Clostridium difficile has a well documented role in pseudomembraneous colitis and diarrhoea associated with antibiotics. Evidence has also implicated C. difficile in colitis not induced by antibiotics, exacerbations of chronic inflammatory bowel disease and postoperative diarrhoea. Many laboratories rely on the detection of C. difficile cytotoxin to diagnose infection. However, investigations have shown that the detection of faecal cytotoxin is not always consistent with active infection. This, plus the fact that some laboratories do not have the specialised facilities that are required to detect faecal cytotoxin, has highlighted the need for a test that is easy to use and gives rapid results for the identification of C. difficile.

Use of Microgen® C. difficile allows identification of C. difficile from selective agar, within two minutes.

Traditional methods for identification of C. difficile includes GLC and cytotoxin detection. However, GLC is expensive in terms of time, equipment and cost per test and the link between presence of cytotoxin and symptoms is inconsistent. The test latex particles in Microgen® C. difficile are coated with antibodies specific for C. difficile somatic antigens, resulting in a highly specific test with a high predictive negative value. The flexibility of the test means that it can be used to test isolates directly form selective agar plates, such as CCFA.

The kit, which is simple-to-use and gives clear cut results, contains a positive control, has a long shelf-life, and offers the confidence of high sensitivity and specificity.

Campylobacter gastro-enteritis is most prevalent in young children, but outbreaks are also common in adults. In temperate climates the incidence of Campylobacter gastro-enteritis peaks in summer and early autumn, whereas in tropical climates peak incidences may occur in the cooler months. Campylobacter infection may be effectively treated with antibiotics but in the majority of cases, due to the length of time required by traditional detection methods, the acute phase has passed by the time a laboratory diagnosis is communicated to the physician. Microgen® Campylobacter is a rapid, easy-to-use test which can be reliably performed on selective agar colonies. Microgen® Campylobacter can therefore reduce the time taken to produce a laboratory diagnosis and improve patient prognosis.

Much of the rapidly expanding interest in E. coli 0157 results from the increasing capability to isolate and identify this organism. E. coli 0157 is a verotoxin (VT) producing strain responsible for Haemorrhagic colitis (HC), Haemolytic Uraemic Syndrome (HUS) and community acquired diarrhoea. Until recently it was thought that E. coli 0157 was only isolated from samples of bloody diarrhoea as this is classically seen with verotoxin producing strains. However, it is now increasingly being isolated from non-bloody specimens. Sorbitol MacConkey medium is the recommended screening agar for E. coli 0157, but, although this technique shows 100% sensitivity it is only 85% specific. E. coli 0157 does not ferment sorbitol. However, other sorbitol negative Gram negative rods also grow on sorbitol MacConkey agar. As a result of this, a specific rapid identification method is required. Microgen® E. coli 0157 is an easy-to-use one step, latex identification kit which gives results in under two minutes and specifically identifies E. coli 0157 directly from the selective plate, even in mixed culture.

The conventional method for identification of E. coli 0157:H7 utilises specific antisera in a standard agglutination test. However, this method and other commercially available latex tests have been shown to suffer from cross reactions with other enteric organisms such as E. hermanii, which antigenically and biochemically resembles E. coli 0157, resulting in false positive results. Microgen® E. coli 0157 contains a test latex which has been sensitised with E. coli 0157 specific antibodies, that are prepared by absorption with potentially cross reacting strains. Microgen® E. coli 0157 has demonstrated no cross reactions when tested against a panel of 161 E. coli 0157 ‘O’ serotypes and 56 ‘H’ serotypes, and shows strong specific agglutination. Microgen® E. coli 0157 gives strong and specific results within two minutes and comes complete with a control latex which can be easily compared with test results to confirm non-specific reactions.

L. pneumophila is the most common cause of Legionnaires Disease. At present 16 serogroups exist, of which L.. pneumophila serogroup 1 accounts for the majority of cases. In addition, a range of other species of Legionella have also been implicated as the causative agent of Legionnaires disease. Clinical and environmental awareness, and testing for Legionella, is rapidly increasing assisted by the introduction of several commercially available selective culture media. However, none of the media are specific for L.. pneumophila serogroup 1 and not everything that grows on them is Legionella. Therefore a specific test is needed to quickly confirm the identification of isolates so that appropriate action can be taken. Microgen® Legionella is an easy-to-use latex agglutination test for L. pneumophila serogroups 1 and 2-15 and now a range of non-L. pneumophila species which have been implicated as causes of diseases in humans. The kit provides specific results in under two minutes directly from a selective medium.

Conventional methods for identification of Legionella utilise specific antisera in standard agglutination or immunofluorescence tests. However, these methods lack sensitivity, are time consuming and can require specialised training and expensive equipment. The test latexes used in Microgen® Legionella are coated with immunoglobulins which have been specially processed to minimise cross reactions with potentially cross reacting organisms, eliminating the need for a control latex, simplifying the procedure and increasing efficiency. Microgen® Legionella has recently been improved by the addition of a new latex reagent which identifies a range of non-L. pneumophila species of clinical significance with a high specificity and no cross-reactions with L. pneumophila. Microgen® Legionella contains test latexes for both L. pneumophila serogroup 1 and 2-15, and now a latex reagent for the following additional species; L. micdadei, L. bozemanii 1 & II, L. dumoffii, L. longbeachae 1 & II, L. jordanis, L. gormanii, L. anisa and L. feelei in a convenient 50 test pack with a long shelf life to offer the most rapid, specific and sensitive test for Legionella currently available.

Listeria spp. are commonly found in soil and are transferred onto vegetable matter. Foods either derived from vegetables or which are derived from grazing animals and which are exposed to minimal processing i.e. dairy products, are the most common vector for the transmission of Listeria spp. to humans.

A total of 6 species have been recognised to date including Listeria monocytogenes which has been associated with human disease. L. monocytogenes is an opportunistic pathogen in that it will only cause disease in people with certain predisposing factors such as the immunosuppressed and pregnant women.

Once colonies have been isolated on agar plates, the next stage requires that any colonies resembling Listeria spp. are tested to confirm that they belong to that genus. Other organisms may be isolated from food samples which may resemble Listeria spp. To confirm the presence of Listeria spp. a number of tests are normally used including aesculin hydrolysis, oxidase, catalase, motility (differential at 25°C and 37°C).

Microgen® Listeria latex was developed as an alternative to this battery of tests. Although individually these tests are inexpensive, they are time consuming to perform. The use of a simple latex agglutination test provides a real-time 2 minute test for the confirmation to genus level of Listeria spp.

Conventional culture methods for Listeria spp. rely on a combination of enrichment and selective media to achieve satisfactory growth of the organism These conventional methods are time consuming and result in unnecessary delays in releasing product from quarantine. Microgen® Listeria can be used to obtain rapid, presumptive-positive results from selective media thus giving an early indication of a failure in the production process. Where standard methods require it, Microgen® Listeria can be used to screen colonies before full biochemical identification and can replace traditional agglutinating antibody methods.

Microgen Salmonella is a latex slide agglutination test for the confirmatory identification of presumptive Salmonella colonies from selective agar plates. The genus Salmonella consists of over 2,000 serotypes which can cause acute infectious gastroenteritis frequently associated with food poisoning. Since many Salmonella spp. infect domestic animals, either clinically or sub-clinically, cases of Salmonella food poisoning usually originate from animal sources. The rapid identification of Salmonella is not only vital for appropriate patient management but also for food manufacturers where prompt results minimise the delay associated with routine testing for Salmonella contamination. The detection and identification of Salmonella spp., using traditional techniques, usually requires identification by serological and biochemical techniques. Microgen® Salmonella offers a rapid alternative to the traditional identification methods and detects Salmonella spp. including S. typhimurium and S. enteritidis.

Staphylococcus spp. occur as commensal organisms on the mucous membranes and skin of humans. However, Staphylococcus aureus can be responsible for a wide range of pyogenic infections including superficial suppurant lesions, food poisoning and toxic shock, as well as a variety of other conditions. It is therefore one of the most important pathogens regularly encountered in the laboratory. Due to the frequency and importance of Staphylococcal infection a rapid and accurate diagnosis is needed for correct patient management, especially as the bacterium can show resistance to numerous common antibiotics. Food manufacturers find Staphylococcal detection to be a good indicator of the general level of microbial contamination. In these circumstances a highly specific test is needed to identify S. aureus, often from heavily contaminated samples. Traditional identification methods, and a number of commercially available products, may suffer from false positive results due to non-specific reactions, and false negative results due to an inability to detect certain strains of S. aureus. Microgen® Staph is a sensitive and specific test for the identification of S. aureus from primary plate culture. Also available separately is the Staph latex control reagent (product code M43CCE), helping to reinforce the accuracy and reliability of the Microgen Staph rapid test. Two of the traditional diagnostic tests for S. aureus, the tube and slide coagulase tests, only detect the activity of a characteristic coagulase enzyme. However, these tests can be difficult to interpret and are liable to suffer from false positive results associated with non-specific factors frequently found in plasma. Non-specific coagulation due to other bacterial enzymes may also occur. However, 99% of S. aureus isolates also produce detectable levels of protein A on their surface, a characteristic which can be used to differentiate S. aureus from other species of Staphylococci in which this protein is rare. Microgen® Staph is a test for the identification of S. aureus which overcomes the problems associated with the traditional tests by detecting both coagulase and protein A offering rapid and accurate identification of S. aureus in 2 minutes. Microgen® Staph contains all ancillary reagents needed to complete the test and is suitable for clinical, food and environmental laboratories.

Most strains of Streptococci which have been isolated from human infections possess serological group specific antigens. Identification of the organisms includes extraction and characterisation of these antigens from organism grown in culture. The classical method of grouping Streptococci has been the Lancefield method; however, it is very time consuming and requires considerable technical expertise. This has restricted its use to only a very few reference laboratories. The use of Latex Technology coupled with high quality, specific antisera and a specific enzyme extraction protocol has brought Streptococcal grouping within the reach of most laboratories. Microgen® Strep utilises latex particles that have been sensitised with high quality group specific antisera. Cell wall antigens are extracted by rapid enzymic digestion and then tested by a panel of latex reagents. A positive reaction is denoted by a strong agglutination reaction. The negatives remain smooth thus ensuring confidence in the final result.

The Microgen® GNA-ID System is for the identification of the commonly encountered Enterobacterceae from food or medical samples including urinary samples and common wound isolates. The readability of a negative haemolysis reaction has been improved; if an organism does not produce haemolysin, the red blood cells will remain intact and form a distinct red pellet. If an organism does produce haemolysin, the red cells will rapidly be lysed and cellular contents released as a red/brown solution, indicating a positive result.

MicrogenTM Staph-ID has been developed for the identification of commonly encountered Staphylococcus spp. isolated from clinical and veterinary samples. These bacteria, although commensal, are recognised as potential human pathogens e.g. S. aureus and S. epidermidis. Coagulase negative Staphylococcus spp. are common colonisers of prosthetic valves and in-dwelling catheters.

Gram stain (positive), catalase (positive) and latex agglutination / coagulase tests are performed as pre-tests on the isolate. Colony pigmentation is recorded and then 1-2 colonies are inoculated into the suspending medium. The suspension is inoculated into the 12 well test strip and incubated at 37°C. After 24 hours incubation the strips are read and the Nitrate and PYR reagents added. The re-sulting 5 digit numerical code is entered in to the software and an identification re-turned.

Microgen® Strep-ID is a biochemical test system which utilises a 12 well (12 test) microwell test strip and 3 off-strip tests; hippurate hydrolysis (provided), alpha-haemolysis and beta-haemolysis for the identification of Streptococcal and Enterococcal species important to clinical, veterinary, food and environmental laboratories. Performance Characteristics: Microgen® Strep-ID has been evaluated in comparision with a well-established commercially available product for identification of cultured bacterial isolates. A total of 69 cultures, comprising cultures from recognised culture collections and clinical isolates were examined.

The Microgen® Strep-ID correctly identified 70 (93%) and the competitor product correctly identified 52 (74%) of the isolates tested. The superior specification of Microgen® Strep-ID is complimented by an excellent differentiation of Entercocci spp. and a reduced test system minimises handling and manipulation whilst reducing procedural error and time.

As food manufacturers and processors realise the importance of microbiological environmental hygiene monitoring as part of a good manufacturing practices in the food industry, the need for a cost-effective, easy-to-use, rapid and accurate method for environmental sampling has become increasingly more important. Microgen Bioproducts Ltd has met this need by developing a new range of environmental pathogen detection systems: Path-Chek Hygiene Pathogen Systems. These systems meet all of the requirements of ISO Standards for environmental sampling (ISO 18593:2004(E)) through the use of pre-moistened swabs for the sampling of surfaces, combined with organism specific chromogenic detection broth into which the swab is placed and incubated at the appropriate temperature.

The range of Path-Chek Hygiene Pathogen Systems consist of tests for 3 organism types; Listeria, Coliforms and Salmonella.

All three systems utilise an identical test principle. Bacteria are recovered from the surface of food manufacturing or processing equipment using a pre-moistened swab, which is capable of neutralising commonly employed detergents and sanitisers. The sample swab is then transferred into an organism specific media (swabs have a conveniently located break point so that they can easily be broken off into the growth media). The test media are then incubated at 35-37°C for 18-24 hours for Coliforms and Salmonella, and at 28-30°C for 24 -48 hours for Listeria (If Path-Chek Hygiene Listeria is incubated at 35-37°C, there will, in certain circumstances, be an increased risk of false positives). If the specific target bacteria are present on the sample swabs, bacterial growth will cause the colour of the media to change.

The increased awareness of the need for high levels of hygiene in food handling and processing environments combined with the widespread implementation of HACCP Programs has resulted in the need for simple and effective methods for the rapid assessment of hygiene and the effectiveness of cleaning programs. PathchekTM Hygiene is a chemical test based on the detection of protein, a common component of most food types. This test is not targeted specifically at bacteria. When cleaning procedures are not effective or have been performed poorly, both food residues and bacteria will be present and will result in a positive reaction. PathchekTM Hygiene is provided in a simple peel open plastic pouch which contains a pre-moistened swab for sampling test surfaces. After sampling, the swab is returned to the pouch and pushed down into the developing reagent. If positive (indicating the presence of food residues and bacteria), a colour reaction (green to blue) will develop within 5 seconds.

Helicoscreen is a sensitive and specific rapid cassette test which detects H. pylori antigen in faecal samples within minutes. Helicoscreen complements the range of rapid cassettes for the detection of infectious agents responsible for gastro-enteric diseases from Microgen Bioproducts. The company’s range includes rapid tests for the leading causes of gastro-enteritis; Rotavirus, Adenovirus and Norovirus, allowing for a laboratory to test in-patients exhibiting symptoms of diarrhoea and vomiting rapidly with sensitive and specific tests, allowing for the correct treatment to be implemented as well as patient isolation policy. Helicoscreen is an easy-to-use cassette which includes a sample preparation device:

1. Remove sample probe from collection device and coat liberally with sample. 2. Replace probe and shake to disperse material. 3. Snap top from collection device and invert vertically. 4. Add 3 drops (120 µL) to the sample well of the cassette. 5. Leave at room temperature and read the results after 10 minutes. Strong

positive results will be observed sooner. Performance Data:
