President’s Message (Dr. Brad Hester) Colleagues,
The work on our new website (http://centraloregondentalsociety.org) is progressing well
and I anticipate it will go live here in the next few weeks. We have worked to incorporate
all the pertinent information that you might want concerning our society. You will find
information on the current C.E. available through our society, the current and archived
newsletters, a classified advertising section, and links to the state and national websites. I
think you will find it a great resource.
Our September meeting is fast approaching and we will be electing the new Executive Team
for their two-year terms. Positions that will need to be voted on are President, Vice-
President and Secretary-Treasurer. We will also want someone (or two) to head up the C.E.
committee. Please consider contributing your time and talent to our society.
The House of Delegates meeting for the Oregon Dental Association will be held here in Bend
in early September. Dan Bitner, Mike Shirtcliff, Tony Ramos and I will be attending as
delegates for Central Oregon. Steve Timm our state president and Gregg Jones president
elect will also be attending. Thanks to these members for their time representing our
society.

At the Kemple Memorial Children’s Dental Clinic, our mission is to improve the health and well-being of children in Deschutes County by facilitating critical preventative, educational and dental treatment services for children and young adults whose families cannot access basic dental care. It starts at 4:00 pm with the training table and dinner and the Texas Hold’em starts at 5:00 pm. The event is for both dental and non-dental players, men and women, experienced and new players. We have space for 70+ players. Common Sense, Practical, Practice Management Advice for Dental Professionals

Dear Dr. Rupert, How do you manage the patient who is obsessed with the smallest little imperfections in their teeth and restorations, and is never happy with the esthetics of their teeth or your work? Signed, Patient Who Stares at their Teeth for Too Long - - - - - - Dear Patient Who Stares at their Teeth for Too Long, There’s a population of patients who despite being drop-dead gorgeous & perfect, are convinced that they see a little longer incisal edge, tad overly-rotated tooth, too dark a filling, an overly rounded line angle, minor under-contoured tooth, or a myriad of other miniscule imperfections in their teeth or restorations that are practically impossible to discern even with loupes! To properly manage these patients, the Dentist must have a firm understanding of the inner workings of these individuals. These patients don’t just obsess over the tiniest imperfections of their teeth, but also all other facets of their appearance. Many simply have too much idle time on their hand, and have nothing better to do with their time, than stare into a mirror for long periods of time! Your job as a Dentist when confronted with such a patient is to recognize that you need to give this patient something else to keep them occupied (beside their teeth), otherwise … they will nitpick your restoration apart, and leave you 45 minutes behind schedule with the rest of your patients. I have had great success managing these patients, by pointing out other miniscule imperfections just before handing them the hand mirror to look at their new restoration. This approach immediately shifts their short attention span to a different area and away from their teeth. Some examples of comments I will make are:
I love what you’ve done with your hair, but you should have them even out the sides just a tad.
I love your outfit, but have you noticed the light color fade on the back of the shoulders?
I love your glasses, but they’re sitting a little crooked on your nose. You should have them straightened out.
The point is that these patients will not spend an hour looking at their teeth, but will instead focus on the obvious imperfection which you so kindly pointed out on their hair, outfit or glasses. You can stay on schedule if you manage the patient properly. Glad I could help, Dr. Rupert

Dear Dr. Rupert, I have a few patients who despite being regularly rinsed and suctioned, insist on bringing a small pool of spit to the front of their lips and holding it there until my Assistant comes back to suction the spit off the tip of their lips! How do I get the patient to quit grossing us all out and simply swallow the tenth of a teaspoon of their own saliva? Signed, Don’t Appreciate the Extra Spit after the Suction - - - - - - Dear Don’t Appreciate the Extra Spit after the Suction, I don’t like those Extra-Spitters either! They swallow their own saliva all day long, except when you’re doing dental work on them and then it’s suddenly some toxic stuff that needs continuous and constant suctioning. I simply do not put up with those patients and refuse to accommodate their demand for having their spit suctioned off the tip of their lips. Once again my approach to managing these patients lies in the art of distraction. You cannot allow the patient to focus on the taste or feel of the foreign substances in their mouth. What I usually say is that … there is an increased risk of adverse soft tissue reactions to the restorative materials, secondary to over-desiccation or suctioning; and that we need their natural saliva and spit to serve as a buffer to the restorative materials that are being used. The faster you memorize and say that sentence, the more effective it will be against the Extra-Spitters. Once you start throwing around long sentences and big words … the patient will settle down and quit obsessing over the tiny bit of remaining saliva in their mouth and try to figure out what the heck you actually said. By the time they figure out what you were saying, you’re done with the restoration and will have released them to go and rinse out at the sink. Glad I could help, Dr. Rupert
