“FIXED APPLIANCE MANAGEMENT OF CLASS II MALOCCLUSION”

During this session and panel discussion Dr. Miller will outline the differences andadvantages of the Esprit Spring recently developed by Opal Orthodontics. The advantagesof a hidden spring and distal plunger as well as a comparison of force levels, and its use indirect archwire, XBow and class III correction will be discussed. Overall, the focus will be onimproving patient comfort during the class II correction process.

Dr. Robert Miller, BS, DMD†

Dr. Miller earned his B.S. degree in Zoology from Clemson University in1979 and his DMD degree in 1983 from The Medical University of SouthCarolina. After dental school, Dr. Miller completed an orthodonticresidency at the Medical College of Virginia in 1985, and then served in theUSAF, as Chief of Orthodontics at Clark Air Base in the Philippines for threeyears.

Dr. Miller began practicing in Culpeper in 1988 and became a Diplomate of the American Boardof Orthodontics in 1992. He is a pioneer in the area of orthopedic correctors, having developedthe Flip Lock™ Herbst device in the early 1990's. More recently, he assisted Dr. Duncan Higgins,the inventor of the Crossbow™ device, to broaden its use as an efficient overbite corrector andour most state of the art device used just before braces. Dr. Miller is the author of numerousprofessional publications in orthodontics.

10:30 am – 11:15 am THE CROSSBOW® CLASS II CORRECTOR “WHY BOTHER WITH AN EXTRA PHASE ONE APPLIANCE?”

The focus will be on:1. Combining Class II correction with maxillary expansion.2. Distalizing the upper bicuspids and molars only, allowing over-correction of the Class II

buccal segments without over-retracting the upper incisors. Passive upper incisorretraction minimizes root resorption.

3. Allowing the side effects of the Class II springs to rebound before phase two braces.This avoids dealing with the side effects late in treatment with a single phase approach.It also decreases the time in braces and minimizes decalcification.

Dr. Duncan Ward Higgins, DDS, MSD, FRCD(C)†

Duncan has two Canadian patents related to the Crossbow® Class IICorrector, or Xbow®. There are three research articles on Xbow in the Angle Orthodontist, one in the AJODO, and one in the European Journal of Orthodontics. His indirect bonding technique was in Seminars inOrthodontics.

Duncan has lectured internationally on indirect bonding and Xbow to groups including theCAO, Angle Society, PCSO, GLAO, and MASO. He is a regular lecturer at UBC. Duncan is a Fellowof the Royal College of Dentists in Canada.

Class II correction devices are commonly used in orthodontics and exist in manydeclension. Literature reviews show that such devices do not appear to cause anysignificant changes in mandibular length and their effectiveness in correcting class IImalocclusion can be explained by a combination of some skeletal (mainly maxillary) anddentoalveolar (maxillary and mandibular) modifications. The SUS2 corrector device will bepresented using bondable head gear tube and self-ligating mandibular molar tube. A casepresentation will be used to explain how to use the SUS2 device in a successful manner.

Dr. Sylvain Chamberland, DMD, FRCD(c), MSc.

Dr. Chamberland received his DMD degree from Laval University in 1983, a Certificate in Orthodontics from University of Montreal in 1990 and aMaster degree in dental science in 2008 from University Laval where he waspart time clinical instructor and teacher at the undergraduate program ofthe dental faculty from 1991 to 2008. He lectures in several graduateprograms and scientific meetings in the US, Canada and Europe. He is a

published author. Dr. Chamberland is a Diplomate of the American Board of Orthodontics anda Member of d’Angle East Society of Orthodontists.

particularly as they enter and exit the profession. He began his financial career in 1987 managing thefinancial affairs of high net worth individuals for Canada’s largest trust company.

David is a regular speaker at Dental functions and trade shows and is an author in The ProfessionalAdvisory which is delivered with Oral Health.

In 2012 David was awarded the prestigious Arbor Award from the University of Toronto for his longstanding volunteer service and support of the Faculty of Dentistry and his leadership with the AnnualGala fundraising dinner.

2:00 pm – 3:15 pm SURGICAL CONSIDERATIONS IN TREATMENT PLANNING ADOLESCENT CLASS II CORRECTION

This presentation will review how orthodontists and surgeons can work together usingappropriate diagnostic and surgical procedures to achieve a successful outcome inadolescent Class II patients. The orthodontist will learn how to identify craniofacialabnormalities that contribute to narrowing of the pharyngeal airway and predispose toadult Obstructive Sleep Apnea. Long term esthetic and functional benefits of correctioncertain types of skeletal outcomes of surgical correction of skeletal deformity will bediscussed.

Dr. Reginald H. B. Goodday, DDS, MSc, FRCD (C), FACD, FICD

Dr. Goodday obtained his MSc in Oral and Maxillofacial Surgery fromDalhousie University and subsequently did a clinical OMFS fellowship inAustralia. He is a full professor and past Chair of the Department of Oraland Maxillofacial Sciences at Dalhousie, past President of the CanadianAssociation of Oral and Maxillofacial Surgeons, and has served as both the Chief Examiner of OMFS and Examiner in Chief for the Royal College of

Dentists of Canada. Dr. Goodday is on active staff at the QEII Health Science Center and the IWK Children’s Hospital in Halifax, Nova Scotia. His research interests include the diagnosis,treatment planning, and appropriate surgical management of Obstructive Sleep Apnea. He has published 4 chapters, several articles and multiple abstracts on this topic.

“THE FUTURE OF ORTHODONTICS”

10:45 am - 12:00 pm VOLUMETRIC IMAGING AND RADIATION ASSOCIATED WITH CONE BEAM IMAGING FOR ORTHODONTIC DIAGNOSIS AND TREATMENT PLANNING

Volumetric imaging offers unprecedented access to visualize the patient’s “anatomic truth”and fulfills a need in orthodontics to evaluate patients comprehensively. Despite theseadvantages there remains concern regarding radiation exposure to the patient. Thispresentation will discuss the need for improved imaging in orthodontics, comprehensivepatient evaluation and radiation dosimetry. In the latter, comparisons will be madecomparing cone beam imaging with other common dental radiographs. In addition,guidelines for operational settings and visualization approaches will be provided.

Dr. James Mah, DDS, MSc, MRCD(C), DMSc

Dr. James Mah is a Clinical Professor and the Program Director inOrthodontics at the University of Nevada, Las Vegas. His research isfocused on 3-Dimensional imaging and modeling for the purposes ofdiagnosis, treatment planning and therapeutics as well as anthropologyand forensics. He has been involved in the research and development ofCone-Beam CT scanners, 3-D facial imaging devices, intra-oral scanners

and CAD/CAM applications in dentistry. He obtained his Doctorate of Dental Surgery, Master ofScience degrees and his Certificate of Specialization in Orthodontics from the University ofAlberta. Following that, he graduated from Harvard Medical School with a Doctorate ofMedical Science degree and completed a Post-Doctoral Fellowship in the Department ofOrthopaedics at Children’s Hospital, Boston. Dr. Mah has authored numerous publications,textbooks, and book chapters and regularly presents nationally and internationally. In addition,his work has been featured in the Los Angeles Times, The National Post, Tech TV and Men’sHealth. He also reviews for the Journal of Clinical Orthodontics, the American Journal ofOrthodontics and Dentofacial Orthopedics and other dental journals.

2:00 pm – 3:15 pm 3D IMAGE-AIDED TREATMENT OF IMPACTED ANDTRANSPOSED TEETH

Three-dimensional (3-D) imaging rendered by Cone Beam Computed Tomography (CBCT)facilitates refined diagnosis and enables fine-tuning of treatment plans in specific types of cases. In patients with impacted and transposed teeth, the information derived isparticularly important in enhancing efficiency of treatment, rates of success andminimizing collateral damage during treatment. In his talk, Dr. Kapila will present theindications and uses of CBCT imaging in diagnosis and treatment planning for impactedand transposed teeth, including protocols for communicating with the surgeon to achieveoptimal surgical access, bond placement and soft tissue support, and the utilization of thisinformation to implement efficient three-dimensional mechanics. He will also discuss theuse of refined spring designs to tract the teeth along the shortest path through bone whileminimizing damage to roots of neighboring teeth.

The attendees will learn about:1. The use of CBCT in refining the diagnosis of impacted and transposed teeth.2. Defining 3-D treatment plans, surgical access, bond placement and biomechanical

objectives for impacted and transposed teeth.3. Designing and implementing optimal traction auxiliaries for 3-D tooth control while

minimizing damage to neighboring teeth and adverse movements of anchor teeth.

Dr. Sunil Kapila, DDS, MS, PhD

Dr. Sunil Kapila is the Robert W. Browne Professor and Chair of theDepartment of Orthodontics and Pediatric Dentistry at the University ofMichigan. Dr. Kapila obtained his dental education at the University ofNairobi, Kenya. He completed his orthodontic education at the Universityof Oklahoma and his Ph.D. in Oral Biology at the University of CaliforniaSan Francisco. Dr. Kapila has received several awards including the AADR

Hatton Award, the AAO Milo Hellman Award, the B.F. Dewel Honorary Research Award andgave AAO’s 2013 Jacob A. Salzmann Lecture. Dr. Kapila is a Diplomate of the American Board ofOrthodontics. Dr. Kapila’s has published many peer-reviewed papers, chapters and books onTMJ pathobiology, orthodontic biomaterials, and imaging. Dr. Kapila is co-author of the bestselling orthodontic textbook “Current Therapy in Orthodontics.” His new textbook titled “ConeBeam Tomography in Orthodontics: Indications, Insights and Innovations” is expected to bepublished shortly.

Orthodontists are continually searching for treatment techniques that are more effectiveand efficient. The ideal orthodontic bracket system would allow orthodontists to providequality orthodontic care in less time and fewer appointments. Rapidly expandingcomputer technologies have been developed to optimize the design and placement oforthodontic brackets in an attempt to realize these goals. Are these new technologies, the quantum leap to a new generation of orthodontics? This presentation is an evidence-based review of the effectiveness and efficiency of these computer-aided systems.

Dr. Lorne D. Koroluk, DMD, MSD, MS, FCRCD(C)

Dr. Koroluk received his dental degree from the University of Saskatchewanand obtained master’s degrees and certificates in pediatric dentistry fromIndiana University and in orthodontics from the University of Minnesota.He was a faculty member at the University of Saskatchewan, College ofDentistry from 1986-1998. Currently he has a joint appointment in thedepartments of orthodontics and pediatric dentistry at the University of

North Carolina – Chapel Hill, School of Dentistry and is the department of orthodontics InterimDepartment Chair. During this session and panel discussion Dr. Miller will outline the differences andadvantages of the Esprit Spring recently developed by Opal Orthodontics. The advantagesof a hidden spring and distal plunger as well as a comparison of force levels, and its use indirect archwire, XBow and class III correction will be discussed. Overall, the focus will be onimproving patient comfort during the class II correction process.

Dr. Robert Miller, BS, DMD†

Dr. Miller earned his B.S. degree in Zoology from Clemson University in1979 and his DMD degree in 1983 from The Medical University of SouthCarolina. After dental school, Dr. Miller completed an orthodonticresidency at the Medical College of Virginia in 1985, and then served in theUSAF, as Chief of Orthodontics at Clark Air Base in the Philippines for threeyears.

Dr. Miller began practicing in Culpeper in 1988 and became a Diplomate of the American Boardof Orthodontics in 1992. He is a pioneer in the area of orthopedic correctors, having developedthe Flip Lock™ Herbst device in the early 1990's. More recently, he assisted Dr. Duncan Higgins,the inventor of the Crossbow™ device, to broaden its use as an efficient overbite corrector andour most state of the art device used just before braces. Dr. Miller is the author of numerousprofessional publications in orthodontics.

The focus will be on:1. Combining Class II correction with maxillary expansion.2. Distalizing the upper bicuspids and molars only, allowing over-correction of the Class II

buccal segments without over-retracting the upper incisors. Passive upper incisorretraction minimizes root resorption.

3. Allowing the side effects of the Class II springs to rebound before phase two braces.This avoids dealing with the side effects late in treatment with a single phase approach.It also decreases the time in braces and minimizes decalcification.

Duncan has two Canadian patents related to the Crossbow® Class IICorrector, or Xbow®. There are three research articles on Xbow in the Angle Orthodontist, one in the AJODO, and one in the European Journal of Orthodontics. His indirect bonding technique was in Seminars inOrthodontics.

Duncan has lectured internationally on indirect bonding and Xbow to groups including theCAO, Angle Society, PCSO, GLAO, and MASO. He is a regular lecturer at UBC. Duncan is a Fellowof the Royal College of Dentists in Canada.

Class II correction devices are commonly used in orthodontics and exist in manydeclension. Literature reviews show that such devices do not appear to cause anysignificant changes in mandibular length and their effectiveness in correcting class IImalocclusion can be explained by a combination of some skeletal (mainly maxillary) anddentoalveolar (maxillary and mandibular) modifications. The SUS2 corrector device will bepresented using bondable head gear tube and self-ligating mandibular molar tube. A casepresentation will be used to explain how to use the SUS2 device in a successful manner.

Dr. Sylvain Chamberland, DMD, FRCD(c), MSc.

Dr. Chamberland received his DMD degree from Laval University in 1983, a Certificate in Orthodontics from University of Montreal in 1990 and aMaster degree in dental science in 2008 from University Laval where he waspart time clinical instructor and teacher at the undergraduate program ofthe dental faculty from 1991 to 2008. He lectures in several graduateprograms and scientific meetings in the US, Canada and Europe. He is a

published author. Dr. Chamberland is a Diplomate of the American Board of Orthodontics anda Member of d’Angle East Society of Orthodontists.

This presentation will review how orthodontists and surgeons can work together usingappropriate diagnostic and surgical procedures to achieve a successful outcome inadolescent Class II patients. The orthodontist will learn how to identify craniofacialabnormalities that contribute to narrowing of the pharyngeal airway and predispose toadult Obstructive Sleep Apnea. Long term esthetic and functional benefits of correctioncertain types of skeletal outcomes of surgical correction of skeletal deformity will bediscussed.

Dr. Reginald H. B. Goodday, DDS, MSc, FRCD (C), FACD, FICD

Dr. Goodday obtained his MSc in Oral and Maxillofacial Surgery fromDalhousie University and subsequently did a clinical OMFS fellowship inAustralia. He is a full professor and past Chair of the Department of Oraland Maxillofacial Sciences at Dalhousie, past President of the CanadianAssociation of Oral and Maxillofacial Surgeons, and has served as both the Chief Examiner of OMFS and Examiner in Chief for the Royal College of

Dentists of Canada. Dr. Goodday is on active staff at the QEII Health Science Center and the IWK Children’s Hospital in Halifax, Nova Scotia. His research interests include the diagnosis,treatment planning, and appropriate surgical management of Obstructive Sleep Apnea. He has published 4 chapters, several articles and multiple abstracts on this topic.

Being an educator for many years has enabled her to develop aunique perspective on human behavior.

Ms. Fontaine has spent several years developing and delivering courses on the relationshipbetween mind and personal and professional growth at Ecole Polytechnique de Montreal,University de Sherbrooke and University du Quebec. 3:45 pm – 5:00 pm OBSTRUCTIVE SLEEP APNEA IN CHILDREN: HOW TO DETECT AND HOW TO TREAT

Sleep related breathing problems (SRBD) constitute a recently recognized common chronicdisorder in children and adolescents that can range from snoring to obstructive sleepapnea. A compromised airway during sleep can have a dramatic impact on systemic healthand development, including cardio-vascular consequences, delayed growth, concentrationand behavior issues. Most children are treated with excision of tonsils and adenoids, whichhas not been proven to completely abolish apnea events in all cases. Craniofacialanomalies can also contribute to the development of SRBD. Early detection is critical, and proper referral/treatment should be included in the orthodontic evaluation.

Dr. Andrée Montpetit

Dr Andrée Montpetit is a professor at the Orthodontic department of theUniversity of Montreal. She received her dental degree in 2004, andcompleted her Master’s and specialty formation in 2010 at theaforementioned University.

Her Master’s thesis subject was Obstructive Sleep Apnea (OSA) in adults,and she now concentrates her research in the child population. She is currently involved in amulticenter nationwide government funded project to help determine diagnostic craniofacialcriteria that could be indicative of OSA in children. The goal of her research team is to increasepublic awareness of this problem and the possible developmental risks for undiagnosed anduntreated children.

Pauline apporte sa passion pour l’éduction des individus dans le domainede la dentisterie au ‘’Transitions Group North America’’. Elle a bâtit saréputation sur la communication efficace avec les patients et ses collèguesde bureau où elle pratiquait. Elle maitrise les programmes de thérapieparodontale non chirurgicale depuis 1995 et a eu l’opportunité deparrainer en français et en anglais.

Le cabinet privé où elle travaillait a participé à une session de coaching du ‘’Transitions Group North America’’ et ceci l’a inspirée à vouloir aider les autres. Pauline a développé etperfectionné ses compétences en appliquant et en enseignant cette nouvelle méthode. Sapassion de partager ses connaissances est démontrée quotidiennement autant dans sa carrièreprofessionnelle que dans sa vie personnelle

Elle est diplômée du Collège Cambrian dans le programme bilingue d’hygiène dentaire ettravaille dans le domaine dentaire depuis 23 ans. Elle a également perfectionné ses aptitudesd’enseignement en tant qu’instructeur en clinique au collège Cité collégiale à Ottawa.

10h45 – 12h00 L’APNÉE OBSTRUCTIVE DU SOMMEIL CHEZ LES ENFANTS : COMMENT DÉTECTER ET COMMENTTRAITER

Les troubles respiratoires du sommeil (TRS) sont des maladies chroniques chez les enfantset les adolescents, récemment reconnus, qui peuvent varier du ronflement au syndromed’Apnée obstructif du sommeil. L’obstruction des voies respiratoires pendant le sommeilpeut avoir un impact dramatique sur la santé et le développement systémique y comprisdes conséquences cardio-vasculaire, un retard de croissance ainsi que des problèmes deconcentration et de comportement. La plupart des enfants sont traités avec une excisiondes amygdales et adénoïdes malgré que cette action n’a pas démontrée, dans tous les cas,l’abolition complète des épisodes d’apnée. Des anomalies crânio-faciales peuventégalement contribuer au développement de TRS. La détection précoce est essentielle ainsi que la bonne orientation du patient/traitement doit être inclus dans l’évaluationorthodontique.

Dre Andrée Montpetit

Dre Andrée Montpetit est professeur au département d’orthodontie à l’Université de Montréal. Elle a obtenue son diplôme en médecine dentaireen 2004 et a complété sa maitrise et sa formation spécialisée en 2010 àcette même université.

Le sujet de sa thèse de maitrise était le syndrome d’apnée obstructive dusommeil (SAOS) chez les adultes, elle concentre maintenant ses recherches chez les enfants. Elle participe actuellement à un projet multicentrique à l’échelle nationale financé par le gouvernement pour aider à déterminer les critères diagnostiques cranio-faciales qui pourraientêtre indicatifs de l’AOS chez les enfants. Le but de son équipe de recherche est de sensibiliser le public à ce problème et aux risques de développement possible pour les enfants non diagnostiqués et non traités.

Isabelle Fontaine est une conférencière puissante et divertissante reconnuepour sa capacité de toucher et inspirer les gens. Grâce à ses présentationsénergisantes qui sont autant divertissantes qu’éducationnelles, elle dévoileun véritable sens de l’humour avec une approche simple mais profondémentpercutante. Être éducatrice pendant de nombreuses années lui a permis dedévelopper une perspective unique sur le comportement humain.

Mme.
