SUMMER CAMP of Academic ENRICHMENT, ENVIRONMENTAL interactions, Arts EDUTAINMENT.

ESPECIALLY this Summer….

We will provide the following;ACADEMIC ENRICHMENT in Reading and Math.ACADEMIC ENRICHMENT and TEST PREP classes with ITBS and EOC Exam.ENVIRONMENTAL studies and Hands-on inactive projects.GO-GREEN initiatives.ENVIRONMENTAL Planting and Gardening with Camp program and COMMUNITY GANDENING.OUTDOOR hiking, camping and ENVIRONMENTAL Fieldtrips.Weekly SWIM FUN and Weekly camp DISCOVERY fieldtrips.PERFORMING ARTS AWARENESS and FUN.PERFORMING EXCELLENCE in African Drumming, African Dance, Hip-Hop Dance, Visual Arts, Voice and group Choral, Clogging, Acting and group Drama.End of SUMMER PERFORMING ARTS RECITAL.End of SUMMER Visual Arts AUCTION.End of SUMMER ENVIRONMENTAL take home PLANTING and CROPS.

Facilitated through, “Children of the Most High”/ACCREDITIED PRIVATE SCHOOL.LICENSED through “BRIGHT FORM THE START” SUMMER PROGRAM.LICENSED USDA FOOD PROGRAM serving BREAKFAST,LUNCH and SNACK.

PROGRAM COSTThe Summer Came program will fund itself, exiting as “For Profit” entity.

Parents will be charged $25.00 to $45.00 weekly/ BASED UPON HOUSEHOLD INCOME.Parents will be assessed a $25.00 one time Registration fee.Parents will be assessed a Transportation fee of $3.00 per week, for Camp Fieldtrips.Program Directors and Facilitators will be paid every Friday.Program Youth Staffers will be paid every Friday.The Summer Camp Program will pay Rental cost of ________ to The City of Atlanta-Parks and Recreation.The Summer Camp Program will pay a predicted cost of _____________ year ________ for Camp equipment and supplies.

PROGRAM NEEDSThe following can be provided, donated or volunteered.

City of Atlanta Parks & Recreations/ 15 Passenger Van, for FRIDAY Fieldtrips.(Parents will be responsible for paying $3.00 per student in need of transportation for Camp Fieldtrips).*Parent SUV’s and Van (when needed). Volunteer Chaperones for Camp Fieldtrips Snacks and Bottled Water for Students attending.Summer Camp Balls, Swim toys, Educational DVD’s, Books, Arts & Crafts, planting seeds, etc.Parent Tutorial services in the area of Math and Reading.

Student & Parent DAILY RULES & REGULATIONS(Please initial next to each number, after you have read, understood and agreed upon)

1. _____ Child must be 5yrs. to 14yrs. of age to attend Summer Camp.

2. _____Program hours are from 8:00a.m. to 4:00p.m., DAILY.

3. _____Student(s) PICKED UP after 4:00p.m., Parents are responsible for paying $1.00 a minute

beginning @4:01p.m. (Must be paid in order for student(s) to attend camp the following day).

4. _____Summer Camp AFTER HOURS are from 4:00 p.m. to 6:00 p.m./added cost!

5. _____If/When Summer Camp attendees are picked up by other providers/camp attendants must

be properly informed.

6. _____Parents must sign form enclosed in packet for impromptu /off-camp grounds fieldtrips, and

all other fieldtrips listed in the Monthly Summer Camp FIELDTRIP SCHEDULE.

7. _____Summer Camp WEEKLY TUITION FEES must be paid every MONDAY MORNING in

the camp office, during DAILY SIGN IN for weekly attendance!

8. _____The Summer Camp REGISTRATION of ____________ must be paid before student(s)

begin their first day of camp.

9. _____Students must bring must bring LUNCH, SNACK and WATER BOTTLE DAILY. This

can be changed if/when Summer Camp enroll in FREE Summer Lunch and Snack.

10. _____Summer Camp T-SHIRTS must be worn DAILY, especially when attending FRIDAY

FIELDTRIPS.

11. _____REQUIRED DOCUMENTATIONS must be signed and completed, before students begin

PAYMENT CONTRACT / RULES & REGULATIONS

o WEEKLY TUITION PAYMENTS are due; every Monday morning upon student(s) arrival. If/When LATE, $20.00 LATE FEE must be paid w/ TUITION.

o WEEKLY TUITION PAYMENTS are due; Regardless of SCHOOL CLOSINGS, HOLIDAYS or PERSONAL ABSENTESS.

o Camp REGISTRATION FEE of _______________ must be paid before student(s) begin Camp. (1 camp T-shirt and Educational Workbook/grade appropriate is provided).
