Include for credit classes only in CMDF
Exclude from your CMDF
• non-credit classes
• dual-enrollment courses offered entirely to high school students
• distance learning classes including practicums, clinicals, internships and co-ops
• lower-level ESL classes
• lab sections associated with a lecture
• individual instruction
• individual study or self-paced classes from the data file

When to send: Submit CMDF as an Excel file to your liaison after your census date

CCSSE sends college random sample (Feb 11-Mar 14)
College approves sample, CCSSE begins building class packets

• Class Information Sheet (Appendix L, Page 26 in P.G.)
• Survey Administration Script (Appendix A, Page 14 in P.G.)
• CCSSE Program Code Sheet (Appendix M, Page 28 in P.G.)
• CCSSE Additional Questions Sheet or combination CCSSE Additional Questions and Individual College/Consortium Questions Sheet
• Surveys
You will also receive pencils, 100 extra surveys, UPS return shipping labels, and neon labels

Survey Administration Process
Survey takes between 35-45 minutes to complete
Survey must be completed in #2 pencil
Deadline for receiving surveys: MAY 21

Return completed surveys and corresponding Community College Student Report Information Sheet to CCSSE in envelopes in which they arrived to you.
You do not need to return unused surveys, Program Code Sheets, pencils, or Supplemental Questions Sheets to CCSSE.
