Dear Colleagues,

We are delighted to welcome you to the CME course of the Rush Department of Neurological Sciences: Therapeutics and Management Updates. The organizing committee led by Dr. Deborah Hall and Dr. Adriana Bermeo-Ovalle, together with our outstanding Rush faculty, have prepared a rich educational program that is certain to bring you up to date with the standards of care in multiple neurology subspecialties, while also giving you a glimpse into what’s in store for the future in neurology care. We have also included panel discussions with experts on different topics, to offer you the opportunity to get answers to your questions first hand, and within the collegial environment that embodies our RUSH institution’s values.

We would also like to take this opportunity to thank our industry and other meeting partners. Several companies are exhibiting at this course, and their support and collaboration is essential to the success of this meeting. I hope you take time during your breaks and the welcome reception to visit them in the exhibit hall.

The Rush Department of Neurological Sciences is experiencing exciting times, with the growth and strengthening of our programs, and impact both regionally and nationally. Indeed, we are already starting to execute our plan to expand our clinical care, educational programs, and clinical research. One example of through the planned expansion of services with the opening of a new ambulatory care building on the main RUSH campus, in the next couple of years. THERAPEUTICS AND MANAGEMENT UPDATESThis course will provide neurologists with a comprehensive update in therapeutics and management across the major areas of neurology. Content is designed to be engaging and will be delivered in lecture, panel and video format by subspecialty faculty in each area of neurology. The following topics will be included: movement disorders, multiple sclerosis, dementia, stroke, epilepsy, critical care, emergency neurology, headache, and neuromuscular disease. Education covers state-of-the-art practices and evidence-based review with the goal of improving care for your patients with neurological diseases. At the conclusion of the meeting, attendees should be able to provide modern management of neurologic disorders based on the most current data and integrate the neurological and neurosurgical management of patients with acute neurovascular disease, movement disorders, brain tumors and epilepsy.

FACULTY

DEBORAH HALL MD, PHD is an adult neurologist and movement disorder specialist at Rush University Medical Center in Chicago. She received her MD from Indiana University and her PhD from University of Colorado, where she completed residency and fellowship. She has training in genetics, epidemiology and human subjects research. She conducts research primarily in two areas. Her work in Parkinson’s disease focuses on early interventions, such as neurotrophic factors and exercise, genetics and genomic causes of disease, and treatment for complications including falls. She also conducts research in the field of ataxia, specifically fragile X-associated disorders, by investigating epidemiology, clinical features of movement and balance, and interventions. She has been National Institutes of Health funded for the last ten years as a primary investigator and she received the Rush University Faculty Award for Research in 2014. She is the Chair of the Rush Institutional Review Board, which reviews research studies at Rush. She has a busy clinical practice focused on movement disorders within the Department of Neurological Sciences at Rush. She is the Director of the FXTAS Clinic at Rush and the Movement Disorder DNA Repository within the Section of Movement Disorders at Rush. In addition, she is Clinical Director of the Neuroscience block for the first year medical students at Rush University.

ZOE ARVANITAKIS, MD, MS, is the Acting Chairperson of the Department of Neurological Sciences, at the Rush Medical College of Rush University. She is the Medical Director of the Rush Memory Clinic, at the Rush Alzheimer’s Disease Center. A Professor of Neurological Sciences, she is a board-certified neurologist (certified by the American Board of Psychiatry and Neurology, with re-certification) and subspecialist in cognitive neurology and dementia, with fellowship training from the Mayo Clinic. She has more than two decades of clinical experience in cognitive clinical care, research, and education. The Rush Memory Clinic, under the leadership of Arvanitakis, provides expert subspecialty clinical care to improve the overall health and well-being of older adults with memory and cognitive-behavioral changes through expert diagnosis, care planning, and opportunities for research participation. Arvanitakis is recognized as an effective and energetic educator who teaches at the medical school and within Rush, as well as at national conferences and meetings, and has received an award from Rush for her excellence in mentoring. Arvanitakis is also an academic clinical neuroscientist with expertise in biomedical research, with a Master of Science (MS) in Clinical Research, and subspecialization in epidemiology. Arvanitakis is Principal Investigator of research grants from the National Institutes of Health (NIH), including investigator-initiated grants (R01s), and is Principal Investigator of clinical trials and co-investigator on several other NIH-funded and additional projects. Arvanitakis has published original research in peer-reviewed medical journals in the field of neurodegenerative, vascular, and metabolic cognitive impairment and dementia, including publications on the topics of Alzheimer’s disease and mechanisms of dementia. In addition to her many publications, Arvanitakis has also co-edited a medical textbook. Her collaborative research leverages clinical, laboratory, neuroimaging and other data, and human biospecimens, from several large, epidemiologic, longitudinal, clinical-pathologic cohort studies of aging at the Rush Alzheimer’s Disease Center. She serves on numerous national and international funding agency, scientific journal, and meeting abstract review panels. She is a member of the NIH Neurological, Aging and Musculoskeletal Epidemiology (NAME) study section. Arvanitakis is internationally recognized scientific speaker in the field of neurologic aging and dementia. She is a past chair of the Geriatric Neurology Section of the American Academy of Neurology (AAN), the largest professional organization of neurologists in the world. Arvanitakis is an elected Fellow of the American Academy of Neurology (FAAN) and an elected Fellow of the American Neurological Association (FANA).

JAMES J. CONNERS, MD, MS, is an associate professor in the Department of Neurological Sciences at Rush University Medical Center. He is head of the section of cerebrovascular diseases. He received his medical degree at the University of Illinois College of Medicine at Chicago and a Master of Science degree in neuroscience from Loyola University, where his research focused on neuronal regeneration. He then completed a medical internship, neurology residency and vascular neurology fellowship at Northwestern University. His interests include the acute treatment of stroke, cryptogenic strokes and stroke prevention.

MITRA AFSHARI, MD, MPH, is an assistant professor of Neurology at Rush University Medical Center in Chicago, the newest member of the Faculty in the Section of Movement Disorders. She is originally from Chicago, where she completed her undergraduate, medical school, and residency training at Northwestern University. She completed her fellowship in Movement Disorders at the University of California, San Francisco (UCSF), where she received comprehensive training in neurostimulation and neuromodulation for various Movement Disorders. During medical school, she also obtained her Masters’ in Public Health and traveled to Mexico, India, and Uganda for both clinical and research-related trips.Clinically, Afshari is adept at making decisions as it pertains to deep brain stimulation candidacy and programming, as well as performing botulinum toxin injections for dystonia, including limb injections. In addition to seeing patients, her primary research interests lie in Parkinson’s Disease, as it pertains to epidemiology, biomarkers, and clinical trials; deep brain stimulation therapy and other interventional therapies for advanced Movement disorders; telemedicine, teleneurology, and telerehabilitation; and palliative/supportive care in advanced Parkinsonian disorders. She also has interest in quality-improvement projects to enhance patient care, and medical student/resident education.”

ADRIANA BERMEO-OVALLE, MD in an associate professor in the Department of Neurological Sciences at Rush University medical center. She holds board certification in Neurology, Central Clinical Neurophysiology and Epilepsy. Dr. Bermeo completed medical school at Universidad Nacional de Colombia in her native Bogotá, Neurology residency at Ponitficia Universidad Javeriana in Bogotá and Rush University Medical Center and Fellowship in Epilepsy and Clinical Neurophysiology at The Cleveland Clinic and University Hospitals of Cleveland. She is the Director of Laboratory of Electroencephalography at Rush. He areas of research interest include: Sudden death in Epilepsy, Psychiatric co-morbidities in epilepsy, Psychogenic non epileptic seizures and Electroencephalography. Dr Bermeo is also the Chair of the Professional advisory board of the Epilepsy Foundation of the Greater Chicago region, Chair of the Chicago Epilepsy Centers Consortium as well as a fellow and an active member of the American Clinical Neurophysiology Society (ACNS) and the American Epilepsy Society (AES).

FACULTY

RICHARD WEBSTER (R. WEBSTER) CROWLEY, MD, joined the department in 2016 after several years as a member of the neurosurgical faculty at the University of Virginia. A board-certified neurosurgeon specializing in cerebrovascular and endovascular neurosurgery, Crowley has close to 100 publications in peer-reviewed journals and book chapters. His research interests include stroke, aneurysm treatment and subarachnoid hemorrhage. Crowley’s clinical practice focuses on arteriovenous malformations, brain tumors, carotid artery disease, cerebral (brain or intracranial) aneurysms, cerebrovascular disease, hemangioblastomas, ischemic stroke, pseudotumor cerebri, subarachnoid hemorrhage, and transient ischemic attack (mini-stroke). Crowley completed his undergraduate studies at the University of Iowa in 1998, where he played football under legendary coach Hayden Fry. He received his medical doctorate at The George Washington University School of Medicine in 2005. Crowley completed his neurosurgical residency training at the University of Virginia, during which he also performed a fellowship at Auckland City Hospital in Auckland, New Zealand. Following completion of his residency, he completed an endovascular neurosurgery fellowship at the Barrow Neurological Institute in 2013.

RIMA M. DAFER, MD, MPH, is a professor of neurological sciences at Rush University and an active member of the stroke team at the Rush University Medical Center. She is a stroke and headache expert, mainly studying the link between migraine and stroke. She has been a principal investigator on numerous clinical trials in acute stroke intervention and secondary stroke prevention. She sits on the American Board of Vascular Neurology Examination Committee, is a fellow at the American Heart Association and is a past member of the Michael Pessin award at the American Academy of Neurology. She is a clinical educator and has received numerous teaching accolades as best teacher of the year by neurology residents and medical students. She has written numerous articles and chapters on stroke management, stroke in young adults and uncommon causes of stroke.

ANDREW DORSCH, MD is an assistant professor in the Department of Neurological Sciences at Rush University Medical Center. He received his medical degree from Case Western Reserve University. He completed his neurology residency at the University of California, Los Angeles followed by a fellowship in neurorehabilitation. His major interest is the recovery of function after neurologic injury. He is particularly interested in utilizing wearable and wireless technologies to quantify and optimize the process of recovery as it occurs outside of the hospital and clinic settings.

JORI FLEISHER, MD, MS, is an Assistant Professor of Neurological Sciences at Rush University. She is a movementdisorders fellowship-trained neurologist with a special interest in the intersection of movement disorders, geriatrics, and palliative care.Dr. Fleisher was on faculty at New York University for 3 years where she piloted a novel model of care for patients with advanced PD,before joining Rush in 2017. Supported by the NIH/NINDS and CurePSP, Dr. Fleisher has several research studies underway focused oninterdisciplinary home visits, telemedicine, and caregiver peer mentoring to improve the lives of patients and families living with PD andrelated disorders. Dr. Fleisher is also studying the impact of karate on mobility, mood, and cognitive outcomes in people with PD, andinvestigating the prevalence and risk factors for PD in low-resource settings. Dr. Fleisher has been recognized as an Emerging Leader andoutstanding patient advocate by the American Academy of Neurology.

NEUROLOGICAL SCIENCES FACULTY

RAJEEV K. GARG, MD, MS received his medical degree with honors from the University of Illinois at Chicago and completed his residency in neurology at Northwestern Memorial Hospital in Chicago. After completing his chief resident year, Garg stayed at Northwestern to complete a fellowship in neurocritical care. He received his Master of Science in clinical research from Rush University and is board-certified in neurology, vascular neurology, and neurocritical care. He has clinical and research interests in secondary ischemic injury in acute intracerebral hemorrhage, neuroimaging in brain injury, and outcomes based research in the Neurointensive care unit. He has received numerous honors, including the James Scholars Program for Independent Study and Alpha Omega Alpha Honors Medical Society. He is a member of the American Academy of Neurology, the Neurocritical Care Society, the Society of Critical Care Medicine, and the American Stroke Association. He is currently principal investigator of several funded clinical research studies involving patients with acute intracerebral hemorrhage.settings.

LAUREL J. CHERIAN, MD, MS is an assistant professor in the Department of Neurological Sciences at Rush University Medical Center. She is the director of the Vascular Neurology Fellowship Program and also serves as a faculty advisor to the medical school. Cherian received her medical degree from Loyola University Chicago Stritch School of Medicine of Medicine in Maywood, Ill. She completed neurology residency at the University of Chicago and a vascular neurology fellowship at Rush University Medical Center. In addition to her clinical practice, she has completed a Master of Science degree in clinical research. She is a member of the American Heart Association and the American Academy of Neurology and is board-certified in neurology with a subspecialty certification in vascular neurology. Her clinical interests include peripartum stroke, intra-arterial therapy for stroke, post-stroke depression, and cerebral amyloid angiopathy.

CHRISTOPHER G. GOETZ, MD, is professor of neurological sciences and pharmacology at Rush University Medical Center in Chicago and serves as director of the movement disorders program. Goetz is also the United Parkinson Foundation Professor of Neurology. He is the director of the Rush Parkinson’s Disease Foundation Research Center. Dr. Goetz is a senior Fulbright Scholar and worked for two years at the College de France in Paris. He has served on several national and international advisory boards, including the Tourette Syndrome Association Scientific Advisory Board and Dystonia Medical Research Foundation Advisory Board. He is a reviewer for several journals and is a McHenry Award recipient of the American Academy of Neurology. He is a member of the Movement Disorder Society, the American Neurological Association, the American Academy of Neurology, The American Olser Society, and the French Neurological Society. Dr. Goetz has published over 450 peer-reviewed articles and over 200 book chapters in the field. He has also published and co-authored fourteen books and monographs. He has been one of two co-editors-in-chief of the Movement Disorders journal, and is the former editor-in-chief of Clinical Neuropharmacology. He is the past history section editor of the Archives of Neurology journal, and he has served an editorial board member of the Archives of Neurology. He is an advisory editorial board member of the Journal of the History of the Neurosciences, as well as reviewer for several neurology journals. He has received the American Academy of Neurology Senior Research Award in Movement Disorders (2015) and the Presidential Stanley Fahn Award from the International Parkinson and Movement Disorder Society (2009). He is the president-elect of the International Parkinson and Movement Disorder Society. His major interest is the pharmacology of various movement disorders, including Parkinson’s disease, Gilles de la Tourette’s syndrome, Huntington’s disease and pharmacologic studies of dystonia. He is particularly interested in non-motor aspects of movement disorders, including specifically, hallucinations, cognitive decline and depression. Further research efforts have focused on scale development for evaluating movement disorders, and he has led the Movement Disorder Society Task Force to develop a new version of the UPDRS, termed the MDS-UPDRS, unveiled in 2008. He has a special interest in placebo effects and how they influence the interpretation of clinical trials in Parkinson’s disease and dyskinesia. He has also led pharmacologic and surgical efforts to treat movement disorders. Dr. Goetz and his team are currently investigating the safety and efficacy of gene therapy in patients with Parkinson’s disease and are involved in other studies aimed to prevent the progression of Parkinson’s disease in its early stages. He is an active researcher in the history of neurology and has written extensively with his major research interest being the study of nineteenth century French and U.S. neurology with particular interest in J-M. Charcot and his school. He has written the definitive biography of Charcot (Charcot: Constructing Neurology, Oxford University Press) and Charcot: The Tuesday Lessons (Raven Press).

SAYONA JOHN, MD, completed her residency training from the University of Mississippi in 2005 and fellowships in neurocritical care and stroke from the Massachusettes General Hospital in Boston in 2007. She joined Rush University in 2008 after working for one year at the University of Texas Southwestern in Dallas. She became the medical director of the neuroscience intensive care unit in 2011 and the section head of critical care neurology in 2012. Her research interests primarily involve patients with intracerebral hemorrhage. She was nominated as an executive committee member for the NIH-sponsored CLEAR III trial and was the top enroller for this trial.

NEUROLOGICAL SCIENCES FACULTY

RABIA MALIK, MD is a neuromuscular attending at Rush University Medical Center. She is board certified in Neurology and Clinical Neurophysiology. Malik received her medical degree from Aga Khan University in Pakistan. She completed her Neurology Residency at University of Iowa Hospitals and Clinics in Iowa City, IA and then went on to complete a Clinical Neurophysiology Neuromuscular fellowship at UCSF Medical Center in San Francisco, CA. She is board certified in neurology and Clinical Neurophysiology. She is a member of the American Academy of Neurology and the American Association of Neuromuscular and Electrodiagnostic Medicine. Currently, Malik serves as the EMG liaison for the Rush Clinical Neurophysiology Fellowship. She has a busy clinical practice in the Neuromuscular Section and performs diagnostic EMG studies as well as skin biopsies.

JOO YEON NAM, MD graduated from Northwestern University Feinberg School of Medicine in Chicago and completed her neurology training at Beth Israel Deaconess Medical Center in Boston, followed by neuro-oncology fellowship at University of Texas MD Anderson Cancer Center. Her clinical and research interests are novel treatment of primary brain tumors, especially in gliomas, as well as supportive care for brain metastasis and primary brain tumor patients.

NICHOLAS D. OSTERAAS, MD is an assistant professor of Neurologic Sciences at Rush University Medical center. He completed internship in internal medicine, residency in neurology and fellowship in vascular neurology all at Rush University Medical Center. He is actively investigating coagulopathy occurring post administration of intravenous tissue plasminogen activator and has interest in acute care of ischemic stroke patients. He has presented at several international conferences and has multiple manuscripts under submission for publication. He is a member of the American Academy of Neurology and American Heart Association.

RYAN JACOBSON, MD is an assistant professor in the neuromuscular division in the Department of Neurological Sciences at Rush University. He received his medical degree from the University of Illinois. He completed his neurology residency at the University of Michigan where he was chief resident. He also completed his fellowship in neuromuscular medicine at the University of Michigan. He is primarily a clinical neuromuscular specialist, evaluating and treating patients with a broad range of conditions affecting the nerves, muscles and neuromuscular junction. He is also involved in the education of medical students, neurology residents, and fellows at Rush University.

FACULTY

GIAN PAL, MD, MS is an adult neurologist and movement disorder specialist at Rush University Medical Center in Chicago. He received his MD from Robert Wood Johnson Medical School, completed his residency at Georgetown University, and then came to Rush University to complete his movement disorders fellowship and a master’s degree in clinical research. His main area of clinical expertise includes surgical treatments for movement disorders, particularly deep brain stimulation. He conducts research primarily in two areas. His work in Parkinson’s disease (PD) focuses on understanding the cognitive effects of deep brain stimulation (DBS) in genetic forms of PD. In collaboration with a gastroenterologist, Ali Keshavarzian, MD, Dr. Pal conducts research aimed at understanding the role of the gastrointestinal system in the pathogenesis of PD. His research has been funded by the Parkinson’s Disease Foundation (PDF), Cohn Family Foundation, and the Department of Defense. He regularly presents his research at national and international meetings including the American Academy of Neurology and the International Congress of Parkinson’s Disease and Movement Disorders.

MARVIN A. ROSSI, MD, PHD, is a board-certified neurologist and epileptologist with a doctorate in molecular biophysics. Rossi has over 25 years of experience within regional NAEC-affiliated specialized epilepsy centers (predominantly the Rush Epilepsy Center) as a translational neuroscience researcher and subspecialist clinician.

STARANE SHEPHERD, MD, is an assistant professor of Neurological Sciences in the Section of Neurocritical Care. He completed an accelerated 6-year combined BS/MD program at Howard University in Washington, D.C. and subsequently completed a neurology residency in Miami, FL at Jackson Memorial Hospital/University of Miami. After completion of the residency, he completed a neurocritical care fellowship at the Harvard Medical School, Massachusetts General Hospital and Brigham and Women’s Hospital combined training program in Boston, MA. He is board-certified in neurology by the American Board of Psychiatry and Neurology. He is a member of the American Academy of Neurology, Society of Critical Care Medicine, Neurocritical Care Society and the National Medical Association.

THOMAS SHOEMAKER, is an assistant professor of neurology in the MS Center at Rush. He completed undergraduate education at University of Illinois at Urbana Champaign. He attended Rush Medical College followed by internship and residency at University of Pittsburgh medical center. He was a National MS Society Sylvia Lawry fellow and completed fellowship at Johns Hopkins in neuroimmunology with a focus on MS.

NEUROLOGICAL SCIENCES FACULTY

DUSAN STEFOSKI, MD, a Fellow of the American Academy of Neurology, has been doing novel research in the field of multiple sclerosis(MS), both original and collaborative, with researchers in the USA and internationally. In association with Floyd A. Davis, MD, he is the coinventor of dalfampridine’s therapeutic efficacy in MS, leading to its approval under the US trade name Ampyra (Acorda Therapeutics) andFampyra elsewhere (Biogen Idec) as the first-ever function-restoring treatment to improve walking and leg strength in people with MS. Dr.Stefoski, with his team of professionals, specializes in advanced and experimental treatments for MS. Among these are immune ablation,natalizumab, cladribine, dimethyl fumarate, fingolimod, alemtuzumab, mycophenolate, cyclophosphamide, and intrathecal baclofen. Researchinvolvements include immunologic phenomena in MS and Devic’s disease, and testing new treatment modalities for MS and PML. In additionto having contributed to over one hundred scientific and scholarly publications, he has served on the review boards of professional journals,including Neurology and Archives of Neurology. He lectures nationally and internationally.

AIMEE SZEWKA, MD, is an assistant professor in the Departments of Neurological Sciences and Ophthalmology at Rush University MedicalCenter. She received her medical degree from Indiana University. She completed her Neurology residency at Rush University Medical Centerand then completed Neuro-Ophthalmology fellowship training at Emory University. She is currently practicing neuro-ophthalmology at RushUniversity Medical Center. She is an active member of the American Academy of Neurology and a Fellow of NANOS (North American Neuro-Ophthalmology Society). She is the Assistant Dean for Clinical Education for Rush Medical College and is actively involved in resident andmedical student education. She provides Neuro-Ophthalmology curriculum to Neurology, Ophthalmology and Neurosurgery residents yearlyand teaches medical students on a regular basis as well. She has received numerous teaching awards from residents and medical students, aswell as the AB Baker Teaching Recognition Award from the American Academy of Neurology.

SIERRA MORALES, MD is a board certified neurologist and neuroimmunology specialist at Rush University Medical Center in Chicago. He earned his medical degree from Sabana University and completed his residency in neurology at Drexel University. He has been fellowship trained in multiple sclerosis and neuroinfectious diseases at Harvard University as well as neuroimmunology at Rush University Medical Center. His research interests include PML, cognition in MS and DMT’s side effects.
