CelluliteisatermcoinedinEuropeansalonsandspastodescribedepositsofdimpledfatfoundonthethighsandbuttocksofmanywomen.WidespreadpromotionoftheconceptintheUnitedStatesfollowedthe1973publicationofCellulite:ThoseLumps,BumpsandBulgesYouCouldn'tLoseBefore,byNicoleRonsard,ownerofaNewYorkCitybeautysalonthatspecializedinskinandbodycare.Celluliteisallegedtobeaspecialtypeof"fatgonewrong,"acombinationoffat,water,and"toxicwastes"thatthebodyhasfailedtoeliminate.Alleged"anticellulite"productssoldthroughretailoutlets,bymail,throughmultilevelcompanies,andthroughtheInternethaveincluded"loofah"spongescactusfibersspecialwashclothshorsehairmittscreams[A,B,C]andgelsto"dissolve"cellulitesupplementscontainingvitaminsmineralsand/orherbs[A,B,C]bathliquidsmassagers[A,B]rubberizedpantsexercisebooksbrushesrollersbodywrapsandtoninglotions.Manysalonsoffertreatmentwithelectricalmusclestimulation,vibratingmachines,inflatablehiphighpressurizedboots,"hormone"or"enzyme"injections,heatingpads,andmassage.Someoperatorsclaimthat5to15inchescanbelostinonehour.Aseriesoftreatmentscancosthundredsofdollars.

"Cellulite"isnotamedicalterm.Medicalauthoritiesagreethatcelluliteissimplyordinaryfattytissue[1].Strandsoffibroustissueconnecttheskintodeepertissuelayersandalsoseparatecompartmentsthatcontainfatcells.Whenfatcellsincreaseinsize,thesecompartmentsbulgeandproduceawaffledappearanceoftheskin.Manyyearsago,NeilSolomon,M.D.,conductedadoubleblindstudyof100peopletoseewhethercellulitedifferedfromordinaryfat.Specimensofregularfatandlumpyfatwereobtainedbyaneedlebiopsyprocedureandgiventopathologistsforanalysisandcomparison.Nodifferencebetweenthetwowasfound.

Morerecently,researchersatRockefellerInstituteusedultrasonography,microscopicexaminations,andfatmetabolismstudiestosee"affected"andunaffectedskinareasdifferedinsevenhealthyadultsubjects(fivewomen,twomenfouraffected,threeunaffected).Theresearchersconcluded:(a)certaincharacteristicsofskinmakewomenmorepronethanmentodevelopcellulite(b)theprocessisdiffuseratherthanlocalizedand(3)therewerenosignificantdifferencesintheappearanceorfunctionofthefattytissueortheregionalbloodflowbetweenaffectedandunaffectedsiteswithinindividuals[2].

ElectricalMuscleStimulators(EMS)andIontophoresisDevices

Musclestimulatorsarealegitimatemedicaldeviceapprovedforcertainconditionstorelaxmusclespasms,increasebloodcirculation,preventbloodclots,andrehabilitatemusclefunctionafterastroke.Butmanyhealthspasandfiguresalonsclaimthatmusclestimulatorscanremovewrinkles,performfacelifts,reducebreastsize,reducea"beerbelly,"andremovecellulite.Iontophoresisdevicesareprescriptiondevicesthatusedirectelectriccurrenttointroduceionsofsolublesalts(i.e.,medications)intobodytissuesfortherapeuticordiagnosticpurposes.Theonlyapproveduseisfordiagnosingcysticfibrosis.

TheFDAconsiderspromotionofmusclestimulatorsoriontophoresisdevicesforanytypeofbodyshapingorcontouringtobefraudulent[3,4].Themostinfamousofthesedevices,theRelaxACizor,wasclaimedtoreducegirthbydeliveringelectricshockstothemuscles.Morethan400,000unitsweresoldfor$200to$400eachbeforetheFDAobtainedaninjunctionin1970tostopitssale.Atthetrial,40

witnessestestifiedthattheyhadbeeninjuredwhileusingthemachine.Thejudgeconcludedthatthedevicecouldcausemiscarriagesandaggravatemanypreexistingmedicalconditions,includinghernias,ulcers,varicoseveins,andepilepsy.

BodyWrapping

Manysalonsandspasexistwhereclientssupposedlycantriminchesoffthewaist,hips,thighs,andotherareasofthebody.Thesefacilitiesusewrapsorgarments,withorwithoutspeciallotionsorcreamsappliedtotheskin.Thegarmentsmaybeappliedtopartsofthebodyortotheentirebody.Clientsaretypicallyassuredthatfatwill"meltaway"andtheycanlose"upto2inchesfromthoseproblemareasinjustonehour."SuddenlySlender,whichlicensesbodywrapshopsintheUnitedStatesandCanada,claimsthat"wrappingworksbecausecelluliteiswaterloggedfattytissue."Aspartofitssalespitchtoprospectiveowners,thecompanynotesthatfreepublicitymaybeobtainable.ItsWebsitestates:"Becauseclientsgetdressedupas"mummies"andthen,almostmiraculously,achievemajorinchlossandstartlingimprovementstotheirfigures,localandnationalmediahavebeenoverwhelminglyreceptivetofeaturingpresentationsabouttheservice."

HomeusesystemsarealsobeingmarketedthroughtheInternet[A,B]andthroughmultilevelmarketing[X,Y].Manyofthesystemsareclaimedto"removetoxins."Somemarketerssuggestmeasuringalargenumberofbodyareasbeforeandafterwardandaddingupthedifferencestoget"totalincheslost."LifeForceInternational,forexample,recommendsaddingtheresultsof17measurements.Thisenablesminorchangesduetotemporaryeffectsortomeasurementvariationstoappeartobelargenumbers.

Noproducttakenbymouthcancauseselectivereductionofanareaofthebody.Althoughwrappingmaycausetemporarywaterlossasaresultofperspirationorcompression,anyfluidwillsoonbereplacedbydrinkingoreating[5].Theideathatherbalwrapsdetoxifythebodyisabsurd.

Cellasene

AnherbalproductcalledCellaseneisbeingvigorouslypromotedasacelluliteremedy.TheproductwasdevelopedbyanItalianchemistnamedGianfrancoMerizzi.Itsingredientsareeveningprimroseoil,driedfucusvesiculosisextract,gelatine,fishoil,glycerol,soyaoil,grapeseed,bioflavonoids,soyalecithin,fattyacids,driedsweetcloverextract,driedginkgobilobaextract,andironoxide.Theproduct,tobetakentwicedaily(orthreetimesperdayforan"intensive"program)fortwomonthsandthenoncedailyformaintenancecosts$1.50to$2.00percapsule.Here'swhatoneInternetmarketersays[followedbymycommentsinbrackets]:

Cellaseneisasafe,clinicallystudiedformulathatworksovertimeatthesourceoftheproblembelowthesurfaceoftheskin.Thisuniqueformulaofplantextractsandotherbeneficialdietarysupplementsnourishesconnectivetissuefromwithinandhelpsreducecellulite.TheherbalingredientsinCellaseneworktoincreasebloodcirculation,reducefluidbuildup,stimulatemetabolismandreducelocalizedfats.CONVENIENTANDEASYTOUSE....

YoudonotneedtochangeyourdietandexerciseroutineforCellasenetowork.ItissimpleandeffortlesstoincorporatetheeasytoswallowCellasenesoftgelsintoyourdailyregimen.

OnMarch15,1999,duringaninterviewonCNBCTV,Rexall'schiefexecutiveofficerclaimedthatthreeclinicaltrialssponsoredbythecompanyhaddemonstrateda90%successrate,buttheresultswouldnotbesubmittedtoscientificjournalsbecauseRexalldidnotwanttorevealtheamountsofeachingredientinitsformula[6].Thisstatementwaspreposterousbecauseresultscouldpublishedwithoutrevealingtheexactamountsofeachingredient.Twoweekslater,IsearchedMedlinefor"cellulite"and"Cellasene"andfoundnoreportthatanyproducttakenbymouthwasprovenusefulagainstcellulite.

NeartheendofMay,apparentlyinresponsetocriticisminthemedia[7],RexallreleasedvariousdetailsontwoofthestudiesandpostedthemtoitsScienceonCellaseneWebsite.Thefirststudywasperformedon25healthyfemalevolunteerswhosehipandthighandanklecircumferenceweremeasuredbeforeandaftereightweeksofdailyconsumptionoftheproduct.Althoughdifferencesbetweentheinitialandfinalmeasurementswerereported,nocontrolgroupwasused,sothatitwouldnotbepossibletotellwhetheranychangeswererelatedtotakingtheproductsortomeasurementvariations.Inaddition,neitherindividualmeasurementsnorweightswerereported,sothatitisnotpossibletojudgefromthedatawhetherthereductionswererelatedtoweightloss,whethercoincidentalorotherwise.

Thesecondstudycompared25peoplewhotooktheproductwith15peoplewhotookaplaceboforeightweeks.Accordingtothereport,theaverageweightofbothgroupsvariedlittlebutaveragehipandthighcircumferenceandskinthickness(measuredwithanultrasoundtest)decreased.However,theexperimentaldesignwassoseriouslyflawedthatthefindingsshouldnotberegardedasvalid.TheparticipantswerenottoldwhethertheywerereceivingCellaseneortheplacebo,buttheinvestigatorsknewwhowasineachgroupbecauseonlytheCellasenegrouphadblooddrawnfortesting.Thiscouldhaveinfluencedthewaythemeasurementswereperformed,aswellastheparticipants'motivation.Nodataweregiventodemonstratewhetherthemeasurementprocesswasaccurateorwhethertheappearanceorfeelofthewomen'sskinhadchanged[7].Inaddition,althoughmeasurementsweremadeattheexperiment'sbeginning,midpoint,andend,themidpointmeasurementswerenotreportedonRexall'sWebsite.

Itseemstomethatavalidtestshouldinvolve:(a)moreparticipants,(b)alongerinitialinvestigativeperiodplusmonthlyfollowupmeasurementsforatleastayear,(c)standardizationofthemeasurementtechnique,(d)measurementstakenbyatleastthreeinvestigators,(e)blindingoftheinvestigatorsaboutwhoreceivedtheCellaseneandwhodidnot,(f)measuringseveraltimesaweektoseewhethermeasurementstendtochangeorremainconstant,(g)weeklyratingsoftheappearanceoftheskinbyboththeparticipantsandtheexperimenters,and(h)releaseoftheindividualdatainadditiontothegroupaverages.IhavesuggestedthesepointstoRexall'schiefexecutiveofficer[8].

AspokespersonforCellasene'sItalianmanufacturerstatedthatastudyinvolving200womenwouldbedoneattheUniversityofMiamiwithresultsexpectedintheFallof1999[9].InJune2000,however,theleadresearcherstudystatedthatthe$400,000studycouldnotbecompletedbecausesomeoftheparticipantshadnotcometothetestingsitetobemeasured[9].IntheinterimaBritishresearcherreportedfindingnodifferenceinhipandthighmeasurementsbetween11womentakingCellaseneand8womenusingaplacebo[10].

PiggybackAttempt?

InHealthAmerica,ofCarlsbad,California,competedwithRexallbymarketingasimilarproductcalledCelluLean.Thecompany'sWebsite,whichwasregisteredduringtheweekRexallintroducedCellasene,stated:

CelluLeanmeltsawaycellulitebyincreasingyourmetabolismandbloodcirculation,breakingdownthefatsfoundincellulite,andremovingtoxinsfromyourbody.TheherbalextractsinCelluLeanhavebeenproventobeeffectiveinreducingfattydeposits.Simplyput,CelluLeanhelpsyourbodytoreachandbreakdowncellulite,enablingyourskintoregainitsyouthfulsmoothappearance.

OnewaytoattractbrowserstoaWebsiteistoplace"metatags"intothesite'ssourcecode.Thewordsinthemetatagsarenotvisiblewhenlookingatthepage,butsearchenginessearchenginesusethemashintsabouttherelevanceofasite.Forexample,ifausersearchesfor"cancer,"andawebsiteuses"cancer"asametatag,thesearchenginemayconsiderthesitetoberelevanttothesearch,regardlessofthesite'sactualcontent.InHealthAmerica's"keywords"metatagread:

twomotorizedrollerswithasuctiondevicethatcompressestheaffectedtissuebetweenthetworollers.Themanufacturerispermittedtopromoteitfor"temporarilyimprovingtheappearanceofcellulite."TheprocedurecalledEndermologieusuallytakes10to20treatmentstogetthebestresults,andoneortwomaintenancetreatmentspermontharerequiredtomaintainthem.Withoutthemaintenance,thebenefitswillsoonbelost.Thetypicalcostis$45to$65persession.Arecentlypublishedstudyof85womenbetweentheagesof21to61foundthat46patientswhocompletedsevensessionsshowedameanindexreductioninbodycircumferenceof1.34cm,while39patientswhocompleted14sessionsoftreatmentsshowedameanindexreductioninbodycircumferenceof1.83cm[11].However,anotherstudy,involving52women,foundnoobjectivedifferenceinthighgirth(attwopoints)orthighfatdepth(measuredbyultrasound).[12]

EnforcementActions

TheFTChastakensuccessfulactionagainstmanymarketersofallegedcellulitereducingproducts:

In1991,SlenderYou,Inc.,signedaconsentagreementprohibitingunsubstantiatedclaimsthatitscontinuouspassivemotion(CPM)tablesenabletheusertoloseweightloseinchesremovecelluliteflushouttoxinstoneandfirmmusclesorachievephysicalfitnessbenefitscomparableorsuperiortothoseprovidedbyrigorousexercise[13].In1993,asimilaractionwastakenagainstFleetwoodManufacturing,Inc.,ofMesa,Arizona,anditsowner,ThomasA.Fleetwood.[14]In1993,Nature'sCleanser,basedinBeverlyHills,California,andDonaldDouglasTorryagreednottomakeunsubstantiatedclaimsthatitsherbaltabletswouldpromoteweightlossby"cleansing"thebowel.Thecompanyhadclaimedthatweightcouldbeimmediatelycontrolledbywithoutdietingorwatchingcaloriesbyeliminatingwastesuchasfattytissues,cellulite,toxins,mucus,hardenedfecalmatterandharmfuldrugresidues[15].In1993,Revlon,Inc.anditssubsidiary,CharlesRevson,Inc.,signedaconsentagreementnottomakeunsubstantiatedclaimsthattheirUltimaIIProCollagenanticellulitebodycomplexwould:(a)significantlyreducecellulite,helpdispersetoxinsandexcesswaterfromareaswherecelluliteappears(b)reduceskin'sbumpytexture,ripplesorslacknesscausedbycellulite(c)helpdispersetoxinsandexcesswaterfromareaswherecelluliteappearsand(d)increasesubskintissuestrengthandtone[16].In1993,SynchronalCorpagreedtostopunsubstantiatedclaimsforitsAnuskaBioResponseBodyContouringProgramcellulitecreamandtopay$3.5millioninconsumerredress.In1993,NationalMediaCorp./MediaArtsInternational,Ltd.,agreedtoaconsentagreementand$275,000inconsumerredressinconnectionwithinfomercialsclaimingthatitsCosmetiqueFrancaiswouldsubstantiallyreduceoreliminatecellulite,wasmoreeffectivethandietorexercise,andwouldpreventrecurrenceifusedonceortwiceaweek[17].In1995,afederaljudgeinCaliforniapermanentlybannedSiluetaDistributors,Inc.,aChatsworth,Californiabasedcompany,anditspresident,StanleyKlavir,fromdeceptivelyclaimingthattheir"SistemaSilueta"creamandtabletswouldreducecellulite.ThejudgeconcludedsaidthatSistemaSiluetawasnothingmorethanmoisturizeranddiuretictablets,neitherofwhichwouldcausecelluliteloss.Thedefendantswerealsoorderedtopay$169,339inconsumerredress[18].In1995,EuropeanBodyConceptsanditsanditspresidentJamesMarinosignedaconsentagreementprohibitingitfrommakingunsubstantiatedclaimsthatitsbodywrappingsystemwouldcausetheusertoloseinches,pounds,andcellulitequicklyandeasily,withoutdietingorexercising,andthatthesystemcouldreducethesizeofspecificareasofthebody[19].In1995,NationalDietaryResearch,Inc.(NDR)anditsownerWilliamH.Morrisagreedtopay$100,000tosettleFTCchargesofdeceptivelyadvertisingFoodSourceOne(FS1)asaweightlossandcholesterolreducingproduct.Theproductwasacompressedtabletmadelargelyfrom

plantfiber.Theconsentagreementincludedaclausethatprohibitedunsubstantiatedclaimsofcellulitereduction[20].In1998,anAustralianfederaljudgeorderedtheSwissSlimmingandHealthInstituteanditsdirectortopaytheCommission$1.47millioninpenaltiesandinterest,someofwhichwouldberefundedtodefraudedclients[21].In1999,theIowaAttorneyGeneralobtainedanorderprohibitingLipoSlim,Inc.,ofNewYorkCityfromcontinuingtomarketingits"LipoSlimBriefs"inIowa.Adssaidwearingthebriefswould"getridofyourcellulite"and"dissolvefatandhydricdepositsthataccumulateinyourhips,stomach,buttocksandthighs."AnadintheNationalEnquirersaidthat"thousandsofthermoactivemicroporecells"inthebriefs"produceagentlemassagethatdestroysdeepfatparticlesandliquidmoleculeswhicharethecauseofexcessfat."TheorderrequiredthecompanytoofferfullrefundstoitsIowabuyersandpay$12,000tothestateconsumereducationfund[22].In1999,theFDAorderedCelluliteReductionofNewYorktostopsuggestingthatEndodermologycouldhavemorethanatemporaryeffectoncellulite[23].InJuly2000,theFTCchargedRexallSundown,Inc.,withmakingfalseandunsubstantiatedclaimsforCellasene[24].

TheBottomLine

Theamountoffatinthebodyisdeterminedbytheindividual'seatingandexercisehabits,butthedistributionoffatinthebodyisdeterminedbyheredity.Inmostcasesreductionofaparticularpartcanbeaccomplishedonlyaspartofanoverallweightreductionprogram.Endermologiemaytemporarilyimprovetheappearanceofdimpledareas,buttheprocedureistimeconsumingandexpensive.Liposuctionmaypermanentlyhelpinsomecases.
