The training at the University of the Orange FreeState was a combined program of cardiac and thoracic surgery, supervised by Professor Johannes Meyer. In 1981, I was appointed specialist in cardiac surgery at Groote Schuur Hospitaland Red Cross War Memorial Children's Hospital in Cape Town, South Africa. This appointment was held until January 1987. During this period, I was actively involved in adult and pediatric cardiac surgery, with main emphasis in the fieldof heart transplantation. Part of my duties included the training of residents,participation in ward rounds, tutorials, and supervising a group of researchersstudying of the effects of brain-death on the heart and its effects on the metabolism of the body as a whole.

From February 4, 1987, until October 1990, I was a member of the heart transplantation team at the Oklahoma Transplantation Institute, Baptist Medical Center, Oklahoma City, Oklahoma.

Since November 1990, I have been working as an Associate Professor at the University of South Florida in the Department of Cardiothoracic Surgery in Tampa, Florida.

As of July 1998, I have been promoted and received tenure as a full Professor atthe University of South Florida in the Department of Cardiothoracic Surgery inTampa, Florida.

SCHOLARSHIPS

During my medical training, I was initially a junior tutor of anatomy, and aftergraduation during the rotating internship, I became a lecturer of anatomy withthe Anatomy Department, La Plata Medical School, Buenos Aires, Argentina, 1965-1970.

CLINICAL WORK

During the years of training in general surgery, I was exposed to all surgical specialties on a rotation basis during the first two years.

As Chief Resident, I was in charge of the Department of General Surgery, being responsible for all the emergency operations and permanently on call for trauma calls. During this year, I performed all -types of major general surgical proced

ures.

As Resident in Cardiothoracic Surgery, I was guided by Professor C.N. Barnard inthe field of cardiovascular surgery. My primary responsibilities were initially in the care of patients and, during the last two years, performing all types of adult cardiac operations and pediatric cardiac surgery.

From the beginning, my duty was especially dedicated to the care of donors for heart transplantation. After one year, I took over the post-operative managementof patients who had undergone a heart transplant.

I performed my first heterotopic heart transplant as a fellow at Groote Schuur Hospital.

In September 1982, I was appointed by Professor C.N. Barnard as a specialist incharge of the Heart Transplant Unit and full-time Fellow at the Cardiac ResearchLaboratory for one year. During that period, I developed a non-invasive technique for the diagnosis of acute cardiac rejection using radionuclide-scanning methods, first in animals and then afterwards applied the technique to human cardiac transplantation, which was widely used at the University of Cape Town.

From January 1984 to the present, I have continued my research activities. At the Heart Transplant Unit in Cape Town, I worked approximately 50% in the laboratory where I embarked on the experimental study of brain-death in baboons and pig

s looking at the effects on the myocardium and the hemodynamic and endocrine deterioration following brain-death. The metabolic studies in brain-dead animals and patients confirmed that aerobic metabolism is inhibited. Explaining one of the mechanisms participating in the graft dysfunction after organ transplantation.

Based on this work, I developed the "hormonal therapy for brain-dead organ donors", showing its beneficial effect.

As a result of this research, I found that by administering Triiodothyronine (T3

), the metabolic dysfunction is fully corrected prior to organ transplantation.Currently, this therapeutic modality is used widely in the rescue of unstable brain-dead organ donors.

Together with Dr. W.N. Wicomb, I was involved in myocardial preservation in a portable unit, achieving successful preservation of the myocardium over a period of 48 hours, followed by orthotopic transplantation of the hearts of Chacma baboons with excellent survival of the animals.

In immunosuppressed Chacma baboons, I performed using cardiopulmonary bypass, anen bloc heart and lung transplantation finding that the lungs are actively rejected while minimal histological changes occur in the transplanted hearts. Thisis in contraindication to previous reports in which it was stated that the heartand lungs are rejected simultaneously. This work was published in the Annals of Thoracic Surgery.

In September 1982, I was in charge of the Department of Heart Transplantation, working in close collaboration with Professor Christiaan N. Barnard, and later with B. Reichart and D.K.C. Cooper, performing personally a total of 55 heart transplants, of which 8 were retransplantations (1 for irreversible acute rejectionand 7 were for chronic rejection). In 2 patients, the recipient heart was resected (previousheteroptoic heart transplants), and a heart was implanted in orthotopic position, leaving these patients with two transplanted hearts. During this period, 12 h

eart and lung transplants were performed in Groote Schuur Hospital, in which I participated actively in the surgical procedures, resecting the donor heart and lung block and performing a heart and lung transplant in a patient with primary pulmonary hypertension.

In 1987, as a member of the Oklahoma Transplantation Institute, I became co-director of research and director or organ procurement. While in this position, I have been involved in 77 procedures, including heart and heart-lung transplants.

From 1990 to December 1995, I held the title of Acting Chief of Cardiothoracic Surgery at the James A. Haley Veterans' Hospital. On January 1, 1996, I became t

he formal Chief of Cardiothoracic Surgery performing the majority of open heartsurgeries. I am also involved in Thoracic Surgery coverage whenever required and provide surgical stand by for all PTCA elective or emergent.

At Tampa General Hospital, Tampa, Florida, I am a member of the Heart TransplantTeam. I perform open heart surgery and provide coverage for the Thoracic Trauma Service.

From July to present, I have performed over 1,000 total myocardial revascularizations not using the heart-lung machine. This is a new re-introduction of old technology. However, with the advent of beta-blockers and special retractors, I am able to do a coronary bypass on any epicardial artery. This technique has been performed only in one other institution in the United States.

PREVIOUS RESEARCH

The potential applications of Triiodothyronine therapy following acute myocardial ischemic events were further examined in conditions exhibiting the euthyroid sick syndrome.

Experimental work was performed at the Veterinary School at the Oklahoma State University in Stillwater and the Oklahoma Medical Research Foundation. The mainemphasis of this research was to examine the effects of Triiodothyronine in thereversal of myocardial dysfunction in the following conditions: 1) stunned Myocardium, 2) recovery of the cardiac function following catecholamine infusion, and 3) myocardial depression following cardiopulmonary bypass.

Currently, we are looking at the effect of T3 pretreatment in rats with heart fa

ilure, observing a significant cardiac recovery and response to beta agonists.Control rats, after six weeks of heart failure, no longer respond to beta agonists. This may become of relevance mainly in open heart surgery or following PTCA's.

I have been acting as a Consult to SmithKline Beecham Pharmaceuticals for the development of protocols related to T3 in experimental and clinical applications in non-thyroid diseases.

The FDA has issued an IND number for the use of T3 in non-thyroid diseases. A randomized blinded study was done in Oklahoma in patients undergoing coronary artery grafts on cardiopulmonary bypass.

Recently, a fully funded study has been completed (Protocol TS-107). In this study using transesophageal echocardiography, we examined the effects of T3 in patients undergoing open heart surgery in a randomized, prospective blinded study.This was done in collaboration with Dr. Hector Fontanet (Cardiology Department).

We administered open T3 to high-risk heart patients with an estimated mortalityof 15-50%, (using a mathematical model developed by Dr. Parsonnet from Beth Israel Hospital, New Jersey), observing a significant death rate reduction by 60%.

At the James A. Haley Veterans' Hospital, I was the Primary Investigator for a VA Cooperative Study entitled, "Urgent Revascularization in Unstable Angina".In addition, I was Co-Investigator in the following research protocols:

I was a Primary Investigator at the Baptist Medical Center for the use of the "Thoratec" ventricular assist device for the rescue of the myocardium following open heart surgery and as a bridge to transplantation. I implanted the device in apatient who survived 70 days with its support (one of the longest ventricular assist device supported patients in the USA), and was thereafter successfully transplanted.

CURRENT CLINICAL RESEARCH

I am Co-Chairman with Dr. Fred Grover on a VA grant (10.6 million dollars) for acooperative study aOutcomes Following Myocardial Revascularization: On and OffCardiopulmonary Bypassa, which was performed in 18 Veteran Medical Centers.

We have completed the randomization and one year follow up for the Cooperative study CSP 517. Preparation of the main manuscript on the study findings has beeninitiated.

I am the senior author of the primary manuscript of the study CSP 517 entitled:

Outcomes Following Non-Emergent Coronary Artery Bypass Graft Procedures:Revascularization Off versus On Bypass (ROOBY) Trial Findings. NEJM

Multiple manuscripts will follow the primary paper, I estimate that at least twenty relevant papers will be published in prestigious journals and multiple meeting presentations will follow.

Also, I am reviewer for selected articles for the following journals: 1) Annalsof Thoracic Surgery, 2) Transplantation, and 3) The Journal of heart and Lung Transplantation.
