Central Oregon Dental Society News CODS Update – President’s Notes Dr. Gary Dehm Here are a few notes to share with everybody: 1) Dr. Blake Drew … Congratulations! Had a beautiful baby girl, Ryan Edie Drew, 7 lbs. 15 oz. and 21.5 inches long. All are well and healthy. 2) Dr. Nyquist has retired and sold his practice to Pediatric Dental Associates, Drs. Burrus and Ford.

3) Our Continuing Education Accreditation from the AGD has lapsed and is in the process of being reestablished. 4) Dr. Michael Villano gave a wonderful presentation at our January Dental Society Meeting which included worthwhile information on Sinus pathologies, Sleep apnea/disorders, and touched on some facial reconstruction methods: pedicle grafts and cartilage transplants from ear to nose, to mention a couple. 5) Down below, we have included the flyer for the Eastern Washington Medical/Dental symposium coming up. Feel free to share. 6) Our local CE event in February, the 19th, at Eagle Crest. Open to, and highly encouraged to bring staff to update and educate on Infection Control (required element of license renewal) as well as a Periodontal Disease update. 8:30 registration, 9:00 - Noon for presentations, 3 CE available. $60 member dentist, $100 non-member dentist; $40 per office staff (member, non-member same price). 7) The Agenda for the January Dental Society Meeting is included below for those who weren’t able to attend. 7.Guest Speaker: Dr. Michael Villano Board Certified, Facial Plastic & Reconstructive SurgeryAmerican Academy of Facial Plastic Surgery Board Certified, Otolaryngology-Head & Neck SurgeryAmerican Academy of Otolaryngology-Head & Neck Surgery Dr. Michael E. Villano is a facial plastic and reconstructive surgeon practicing in Bend, Oregon. Dr. Villano received his medical training at Wake Forest University School of Medicine. After obtaining his M.D. in 1995, he went on to complete an Otolaryngology and Head & Neck Surgery residency followed by a fellowship in Facial Plastic & Reconstructive Surgery at the University of Rochester School of Medicine. Dr. Villano has been practicing in Central Oregon since 2001. Prior to medical training, Dr. Villano received an undergraduate degree in Bioengineering at the University of California at San Diego while on a Navy Scholarship. Upon graduation, he entered the prestigious Navy Nuclear Propulsion Program where he served as a nuclear submarine officer aboard the USS Haddo and the USS Hawkbill and earned such honors as the Navy Achievement Medal with Gold Star, the Navy Expeditionary Medal, and the Defense Service Medal.

Update for COCC Dental Clinic Kristi Hammerquist, CDA I wanted to update you on the fact we still have dates open for the Friday clinics at COCC. Please check your calendar and see if you can help us out. The number of people that are in dire need of dental help and have nowhere else to turn may surprise you. Again we are collaborating with DCHS, St. Charles ER, VIM, Kemple Clinic, Veterans Affairs, COCC students and the Latino Comm. Assoc. We provide x-rays, fillings and extractions. Our clinics will be on Friday mornings from 8:00 to around 1:00. You will also earn 6 CE points for your time working in our clinic with our students. February is National Children’s Dental Health Month/Give Kids a Smile: Kemple Clinic is planning several small events during the month of February – most of which are returning to schools to apply sealants, some community education programs, and screening events. As indicated in our 6 months of Screen and Seal program results (see below), we have over 1200 children to seal before the end of the school year. We only have a few acute treatment referrals for both children and more importantly, young adults and are actively contacting our volunteer dentist offices. During the month of February, local Walmarts and various pharmacies in Central Oregon are collecting donations of toothbrushes, toothpaste, floss and mouth rinse for our Smile Drive – America’s Tooth Fairy National Campaign. We welcome any product donations from your offices as well. Be sure to use the month of February to get started with the Brighter Smiles (whitening) Campaign NOW. All proceeds from Brighter Smiles definitely result in Giving Kids a Smile!! BRIGHTER SMILES (whitening) CAMPAIGN- 2016: Kemple Clinic has formally kicked-off the Brighter Smiles Campaign. It is our 13th year of the program. Several of your offices have already contacted us and are getting prepared for the MAY/JUNE event. Please contact us NOW and confirm your desire to participate – we want make sure your office is acknowledged in our extensive media campaign efforts over the next 5 months (radio, TV, newspaper, posters, wedding/event planning websites, social media, etc.) This is a great year to participate: Kemple Clinic received a matching challenge grant of $15,000 from the Maybelle Clarke Macdonald Foundation which will be awarded when we succeed in reaching our Brighter Smiles fundraising goals. At the Kemple Memorial Children’s Dental Clinic, our mission is to improve the health and well-being of children in Central Oregon by

facilitating critical preventative, educational, and dental treatment services for children whose families cannot access basic dental care. We do this

through our dental clinic facility screen and seal program events at local schools and community partner organization venues with subsequent

follow-up and referrals to volunteer dental professionals as needed. We provide portable preventative dental services to children and young adults regardless of insurability at various convenient locations. We advocate for

RECENT RESULTS FROM SIX MONTHS SCREEN AND SEAL PROGRAMS (July through December 2016): Assessment results: 4216 children were screened, various grades (elementary, middle, high school) in Bend La Pine, Sisters, and Redmond School Districts as well as Migrant Summer School and Latino Community Association. 4.9% of 4216 children were assessed as having visible signs or symptoms of serious dental needs. An immediate visit to a dentist was recommended. The range among the schools/community venues with this level of urgent needs assessment is 3.1% to 13.6% of the population screened. A personalized follow-up process on these cases is immediately engaged: School nurses, FAN’s, school administration and/or community organizations follow up with the parent immediately and makes sure the dental visit occurs in a timely manner. Kemple Clinic also advocates for the child to ensure that these cases receive immediate attention. Thanks to all of you who have responded to meet these urgent requests. And thanks to our school nurses for their dedication to this process. Sealant results: 57.7% of children screened are ready for and need sealants. The significant need for sealant applications at all grade levels (including high school) indicates that Central Oregon continues to be in a “catch-up” mode. The parental consent for sealants has improved by 5% in the schools and we are scheduling, with parental consent, 1200+ children to receive sealants over the next few months as we return to the schools to seal. It is our goal in the future to get to a very focused SAVE mode: “Seal At the Very Earliest” – seal children as the permanent molars erupt: 1st & 2nd grades and 6th & 7th grades. We will not be able to get to that SAVE level until this significant backlog of sealant needs at all grade levels is reduced. Hopefully with your preventative dental appointments and our Screen and Seal programs, we will knock down this backlog in the coming years. We have a great cadre of hygienists and dental assistant volunteers that help us with these programs. These volunteers make the events very efficient, effective and enjoyable! We can always use more help to work the screen and seal events at schools and other community venues – please encourage your staff to volunteer when they can and be sure to thank those that take the time to contribute to the improved oral health of Central Oregon’s children. Dear Dr. Rupert, I recently got married and my wife’s family is overjoyed at having a Dentist in the family. I have had everybody from her parents, to her grandparents, to her aunts/uncles/cousins … you name it, they have all made appointments to come to my office for dental work. I made the mistake of not charging the first few new family members that came in, and now … every relative that comes in expects and wants free dental services.

Where do I draw the line and how do I get myself out of this mess?

Dear Don’t Want to Treat Everybody in the Family for Free,

You can’t run a business if you’re treating everybody for free in your practice. What I did when my in-laws and distant relatives started scheduling appointments for dental work, is I mailed each of them an introductory practice brochure that I downloaded off the website of one of our local Gastrointestinal Doctors in our town.

I told the relatives and in-laws, that because of some additional training that I had acquired in my Residency, I was including a free Colonoscopy along with their Oral Evaluation as part of their New Patient Exam and that I didn’t want them to have anything to eat for eight hours prior to their Dental appointment.

The mere mention of a Colonoscopy by a relative that they would see at every Thanksgiving and family get-together made each and every in-law and relative cancel their appointment and go back to their original Dentist. Not a single relative wanted the free colonoscopy and that worked beautifully in my practice for eliminating family members that wanted free dental work.

A few weeks ago, as I was retracting the patient’s tongue and doing a crown prep on tooth #19, the patient’s tongue got loose and I hit it with my bur! I instantly stopped, told the patient that I had lacerated their tongue with my bur and followed up with the patient for a week and made sure that everything healed up properly.

The other day, I received a letter from that patient telling me that they had received their bill and that they didn’t think it was fair for me to charge them for the crown, since I had traumatized their tongue during the procedure and they had to endure so much discomfort during the healing process.

The right thing to have done would have been to tell the patient that their tongue came over the retraction mirror and hit your bur! You should never tell the patient that your bur hit their tongue!

You have to put the responsibility and blame on the patient from the moment it happens and then make a big deal about the fact that you simply cannot continue to work on them, if they are unable to control their numb tongue and keep it out of the way!

Let the patient know that you cannot see as well when their tongue is bleeding all over the place and that they are making the procedure very difficult with their unruly tongue! Explain to the patient that you have to focus on the preparation of the tooth and expect them to hold still and keep their tongue out of the way.

Sometimes, I will even get up and leave the operatory and tell the patient that I will be back when they are able to hold their tongue out of the way and that they are throwing the entire schedule for the day behind because of their inability to control their tongue!

The more blame you put on the patient, the less likely they would be to hold you responsible or even fathom the thought of not paying for the work that was done when there is some tissue trauma with the bur.

In your particular case, since you incorrectly told the patient that you had hit their tongue with your bur … you sort of took ownership of the problem and are now stuck having to extend them some sort of discount to make them happy.
