What’s in your records: height, weight, temperature, blood pressure, symptoms you bring up in appt., referrals, test orders/results, prescriptions, hospitalization recommendations, previous visits, doc’s diagnosis/treatment !So basically anything you want to know about your physical health is stored in that file but you don’t have easy access to it.

scenario #1 connecting with your doctor

(bri scenario 1)

Page 12: CHART Designs

Ashley begins on the desktop version of the app

Page 13: CHART Designs

Since it’s important to keep her medical records confidential, CHART uses an eye scanning system to log in.

Page 14: CHART Designs

She uses the webcam on her computer to match her eye up to the scanner.

Page 15: CHART Designs

Once logged in, Ashley is brought to the main page of her account. From there, she can view her records (as well as those of her direct family), her doctors, insurance information, and the community section (which would include support groups and recommendations).

Page 16: CHART Designs

call email video

Ashley selects the doctors tab. She wants to contact her OBGYN about some possible issues in her pregnancy.

Page 17: CHART Designs

Once she’s selected Dr. Posey, the program opens up to his profile and contact information. From there she can call, email or video chat with her doctor.

Page 18: CHART Designs

She wants to talk face to face with her doctor so she selects the video chat option. She waits while it connects to her doctor.

Page 19: CHART Designs

During the video chat Dr. Posey suggests she come in to the office for a check up

Page 20: CHART Designs

Ashley leaves the computer and heads to her appointment.

Page 21: CHART Designs

When she arrives at the office, her phone alerts her that she’s been checked in via near-able technology.

Page 22: CHART Designs

It then gives her the option to update any information in her file, such as insurance information, updates in symptoms, etc.

Page 23: CHART Designs

She selects the notification and unlocks her phone, which automatically brings up the CHART mobile app.

Page 24: CHART Designs

The same eye scan technology is present on the mobile version to keep all information private

Page 25: CHART Designs

She uses the front facing camera on her phone to scan her eye

Page 26: CHART Designs

This brings her to mobile version of her account with the two most recent notifications at the top.

Page 27: CHART Designs

She dismisses one of the notifications with a swipe and then selects the other to make updates to her file.

Page 28: CHART Designs

A form unfolds for her to fill in.

Page 29: CHART Designs

Once she’s completed the form by filling in the reason for her appointment she closes the form.

Page 30: CHART Designs

She’s then returned to the main page of her account

Page 31: CHART Designs

Ashley enters the exam room.

Page 32: CHART Designs

Ashley’s phone syncs with the exam room’s interactive wall.

Page 33: CHART Designs

Once synced, the wall loads the updates done by the nurse/technician, such as weight, height, temperature, blood pressure

Page 34: CHART Designs

Enter Dr. Posey. He has Ashley’s files pulled up on his iPad.

Page 35: CHART Designs

He wants to show Ashley her sonogram so he holds down on the Sonogram Reading title

Page 36: CHART Designs

When he releases an icon pops up for him to send all the images to the interactive wall in the exam room.

Page 37: CHART Designs

Sonogram images appear on the wall so both Ashley and Dr. Posey can view them at large scale. In the corner of the wall, an audio recording is taking notes on the dialog of the appointment.

Page 38: CHART Designs

Dr. Posey selects one of the sonogram images to look at larger and in more detail.

Page 39: CHART Designs

Dr. Posey sees something slightly out of the ordinary and circles it and makes notes directly on the wall.

Page 40: CHART Designs

He then moves on to talk about Ashley’s weight. He clicks on the weight in the side bar and it unfolds to show information graphs regarding her pregnancy weight gain, organized by trimesters.

Page 41: CHART Designs

He notices that he weight is about 3 pounds over the healthy average. He circles and makes a note so she can be sure to keep an eye on it in the future. At the end of the appointment he drags the screen up to the CHART logo which saves the whole appointment and sends it into Ashley’s records.

Page 42: CHART Designs

Ashley leaves the appointment.

Page 43: CHART Designs

Her phone alerts her to the updates made to her records.

scenario #2 managing your own health

(brittany scenario 2 + closing statement)

Page 45: CHART Designs

Jacob at school.

Page 46: CHART Designs

Jacob’s phone alerts him every time he needs to check his blood sugar. Right now it’s lunch time!

Page 47: CHART Designs

She selects the notification and unlocks her phone, which automatically brings up the CHART mobile app.

Page 48: CHART Designs

Jacob also has to sign in using the secure eye scanner

Page 49: CHART Designs

Signing in is quick and easy for him.

Page 50: CHART Designs

CHART then brings up his account, a simpler version of Ashley’s. The notification reminding him to check his glucose levels is at the top.

Page 51: CHART Designs

He clicks the notification and it brings him to his daily log, which show the week so far.

Page 52: CHART Designs

He selects today and it unfolds for him to start checking his levels.

Page 53: CHART Designs

Jacob is using a new diabetic meter that syncs directly with his smartphone

Page 54: CHART Designs

CHART receives and plugs in the readings without any effort from Jacob

Page 55: CHART Designs

It then displays his current glucose levels which then helps the school nurse to calculate the amount of insulin Jacob will need before lunch as well as what his levels have been for the past week.

Page 56: CHART Designs

Once that’s complete the pre-lunch bar is filled in on the Wednesday tab as “complete.” Jacobs entries then sync with CHART and can be seen from the desktop version.

Page 57: CHART Designs

Ashley back at home.

Page 58: CHART Designs

once Jacob has completed his lunch time log, Ashley is alerted via pop up.

Page 59: CHART Designs

She selects his profile, which includes his health as well as info graphs showing his growth. She sees that the update that’s been made is lower down.

Page 60: CHART Designs

She scrolls down to the graph showing Jacob’s glucose levels from this week and sees that the most recent pre-meal entry has been made. This way she feels comfortable knowing that Jacob is taking care of his health even when she’s not around.
